3.) Per ction 901.2.1 of the 2012 International Fire code, Statement of Compliance.
Before requesting final approval of the installation, the installing contractor shall furnish a written statement to the fire code official that the subject fire
protection system has been installed In accordance with the approved plans and has been tested in accordance with the manufactures specifications and
the appropriate installation standard. Any deviations from the design standards shall be noted and copies of the approvals for such deviations shall be

4.) This approved for the installation of an addressable fire alarm system installed in accordance with NFPA 72 and monitored by the UL certified
monitoring company specified in the permit application, Alarm Center.
FINAL INSPECTION APPROVAL is subject to the following requirements:
1. Provide smoke detectors in accordance with NFPA 72 to address spacing of a peaked ceiling in all areas.
2. Correct as needed, strobe light locations and glu-lam visual obstructions.
3. Provide corrected, updated plans to the Mason County Building Department. Deliver to 426 West Cedar St, Shelton,WA 98584,
4. Provide corrected, updated plans onsite during for inspection.

Fire Systems West Inc. will provide labor, parts, permits, programming and testing for the fire
alarm system at the above referenced building for the amount of $9,803.00 plus applicable
state and local taxes. Price includes only that described in the attached scope of work.
All work will be performed in accordance with, and the approval of the local AHJ.
Line Item #1: Fire Systems West, Inc will also provide UL listed central station monitoring for
free the first year and every year thereafter will be an annual fee of $360.00. A separate
monitoring contract will need to be signed for this service. Initials..

1. Provide labor, parts, programming and testing to install a new fire alarm system.
2. Provide design and CAD of fire alarm system.
3. Provide fire alarm permit and submittals with AHJ.
4. Provide testing of fire alarm system with AHJ.
CLARIFICATIONS
A) Fire Systems West will install a new fire alarm panel and devices.
B) This proposal is based upon a verbal and written direction from Karl. No revised drawings
were given at this point,
C) This proposal is based upon no attic space or drop ceilings.
D) This proposal is based on the fire alarm cabling able to be run "free air" and not in conduit.
E) Any changes by the AHJ or that are requested by the owner that are outside of this
proposal will require a signed change order in order for that work to proceed.
F) This proposal is good for 30 days from the date of this proposal.
G) All work will be performed during normal business hours, Mon - Fri, 7:00am to 4:00pm.
H) CAD files must be provided to Fire Systems West at no charge for submittals. If CAD
drawings are not provided we will need a signed change order in order to create them.
1) Plan review is subject to the speed of the local authority, usually less than 3 weeks.
EXCLUSIONS
A) We are a low voltage electrical contractor, therefore all high voltage work is excluded.
B) EMT and or conduit. All EMT and or conduit shall be supplied and installed by others.
C) Any extra fire alarm devices not listed above.
D) Any trenching or underground work or Core drilling if necessary.
E) Any additional equipment or devices required by AHJ or Owner.
F) Installation of duct detectors or smoke dampers. (As per Larry Waters, Inspector)
G) Prevailing wage and or certified payroll.
H) Off hours and taxes.
