There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at 1-800-647; �Th son signing this condition is either the homeowner, agent for the owner or a registered contractor according to WA state law. X 2) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building Department r to any further inspections being performed or approvals granted. X 3) In accordance with the Uniform Building Code, all sites shall have approved numbers or addresses located in such a position as to be plainly visible and legible from the street or road fronting the property. Mason County Building Department requires that this be completed prior to calling for any site inspections. A re-inspection fee based on rates as adopted by the jurisdiction and the Uniform Building Code will be assessed if the owner and/or contractor fail to a address on site prior to requesting inspections. X 4) Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to the Mason County Building Department prior to construction. All engineering and/or architectural documents are a part of the approved set of plans and shall remain attached thereto. If documents are removed, approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and by the Building Department prior to any further inspections being performed or approvals granted. X BLD2003-01673 Please referto the following pages for conditions of this permit. 2 of 4 5) 1997 UBC Chapter 17, Section 1701. In addition to the inspections required by UBC Section 108, the owner or the engineer or architect of record acting as the owner's agent shall employ one or more special inspectors who shall provide inspections during construction on the types of work listed under UBC Section 1701.5. The special inspectors' duties & responsibilities shall be as specified in 1701.2 and 1701.3. The engineer of record, Westsound Engineering, or their authorized agent shall perform special inspection for the following. Special Inspection reports shall be submitted the the Mason County Building Department prior to the framing inspection. 1) Epoxy anchoring system installed and reported as specified in applicable ICC/ICBO Evaluation report. See pages 1/19 through 3/19 of the approved plans for additional information. 2) Installation bracing system designed by engineer of record for the north, south, east and west walls. See page S1 and pages 1/19 through 11/19 of the approved building plans for additional information. 3) Perform inspections of the four (4) items listed in the letter prepared by Mr. Craig Baldwin, Westsound Engineering, Inc. dated February 6, 2004. See page 13/19 of the approved building plans. X 6) All construction must meet or exceed all local ordinances and the 1997 Uniform Building Code requirements as adopted and amended by Mason County and the State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in permi / ocation.( 7) All changes to"approved" building plans that effect compliance with the Uniform Codes as amended and adopted, or any other Mason County ordinance or regulation be reviewed and approved by Mason County prior to construction. X 8) The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the Uniform Codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building Inspector shall b de prior to requesting additional inspections. X 9) All property lines shall be clearly identified at the time of foundation inspection. X 10) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with Mason County r ces and building regulations. X Y 11) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit holder have prev ing taken. No more than one extension may be granted. If additional work is required a separate building permit and approval will be required before work is performed. Refer to the list attached to the approved building plans that identifies exempt and non-exe work. X 14) The Uniform Fire Code requires a fire apparatus access road for every facility, building, or portion of a building that is more than 150'from an approved access road. Roads are required to meet the minimum Mason County Fire Marshal standards for Fire Apparatus Access Roads up to the point where such roads connect ith a c maintained public road or to another fire apparatus access road which connects to a county maintained public road. This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. The owners name is Bob Saenz and Kelly Rohr. The design includes reinforcement of the existing north, south, east and west walls including steel rods, connection details, and a "hammer-in adhesive system" to be used with specified holdowns. Please provide the ICBO/ICC code report for the adhesive system so that we can review the installation requirements and make comments accordingly. Our office has tried to locate the report through resources available. If a code report is not available please propose an alternate system. In addition Mason County will require a final inspection by the engineer of record to inspect installation of components specified along with a statement by the engineer that the structure is safe to occupy. In order to complete the building department review additional information will be needed which is listed below. PLAN REVIEW REQUIREMENT COMMENTS: I. The structure involved in the proposed repair was declared a dangerous building by Mason County on September 20,2001 based upon findings specified in a"Review of Structural Conditions"prepared by Mr.Robert L Morrison,Jr.P.E. A copy of the report is enclosed. The report included recommendations and concerns about the structural condition of the foundation, log walls,and floor connections. The proposed building permit identified above requests approval for repair of the foundation only. Most structural concerns specified in the report prepared by Mr. Morrison are not addressed in the permit application and the"Structural Design Report"prepared by Westsound Engineering,Inc,dated October 24,2003.
