Page 1 of 4 CASE NOTES FOR BLD2013-00204 t CONDITIONS FOR BLD2013-00204 1) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at 1-800-647- 982. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to WA state law. X � 2) Ow ne A ent is responsible to post the assigned address and/or purchase and post private road signs in accordance with Mason County Title 14.28. X �-� 3) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building Departr�}eyrior to any further inspections being performed or approvals granted. X 8) REQUIREMENTS FOR ROOF COVERINGS. Roof coverings shall be applied in accordance with the applicable provisions of the current code and the manufacturer's installation instructions. Page 2 of 4 9) Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to the Mason County Building Department prior to construction. All engineering and/or architectural documents are a part of the approved set of plans and shall remain attached thereto. If documents are removed, approval will not be granted., In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and shall be collected by the Building Department prior to any further inspections being performed or approvals granted. X f -7 - 10) All construction must meet or exceed all local ordinances and the international codes requirements as adopted and amended by Mason County and the State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in permit re�Ac a ion. X 11) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements of the stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose. For further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or access connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450. For any construction which is proposed to be located within 25' of a Mason County road right of way, it is suggested to contact that office to review future planned work which may affect your project. 12) Place�nOt of structure must comply with standards set forth per the international codes regarding descending and/or ascending slopes. X 13) All changes to"approved" building plans that effect compliance with the international codes as amended and adopted, or any other Mason County Xrdinanc or regulation, must be reviewed and approved by Mason County prior to construction. 14) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building IInspe shall be made prior to requesting additional inspections. 15) All property lines shall be clearly identified at the time of foundation inspection. X 16) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with Mason Co�ordinances and building regulations. Page 3 of 4 f) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit (olde�e prevented action from being taken. No more than one extension may be granted. 18) Pressure treated wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fasteners, connectors, and flashing. Install metal connectors approved for contact with the new types of pressure treated material. X � - 19) Retaining walls needed to support a surch ge such as structures, roads, or to support slopes, shall require a separate building permit and approval prior to construction of the retaining wall. X . 20) By definition, propane tanks and heatpumps are structures, which must meet setback conditions. Please check your"Approved Site Plan"to ensure these structures meet the setback conditions listed. x 21) Approved per dimensions and setbacks on submitted site plan for proposed garage and DDR2013-00020; includes wetland replanting per MEP2 13-00016. Setbacks are measured from the furthest projection of the structure. X � 22) The proposed project must be consistent with all applicable policies and other provisions of the Shoreline Management Act, its rules, and the Mason County Shoreline Master Program. X 23) All construction and demolition debris must be removed from the site after project completion. Propgr disposal of construction debris must be on land in such a manner that debris cannot enter or cause water quality degradation of State waters. X This permit becomes null and void if work or construction authorized is not commenced within 180 days, or if construction or work is suspended for a period of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of continuation of work is by means of a progress inspection.The owner or the agent on the owners behalf, represents that the information provided is accurate and grants employees of Mawn County access to the above described property and structure for review and inspection. To By Updated Updated Byy BLDC100 Inspection 1/16/2014 None DONE LAW 1/16/2014 LAW I met with the owner on site to look at the foundation:She stated that there were cracks in the fondation an there were pockets where there was no concrete,she alsckconcerned about if the foundation was level or square. There no cracks in the foundation but there are pockets,it appears the contractor did not properly consolidate the concrete when it was poured per section R404.1.2.3.5 of the 2012 International National Residential Code.The forms have not been fully stripped from the foundation,the only pocket that I could see was in the front right Laterial Restraint Panel right at the hold down,The concrete did not fully consolidate around the required hold down at the panel.There may be more pockets,the forms will need to be removed to verify. Also noted was the founation is note level from front to back it is about 4 to 5 inches out of level.And the top of the wall fron side to side is 1/2 inch out of level,section R403.1.5 of the 2012 code allows the base of the of the foundation to slope a maxumum of 1/10,the top of the foundation needs to be level.If the forms are not set level it is standard practice for the contractor to snap lines inside the forms that are level to pour to,it appears that is not the case for this site. The wood framing can not be shimmed 4 or 5 inches to make up for the difference,an engineer will need to evaluate the condition of the foundation for what it would take to make the foundation code compliant. Box 1959 Silverdale WA 98383 RE: Review of Garage Building Permit BLD2013-00204 on Parcel Number;,12220-50-17003 Dear Mr. and Mrs. Root: This letter is regarding the review of the building permit for a proposed garage by Jeff and Chris Cizek on the subject property at 200 E North Bay Rd. Since the proposed location of this garage is within the vegetation buffer of the category 3 wetland to the north, you must submit to the Permit Assistance Center a completed Mason Environmental Permit application to continue the review of your building permit. Staff recently observed in February 2013 that native plant re-vegetation of the adjacent wetland buffer was not carried out. You need to promptly complete this permit condition, which was part of the approval for BLD2007-01885 for the residence now completed on the property. These replanting actions will complete the commitments agreed to in the construction of the residence in the wetland and shoreline buffer area on the property. You may use the re-vegetation plan from the wetland reports from 2007 to 2009 as the site plan for the new Mason Environmental Permit for this new structure; this site plan was included in the enclosed 2007 MEP. You shall implement this wetland buffer planting project to complete the old permit condition. The objective is to have the plants survive, and you or the new owner be responsible to monitor the shrubs or trees that have been planted and report for three years to Mason County the success of growth of the plants by letter or site visit.
