Corps Permits and Appropriate Ecology Permit
Ecology must find out from the Corps the type of perinit. they will be issuing for your project
before we can determine the appropriate Ecology permit for your Project_ The Corps will issue
one of the following lbur pea-nits for your project:
1) An Individual Section 404 permit(under the Clean Water Act)
2) A Nationwide permit, issued under Section 404
.3). An._Individual Section 10 permit(under the Rivers and Harbors Appropriation Act)
4) A letter of Permission, issued under Section 10
The attachment provides information regarding these lour different types of permits that the
Corps may issue for your project and the corresponding Ecology permit_
Please be aware that a Construction Stonnwater Gene", Permit is required for all soil disturbing
activities (including grading, stuinp removal,demolition) where 1 or more acres will be
disturbed,and stormwater will be discharged to a receiving water directly (e.g., wetlands, creeks,
unnamed creeks, rivers,marine waters, ditches,estuaries), or to storrn drains that discharge to a
receiving water. If all stormwater. is retained on-site and cannot enter surface waters of the state
under any condition,you do not need permit coverage. Construction site operators must apply for
a permit 60 days prior to discharging stormwater. For more information on how to obtain this

7a. Describe the proposed work that needs aquatic permits: Complete plans and specifications should be provided for all work
waterward of the ordinary high water mark or line, including types of equipment to be used. If applying for a shoreline permit,
describe all work within and beyond 200 feet of the ordinary high water mark. If you have provided attached materials to describe
your project, you still must summarize the proposed work here. Attach a separate sheet if additional space is needed.
See drawing S-1 by MWD Consultants for description of the work (attached). The only access to the residence is
across Kristina Creek on a 20' long private bridge. The extreme rainfall event in December 3, 2007 allowed the
creek to undercut (create a void space) 1 foot of the soil that supports the creek side of the east bridge
abutment. This void will be filled with concrete to reduce future erosion. The creek side of both abutments will
also be protected with 1 and 2 man rocks.
PREPARATION OF DRAWINGS: See sample drawings and guidance for completing the drawings. ONE SET OF ORIGINAL OR GOOD QUALITY REPRODUCIBLE
DRAWINGS MUST BE ATTACHED. NOTE: Applicants are encouraged to submit photographs of the project site,but these DO NOT substitute for drawings. THE CORPS
OF ENGINEERS AND COAST GUARD REQUIRE DRAWINGS ON 8-112 X 11 INCH SHEETS. LARGER DRAWINGS MAYBE REQUIRED BY OTHER AGENCIES.
7b. Describe the purpose of the proposed work and why you want or need to perform it at the site. Please explain any specific needs
that have influenced the design.
The purpose of the project is to restore the load carrying capacity of the bridge to its design loads and to reduce
the potential of future erosion of the soil under the bridge abutments.
7c. Describe the potential impacts to characteristic uses of the water body. These uses may include fish and aquatic life,water
quality,water supply, recreation and aesthetics. Identify proposed actions to avoid, minimize, and mitigate detrimental impacts
and provide proper protection of fish and aquatic life. Identify which guidance documents you have used. Attach a separate
sheet if additional space is needed.
Most of the work can be done from the shore. Temporary cofferdams can be installed to protect the fish from the
concrete formwork during construction. The work should be done during the lower water flow seasons like the
summer and scheduled to protect the migrating Salmonids, if appropriate.
When the repair work is completed the project should have no more or less affect on characteristic uses of the
water body (creek) than when the bridge abutment was originally constructed.

PROVISIONS
1. Work below the ordinary high water line shall only occur between February 9, 2008 and March
31, 2008.
2. Work shall be accomplished per plans and specifications approved by the Washington
Department of Fish and Wildlife, entitled Hillius Bridge Abutment Repair and faxed by Mike Dully on
February 5, 2008, except as modified by this Hydraulic Project Approval. A copy of these plans
shall be available on site during construction.
ABUTMENT PROVISIONS
3. Excavation for and placement of material shall be restricted to work necessary to protect the
eroding abutments.
4. Work shall be accomplished in a manner to minimize damage to the streambed and banks.
EQUIPMENT PROVISIONS
5. Equipment used for this project shall operate stationed on bridge or banks.
6. Equipment used for this project may operate below the ordinary high water line, provided the
drive mechanisms (wheels, tracks, tires, etc.) shall not enter or operate below the ordinary high
water line.
7. Equipment used for this project shall be free of external petroleum-based products while working
around the stream. Accumulation of soils or debris shall be removed from the drive mechanisms
(wheels, tires, tracks, etc.) and undercarriage of equipment prior to its working below the ordinary
high water line_ Equipment shall be checked daily for leaks and any necessary repairs shall be
completed prior to commencing work activities along the stream.

WATER QUALITY PROVISIONS
8_ Fresh concrete or concrete by-products shall not be allowed to enter the stream at any time
.during this project. All forms used for concrete shall be completely sealed to prevent the possibility
of fresh concrete from getting into the stream.
9. All waste material such as construction debris, silt, excess dirt or overburden resulting from this
project shall be deposited above the limits of flood water in an upland disposal site.
10. If at any time, as a result of project activities, fish are observed in distress, a fish kill occurs, or
water quality problems develop(including equipment leaks or spills), immediate notification shall be
made to the Washington Military Department's Emergency Management Division at 1-800-258-
5990, and to the Area Habitat Biologist listed below.
11_ If high flow conditions that may cause siltation are encountered during this project, work shall
stop until the flow subsides.

speciricaliy Chapter 77.55 RCW(formerly RCW T7.20). Additional authorization rrom other public agencies may be
necessary for this project. The person(s)to whom this Hydraulic Project Approval is issued is responsible for applying
for and obtaining any additional authorization from other public agencies(local, state and/or lbdoral)that may be
necessary for this project.
This Hydraulic Project Approval shall be available on the job site at all times and all its provisions followed by the
person(s)to whom this Hydraulir.Project Approval is issued and operators)performing the work.
This Hydraulic Project Approval does not authorize trespass.
