Mr Hudson described the problems he had encountered while constructing his home and briefly discussed the legal case between the builder and him. He also let us know that he had accumulated considerable expense in trying to correct construction changes and errors, including re-framing the prow and adding a concrete wall to the stepped foundation wall. Mr.Hudson was given a claim form to use if he decides to file a claim against Mason County. He also demanded that Ms.Adkins tell him of other resources to contact to help him with attorney and contractor concerns. He was told that Labor&Industries would address contractor concerns and he told us that he had already tried that resource. Ms.Adkins let him know that if she or anyone in Community Development becomes aware of a resource that they would let him know. The building plans and documents attached to the plans have been removed from our system as is customary 6 mo.after the final occupancy inspection is granted. Apparently Mr.H has a set of plans but he did not have them during the meeting.do/7.1.2011 Page 5 of 5 CaseActivity..rpt Request To Revise An Approved Plan Permit Number: BLD200-- Name V& Parcel Number 1ZZ - --7'j - ( 0 Phone Number daytime Project Address Mailing Address Please provide a complete, detailed de cription of the proposed revisions to the approved plans: ' l Are two sets of the revised plans or addendum indicating the changes included? 0" es ❑ No Are the approved site plans included? 13- ❑ No Are the revisions clearly and accurately identified on the plans or addendum? The following items are included: 1. Lateral Analysis with Basic Wind Speed = 85 MPH, Exposure C, Seismic Design Category D 2. Vertical Analysis 3. Foundation Design We have provided the designer with a set of plans and 2 copies of the structural calculations for his use in obtaining a building permit for the referenced project. The scope of this agreement covers the design phase only. If site inspections are required by the Building Dept., these will be performed at an additional hourly fee of $100,00 per hour. Also, revisions to the original design will be billed at the hourly rate of $100.00 per hour. To reuse these calculations for another site, please contact MDT Engineering for review and approval. A new cover sheet is required with the new site address and an original wet stamp. Questions about the attached information should be addressed to MDT Engineering. Michelle D.
