Good Morning,
At the request of the contractor a site visit was conducted on Friday(6/24)to discuss required setback from the onsite
stream.There was concern that the in-place stakes marking/identifying the stream buffer setback were not measuring
75' has required by the approved site plan provided by the Mason County Planning Department.
During my site visit I walked the edge of the stream adjacent to the house placement and took measurements from a
few different locations.The measurement taken from the edge of the stream to the stream buffer setback stakes along
the south end of the house were measuring around 62'. Placing the edge of the house approximately 77'from the
stream. Based on my conversations with the Planning Department and reviewing the approved site plan the required
setback from house to the stream is a minimum of 90' (75' stream buffer+ 15' stream buffer setback).Therefore,at this
time the required setback is not met or achieved.
Do not place concrete at this time until setback requirements are addressed.The previously approved/passed setback
inspection has been revoked due to the issues noted above. Please review the approved site plan and ensure all
required setbacks are met (property lines and septic system). If you have any questions regarding this matter, please
feel free to contact me. However, if you have questions regarding required setback or would like a second opinion,
please contact the Planner for your area Scott Ruedy.

clearly marked identified. Therefore unable to verify streamline setbacks are acheived.
Please mark house foot print and/or dig and install house footings.
2) Please provide how compilance with setbacks from ascending and decending slopes
adjacant to the structure are being acheived. Complaince was unable to be verified due
to lack of house footprint being clear identified.
3) Appears fill dirt has been added to building site. Please provide compaction reports
for all in-place fill meets 95% compaction requirements.
4) Property lines not clearly identified, please address.
5) all permit documents must be onsite (plans, site plan, permit card, etc).

Jamie Hicks is a Geologist and an ICC certified Special Inspector. Jamie has preformed
numerous Geotechnical investigations throughout Western Washington. He has
landslide evaluation and field investigation experience. His Special Inspection
experience includes reinforced concrete, structural masonry, structural steel and
bolting, concrete post-tensioning, and proprietary-anchors. He also has performed
numerous construction quality control / quality assurance inspections on various
projects.
Jamie has served as a lab manager for a soils, concrete, aggregate, asphalt, and
engineering laboratory. He has started and ran mobile/onsite field laboratories on
various projects throughout Western Washington. Jamie's extensive soils background
and major in Geology provide for an excellent resource when faced with challenging
soils issues or a complex site with various soil types.
Jamie is a certified Special Inspector with an extensive background in concrete testing
both in the lab and in the field. Jamie has served as senior inspector on a number of
projects that have used Washington State Department of Transportation, Federal
Highways, Federal Aviation Administration, or US Army Corps of Engineers
specifications.
As a Special Inspector Jamie has shown proficiency at reading engineered drawings and
plans for the placement of reinforcing steel for reinforced concrete and masonry,

aggregate analysis for use in concrete.
Jamie's varied experience with all types of concrete placement has helped him train
other inspectors in the variations that may occur while testing in the field. This valuable
experience can then be passed on to less experienced inspectors to assist them.

Puyallup,WA 2009-2011
Project consisted of the construction of a new school building and church steeple for the Seventh Day
Adventists Church. Project included a period of pre-loading and monitoring of the site. Inspections
performed included reinforced concrete, lateral wood framing inspections, proprietary anchors,
structural steel bolting and welding,and in-place densities on soils and asphalt
Western Washington Regional Headquarters Seventh Day Adventist,
Federal Way,WA 2006-2007
Project consisted of the construction of a new 2 story regional headquarters office for the Seventh
Day Adventists Church. Project included removal of unsuitable soils from the site. Inspections
performed included reinforced concrete,cold formed steel framing inspections,proprietary anchors,
structural steel bolting and welding,and in-place densities on soils and asphalt
Ft.Lewis Tank Turning Pads, US Army Corps of Engineers,
Ft. Lewis,WA 2006-2007
Project consisted of removal of existing tank turning pads on 4 different firing ranges and replacing
them with new more durable concrete pads. Projects were performed around live firing ranges
which required extremely accurate communication with the firing range patrol during placements
and when retrieving cylinders. Project used Army Corps of Engineers Specifications and required
slump testing each truck delivered to the project site prior to placement. Project consisted of placing
12-18 inch slabs on grade using paving concrete for the tanks to turn while on the firing range.

Ft.Lewis Main-Post Fire Station, US Army Corps of Engineers,

Project consisted of the construction of a Fire Station on Main Post of Ft.Lewis.Project was a Design-
Build. Service included the all geo-technical investigations and design, all special inspections, and
serve as construction quality control. Inspections performed included reinforced concrete, structural
masonry, cold formed steel framing inspections, proprietary anchors, structural steel bolting and
welding,and in-place densities on soils and asphalt
McChord AFB-Taxiway C and H, US Air Force,
McChord AFB,WA 2005-2006
Project consisted of removal and replacement of the existing taxi way. Concrete was placed using a
concrete paver and a Bidwell bridge deck paving machine. Concrete placement was monitored for
excessive edge slump and proper consolidation and finishing. Concrete testing was done in
accordance with Federal Aviation requirements. Project included asphalt paving and soils work
however the vast majority of the materials testing was concrete paving. During concrete paving
concrete beam samples and concrete cylinders were required. Another requirement was periodic
inspection of the batch plant during paving operations which required the collection of aggregate
samples for later testing.
Hawks Prairie Reclaimed Wastewater Satellite, Lott Alliance,
Lacey,WA 2005-2006
Project consisted of the construction of a new Wastewater Reclamation Facility. Project consisted of
at least 10 different sub-surface tanks and chambers all cast in-place for the treatment of Wastewater.
Construction consisted of below grade tanks with all administration and office facilities built above
the tanks. Inspections performed included reinforced concrete, structural masonry, proprietary
anchors, structural steel bolting and welding, and in-place densities on soils and asphalt Another
requirement was periodic inspection of the batch plant during concrete placement operations which
required the collection of aggregate samples for later testing.
4th/5th Avenue Bridge, City of Olympia,
Olympia,WA 2002-2004
Project included the complete replacement of the earthquake damaged original bridge. Bridge
construction was also combined with the re-routing of the original roadway to accommodate current
traffic demand. Project includes Soldier Pile Walls, grouted tie back anchors, deep drilled
foundations, decorative concrete paving of city streets and cross walks, bridge piers, and bridge
deck. Bridge was also post tensioned and required inspection of the stressing of the tendons
placement of all reinforcing steel, welding of reinforcing steel, welding and NDT testing of driven
piling for bridge bent, and compressive strength testing of bonded post tensioning grout along with
flow rate,mud balance,and schupak testing of grout. Entire project required in excess of 10,000 cubic
yards of concrete and had various artistic colored concrete pours throughout the project site. The
project was tested using WSDOT specifications throughout and was Federally Funded through
FEMA Earthquake relief grants. Project was federally audited and was found to be in compliance
with WSDOT and Federal Requirements for testing frequencies.

" Installation of heating equipment in a single-family residence shall meet the requirements of the current IECC/WSEC
R403, applicable sections of the IRC, and IMC.
Heating equipment shall be sized in accordance to ICC/WSEC, Section R403.7. Heating and design load calculations for the
purpose of sizing HVAC systems are required and shall be calculated in accordance with accepted practice, including
infiltration and ventilation. Design calculations shall be available for inspection during inspection.
Referencing IRC M1601.4, all ducts, air handlers, filter boxes, and building cavities shall be sealed. All joints of duct
systems and seams shall be made substantially air tight by means of tapes, mastics, liquid sealants, gasketing or other
approved closure systems. Closure systems used with rigid fibrous glass ducts shall comply with UL181A and shall be
marked 181A-P for pressure-sensitive tape, 181A-M for mastic or 181 A-H for heat-sensitive tape. Closure systems used
with flexible air ducts and flexible air connectors shall comply with UL181 B and shall be marked 181 B-FX for pressure-
sensitive tape or 181 B-M for mastic. Duct connections to flanges of air distribution system equipment or sheet metal fittings
shall be mechanically fastened. Mechanical fasteners for use with flexible nonmetallic air ducts shall comply with UL 181 B
and shall be marked 181 B-C. Crimp joints for round metal ducts shall have a contact lap of at least 1-1/2 inches (38 mm)
and shall be mechanically fastened by means of at least three sheet-metal screws or rivets equally spaced around the joint.
Closure systems used to seal metal ductwork shall be installed in accordance with the manufacturer's installation
instructions.
Duct tape is NOT permitted as a sealant on any ducts. When ducts are located in unheated spaces the ducts hall be
insulated to R-8
DUCT TIGHTNESS TESTING shall be conducted by person(s)trained to perform such testing. A signed affidavit
documenting test results in accordance to IECC/WSEC Section R403.3.3 shall be provided to the Mason County Building
Department prior to the final occupancy inspection. Affidavit forms are available on at the WSU-Energy Program website

where the furnace is located, a utility room, or an approved location inside the building. When located on the elctrical panel,
the certificate shall not cover or obstruct the visibility of the circuit directory label, service disconnect labek, or toher reqyired
Iabels.The certificate shall list the predominant R-values of insulation installed in or on ceiling/roof,walls, foundation (slab,
basement wall,crawlspace wall and/or floor), and ducts outside the conditioned spaces; U-factors for fenestration; and the
solar heat gain coefficient(SHGC)of fenestration. Where there is more than one value for each component, the certificate
shall list the value covering the largest area. The certificate shall list the type and efficiency of heating, cooling, and service
water heating equipment, duct leakage rates including test conditions as specified in WSEC Section R401.3 & R402.4.1.2,

Building envelope air leakage control shall be considered acceptable when tested to have an air leakage less than 5 air
changes per hour when tested with a blower door in accordanve with IECC/.WSEC Section R402.4.1.2.
The blower door test results shall be recorded on the permanent certificate required located near the electrical distribution
panel. Air leakage testing is not required for additions less than 500 square feet. Reference IECC/WSEC R402.4.1.2.
Compliance certificates are available online at the WSU Energy program website titled, "WSEC 2015 Certificate"and are

* A separate inspection in addition to the inspections required in the IBC, Section 110 shall be required in accordance with
