Description of Proposal: This SEPA is associated with a Class IV General FPA to clear 2
acres of a 10-acre parcel for a home site. The area under this
permit is shown as Unit 1 on the attached site plan. Unit 1 has a
150'buffer from Courtney Creek. The unit also contains step
slopes for which a geo-technical report was prepared by
Envirotech Engineering and the setbacks for logging and home
construction are addressed in Section 5.5. The geo-report must
be determined to be complete by Michael MacSems of the
Planning Dept. before the FPA can be issued. No other issues.

CONDITIONS OF THIS DETERMINATION
1) ARCHAEOLOGICAL RESOURCES
Archaeological or historical resources, such as ruins, sites, buildings, artifacts, fossils, or
other objects of antiquity that may have significance from an historical or scientific
standpoint, which may be encountered by the developer/contractor, shall not be further
disturbed. If the developer/contractor encounters any such artifacts, he/she shall
immediately stop work and notify Mason County and the Washington State Department of
Archaeology and Historic Preservation. Failure to do so may result in civil penalties.
2) HOURS OF OPERATION
Noise associated with this application shall not exceed what is allowed under Mason
County Code 9.36. Harvest operations shall start no earlier than 7.00 AM and shall end
before 8.00 PM on a daily basis.
3) GEO-TECHNICAL
A geological report has been prepared by Envirotech Engineering and the submittal
checklist has been reviewed and found complete by Michael MacSems of Mason County
Community Services Department. The applicant shall follow the recommendations of the
sections 5.5 concerning a vegetation buffer for steep slopes.
4) SLASH ABATEMENT
This Forest Practice Application indicates that forest debris resulting from the proposed
logging operation may create an extreme fire hazard under RCW 76.04.600 and WAC
332-24-650. An extreme fire hazard could be created if one or more of the conditions found
in WAC 332-24-650 is located within 100 feet of the running surface of a public road; or is
within 500 of an existing structure and/or designated public use area, which is not owned
by the owner of the fire hazard area.
The land owner is fully liable in the event of fire, if he or she fails to appropriately abate the
hazard. Extreme fire hazards shall be abated by removing the forest debris, or by other
means approved by the Washington State Department of Natural Resources (DNR).

5) STORM WATER & EROSION CONTROL
Escape of silty water or mud from this property shall be considered a violation of County
and State storm water management regulations, and could subject the applicant(s) to civil
fines or penalties. This project is represented as disturbing less than one-acre of ground
and creating less than 5,000 sq. ft. of new impervious surfaces. The contractor shall
comply with the 2005 WSDOE Stormwater Manual compliant 12-point best management
practices narrative written by Ron Godwin of Evergreen Forest Resources for this project.
6) FISH & WILDLIFE HABITAT BUFFERS
This site is subject to a 150' buffer from Courtney Creek which defines the north/northeast
edge of the unit. Buffer has been flagged. Future development is subject to an additional
15' setback from the buffer.

Governmental agencies use this checklist to help determine whether the environmental impacts of you
proposal are significant. This information is also helpful to determine if available avoidance, minimization
or compensatory mitigation measures will address the probable significant impacts or if an environmental
impact statement will be prepared to further analyze the proposal.

description.) This is a timber removal project. All the approved merchantable timber will
be removed from approximately 2 acres of the 10+ acres owned.

2. Air
a. What types of emissions to the air would result from the proposal during construction,
operation, and maintenance when the project is completed? If any, generally describe and
give approximate quantities if known. An occasional exhaust emission will occur when
equipment is utilized.
b. Are there any off-site sources of emissions or odor that may affect your proposal? If so,
generally describe: No known off-site emissions will affect this project.
c. Proposed measures to reduce or control emissions or other impacts to air, if any: None are
required
3. Water
a. Surface Water.
1) Is there any surface water body on or in the immediate vicinity of the site (including
year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe
type and provide names. If appropriate, state what stream or river it flows into. There
are no streams located within the cutting boundaries of this project. However,
Courtney Creek, a fish bearing stream, is located 200 feet north of the northern
cutting line.
2) Will the project require any work over, in, or adjacent to (within 200 feet) the described
waters? If yes, please describe and attach available plans. No work will be conducted
near any stream.

3) Estimate the amount of fill and dredge material that would be placed in or removed
from surface water or wetlands and indicate the area of the site that would be affected.
Indicate the source of fill material. None will occur for this project

withdrawn from the well. Will water be discharged to groundwater? Give general
description, purpose, and approximate quantities if known. No ground water withdrawal
or discharge will occur.
i Describe waste material that will be discharged into the ground from septic tanks or
other sources, if any (for example: Domestic sewage; industrial, containing the
following chemicals. . .; agricultural; etc.). Describe the general size of the system, the
number of such systems, the number of houses to be served (if applicable), or the
number of animals or humans the system(s) are expected to serve. No material
discharge will occur on this project.
c. Water runoff (including stormwater):
1) Describe the source of runoff(including storm water) and method of collection
and disposal, if any (include quantities, if known). Where will this water flow?
Will this water flow into other waters? If so, describe. Storm water might occur during
heavy rains, which will sheet flow on to the forest floor.

waste materials will enter the waters during this project.

so, describe. Some forest floor could be disturbed by tree removal activity. Every
effort will be made to place this property back into satisfactory condition.

pattern impacts, if any: This project will not operate during heavy rain storms, storm water,
if any, will be channeled onto the forest floor.

If so, describe: An oil spill could occur, if a hydraulic hose should fail. However, all
equipment contains a spill kit to handle such emergencies.

and design. This includes underground hazardous liquid and gas transmission pipelines
located within the project area and in the vicinity. None are known at this time

throughout this area, but none will affect this project.

short-term or a long-term basis (for example: traffic, construction, operation, other)? Indi-
cate what hours noise would come from the site. The only noise generated by this project
will be running and working logging equipment engines.

until 7:00 AM and end around 5:00 PM

8. Land and shoreline use

land uses on nearby or adjacent properties? If so, describe. Current land use is 5-acre
home sites. Most neighbors enjoy the forest and pasture lands they have created.
b. Has the project site been used as working farmlands or working forest lands? If so, describe.
How much agricultural or forest land of long-term commercial significance will be converted to
other uses as a result of the proposal, if any? If resource lands have not been designated,
how many acres in farmland or forest land tax status will be converted to nonfarm or
nonforest use? These 5-acre tracts have mostly been hobby farms, some forest land
management and some pastureland management. But none have been used to bring
in a steady income.
1) Will the proposal affect or be affected by surrounding working farm or forest land normal
business operations, such as oversize equipment access. the application of pesticides,
tilling, and harvesting? If so, how: This project will have very little impact on the
neighboring property uses.
c. Describe any structures on the site. There are no dwellings or structures located on this
property.
d. Will any structures be demolished? If so, what? No demolishing will occur under this
permit.
e. What is the current zoning classification of the site? Rural Residential 5 (1 unit/5 ac)

project is being handled separately under a different permit.

public transit stops in the immediate area.

equipment is utilized.

generally describe: No known off site emissions will effect this project.

3. Water
a. Surface Water.
1) Is there any surface water body on or in the immediate vicinity of the site (including
year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe
type and provide names. If appropriate, state what stream or river it flows into. There
are two streams, one is a fish stream (Courtney Creek), the other is a small non-
fish seasonal stream that meanders through the parcel. Both streams flow on to
neighboring property.
2) Will the project require any work over, in, or adjacent to (within 200 feet) the described
waters? If yes, please describe and attach available plans. The tree removal will occur
outside a 75-foot buffer adjacent to the non-fish stream. We will retain all the
timber with in the 175-foot buffer required on Courtney Creek.

3) Estimate the amount of fill and dredge material that would be placed in or removed
from surface water or wetlands and indicate the area of the site that would be affected.

or discharge will occur.
2) Describe waste material that will be discharged into the ground from septic tanks or
other sources, if any (for example: Domestic sewage; industrial, containing the
following chemicals. . . ; agricultural; etc.). Describe the general size of the system, the
number of such systems, the number of houses to be served (if applicable), or the
number of animals or humans the system(s) are expected to serve. No material
discharge will occur on this project.
c. Water runoff(including stormwater):
1) Describe the source of runoff(including storm water) and method of collection
and disposal, if any (include quantities, if known). Where will this water flow?
Will this water flow into other waters? If so, describe. Storm water might occur during
heavy rains, which will sheet flow on to the forest floor.

waste materials will enter the waters during this project.

so, describe. Some forest floor could be disturbed by tree removal activity. Every
effort will be made to place this property back into satisfactory condition.

if any, will be channeled onto the forest floor.

equipment contains a spill kit to handle such emergencies.

located within the project area and in the vicinity. None are known at this time

throughout this area, but none will effect this project.

will be running and working logging equipment engines.

home sites. Most neighbors enjoy the forest and pasture lands they have created.

How much agricultural or forest land of long-term commercial significance will be converted to
other uses as a result of the proposal, if any? If resource lands have not been designated,
how many acres in farmland or forest land tax status will be converted to nonfarm or
nonforest use? These 5 acre tracts have mostly been hobby farms, some forest land
management and some pastureland management. But none have been used to bring
in a steady income.

neighboring property uses.
