9) In addition to the inspections required in IBC, Section 109, the owner, the engine or arc itect of record acting as the owner's agent shall employ
one or more special inspectors who shall provide inspections during construction on the types of work listed under Chapter 17 and as specified by
the design professional. The special inspectors duties & responsibilities shall be as specified in Chapter 17.
Special inspection reports shall be submitted to the Mason County Building Department, PO Box 186, Shelton WA 98584 and available for
inspec " I pection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing and

10) Any changes in construction shall be reviewed by engineer of record and submitted in writing to the Mason County Building Department prior to
construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are

11) ALL CONSTRUCTION MUST MEET OR EXCEED ALL LOCAL CODES AND THE INTERNATIONAL CODE REQUIREMENTS AND OCCUPANCY
IS LIMITED TO THE PERMITTED AND APPROVED CLASSIFICATION. ANY CHANGE OF USE OR OCCUPANCY WOULD RESULT IN PERMIT
REVOCATION. CHANGE OF USE MUST BE APPROVED PRIOR TO CHANGE. x

13) ONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE
ADOPTED BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in
confor n ;with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a
on y Building Inspector shall be made prior to requesting additional inspections.

17) The Applicant shall be responsible for the,ollowing elements:
1) Compliance with the Construction Storm Water Plan (SWPPP No. WAG-50-0064)submitted on March 7, 2013, shall be subject to an on site
inspection, or inspections, by the engineer of record or an authorized representative. Said inspection(s) shall be performed in accordance with the
approved stormwater plan, and prior to any modification that would make a determination of compliance impossible.
2) The SWPPP shall be implemented as detailed in the WAG-50-0064 document; operations shall comply with the March 7, 2013 site plan, the
stormwater monitoring plan, the erosion &sediment control best management practices, and the oil and fuels spill plan procedures for prevention,
response and reporting.
3) Inspect" reports shall be submitted to verify all engineering requirements have been completed in accordance with the approved storm water
plan. o all be submitted to the Mason County Department of Community Development prior to each corresponding inspection and final
per it val. Copies of all special inspection reports shall be made available at time of inspection.
18) All construction and demolition debris must be removed from the work area after project completion. Proper pos ction debris must
be on land in such a manner that debris cannot enter or cause water quality degradation of State waters.
19) Prior to final approval, all upland areas disturbed or new 'cre -bby
activities shall be seeded, egetated or given an equivalent type
of erosion protection (silt fencing or straw matting)

20) Temporary erosion control measures must be implem ted to prevent water quality degradation of adrce �fs'or properties. Silt fencing or
straw matting must be installed and maintained until upland vegetation has become established -X

PURPOSE OF SPECIAL INSPECTION
Special inspection Is the monitoring of materials and workmanship which are critical to the Integrity of the building
structure to assure that the approved plans and specifications are being followed and that relevant Codes are being
observed. : pedal Inspections are in addition to those conducted by the county building inspector and by the
engineer or architect of record. Special inspectors furnish inspection at all times that their presence is required by
the code,the plans and specifications,and the enforcing jurisdiction. Good communication between the special
inspector and thi;designer,contractor,and building department is essential.
DUTIES AND RESPONSIBILITIES OF THE SPECIAL INSPECTOR:
Special Inspectors are individuals with highly developed,specialized skills who observe those critical building or
structural features which they are qualified to inspect. Duties of special inspectors and/or inspection agencies
include the following:

To correlate reports to the construction plans, the following designations are used:
Silo pad — main central silo foundation
Scale piers —scale foundations; N = north of silo pad, S = south of silo pad;
(N1-N3, S1-S3) #1 closest to silo pad.
Washout slabs — (NW1-NW3, SW1-SW3) #1 closest to silo pad.
Approach slabs (North, South)
The project site was already excavated for
formwork. Each pit was cleanly dug and filled with ballast
in preparation for concrete pouring. Formwork was

erected for scale pier S1 and the majority of the silo pad.
The ballast had obvious compaction markings and proved
well compacted with a probe test. (Figure 1) ,; {
The subgrade below the fill was inspected and met j' - ,
the specifications required by the geotechnical documents

proper depths and materials were reached. The subgrade
material delineation is obvious upon visual inspection.

Matt was on site and walked the project during the �I
inspection. The workers were finishing the formwork and
guides for ensuring a level slab. The remaining piers
formwork had been constructed since the site visit on
3/6/13. The majority of the reinforcing steel had been
placed in the forms for the concrete pour on Saturday. The
silo pad and each pier was inspected for proper spacing of
rebar, rebar size and grade, and lengths. The silo pad and
each pier was also inspected for proper depth, length and
width. Unless stated in this report, all measurements met Figure 1:Reinforcing steel
the specifications in the plans.
As shown in figure 1, the reinforcing steel is #6 A615 rebar. Each size of
rebar was inspected to ensure compliance with the plans. All washout slabs and the
approach slabs were not formed or laid with reinforcing steel yet, they will be a later
inspection.
The main silo slab had only three formwork walls constructed for ease of steel
installation, the fourth wall will be installed prior to concrete pour. The measurements
for the silo slab were accurate, and the markings on the formwork for the fourth wall
were accurate as well. (The fully constructed form will be measured prior to concrete
pour to verify correct dimensions.)
The steel reinforcement of the silo slab was changed :.
per request to add #3 bar perpendicular to the plan design
to PP provide support for the design bar. The pier S1 was
P 9 measured and found to be V/2" short of the specified depth.
The change in pier depth will change the factor of safety
from 3.16 to 2.85 (1.5 min. required) and so will be allowed.
Finally, the anchor bolts to be used in the weld-plate design
were inspected to ensure they met the plan specifications.

First inspection of project site.
To correlate reports to the construction plans, the following designations are used.
Silo pad — main central silo foundation
Scale piers —scale foundations; N = north of silo pad, S = south of silo pad;
(N1-N3, S1-S3) #1 closest to silo pad.
Washout slabs — (NW1-NW3, SW1-SW3) #1 closest to silo pad.
Approach slabs (North, South)
The project site was already excavated for
formwork. Each pit was cleanly dug and filled with ballast
in preparation for concrete pouring. Formwork was
erected for scale pier S1 and the majority of the silo pad
The ballast had obvious compaction markings and proved
well compacted with a probe test. (Figure 1)
The subgrade below the fill was inspected and met
the specifications required by the geotechnical documents
and foundation plans. The fill material was also inspected Figure 1:Formwork and compaction
to verify proper ballast material, compaction, placement,
and thickness.
The excavation was inspected to ensure that the

proper depths and materials were reached. The subgrade
material delineation is obvious upon visual inspection.
(Figure 2) There are no shallow foundation elements that
require inspection.

Matt was on site and walked the project during the
inspection. The workers were finishing the formwork and `
guides for ensuring a level slab. The remaining piers
formwork had been constructed since the site visit on
3/6/13. The majority of the reinforcing steel had been ..
placed in the forms for the concrete pour on Saturday. They- .::•.
silo pad and each pier was inspected for proper spacing of s ,� 9• ';
rebar, rebar size and grade, and lengths The silo pad and
each pier was also inspected for proper depth, length and r_ ..'; a
width. Unless stated in this report, all measurements met Figure 1: Reinforcing Steel
the specifications in the plans.
As shown in figure 1, the reinforcing steel is #6 A615 rebar. Each size of
rebar was inspected to ensure compliance with the plans. All washout slabs and the
approach slabs were not formed or laid with reinforcing steel yet, they will be a later
inspection.
The main silo slab had only three formwork walls constructed for ease of steel
installation, the fourth wall will be installed prior to concrete pour. The measurements
for the silo slab were accurate, and the markings on the formwork for the fourth wall
were accurate as well. (The fully constructed form will be measured prior to concrete
pour to verify correct dimensions.)
The steel reinforcement of the silo slab was changed
per request to add #3 bar perpendicular to the plan design -
to provide support for the design bar. The pier S1 was
measured and found to be 1'/2" short of the specified depth.
The change in pier depth will change the factor of safety
from 3.16 to 2.85 (1.5 min. required) and so will be allowed.
Finally, the anchor bolts to be used in the weld-plate design _
were inspected to ensure they met the plan specifications.

The concrete was manufactured by Hard Rock .eilE
Inc., and placed via pump truck in each of the piers '

according to ACI 318-11 C5. All concrete was
thoroughly consolidated by vibration during placement.

Matt and Bill were on site and walked the
project during the inspection. MTC was on site and
preparing to sample the concrete batch for test
cylinders.
The formwork dimensions, depth and placement
were inspected and verified for the silo pad pedestals Figure 1:Double Pedestals and
and scale piers being poured. The dimensions on the Piers N2, N3
weld plate anchor bolts were also verified. The rebar
for the pedestals (figure 3), the outwash slabs (figure
2, SW1, SW2 and SW3), and the remaining two piers
(figure 1, N2 and N3) were inspected and verified.
The placement of the concrete was inspected
according to ACI 318-11 C5. All concrete was
thoroughly consolidated by vibration during placement.
All concrete was thoroughly worked around -
reinforcement and into corners of forms.
The forms and rebar for the approach slabs
were not built or placed as of 3-12-13. The forms and "
