w FINDINGS OF FACT FPA Case#2018-00014 APPLICANT: Anderson and Welch APPLICATION DATE: August 30, 2018 Mason County has reviewed the above referenced application for Class IV General Forest Practices and finds it in conformance with all applicable County land use regulations in effect at the time of application. FINDINGS The proposed FPA is compliant with the following conditions: The proponent has paid the prescribed fees. The proponent has filed a complete, signed application. A map has been received that complies with the requirements of 11.05.120: (I) Harvest boundaries and tree retention areas. (II) The approximate location of any structures. (III) The location of all existing and proposed streets, right-of-ways, easements, skid roads,haul roads, and landings within the proposal. (IV) The location of future land development including stormwater management facilities and vegetation to be retained for site landscaping, open space,wildlife habitat, screening, and/or buffers. (V) Site topography at contour intervals of 40 feet. (VI) Critical areas and critical area buffers regulated pursuant to the Critical Areas Ordinance. (VII) Drainage ways and culverts. (VIII) Site area targeted for further harvest including proposed timing. (IX) North arrow and scale shall be shown on all site plans. The scale shall be no smaller than one inch to 200 feet. That this FPA is in conformance with the following Mason County ordinances: 11.05 That the proposed FPA involves 2.72 acres on three parcels. That the notifications required by 11.05.060 have been made if applicable. Note: The Allyn Waterfront Park is located about 400', and across SR 3from the NE corner of the unit. At this time, it is unclear if the Allyn Waterfront Park is listed in the Mason County Parks Register cited in 11.05.060. The Port of Allyn 1 r was not specifically notified, but this harvest will not be visible from the Allyn Waterfront Park Mason County Resource Ordinance _X_That at the issuance of this permit will not result in a violation of the Mason County Resource Lands and Critical Areas Ordinance. The site was inspected by Michael MacSems of the Mason County Planning Department who determined that any critical area buffer and/or steep slope issues have been resolved. Geo-Technical Report/Assessment Review A Geo-Technical__ Assessment X_Report(check one)was written by Envirotech Engineering and a submittal checklist was approved by Michael MacSems of Mason County DCD on 1/08/19. Washington State Department of Transportation _X_A DOT Access Permit is not required for this proposal. A DOT Access Permit is required for this proposal. State Environmental Policy Act This application for FPA#2018-00014: X Has been reviewed for SEPA compliance under case#: SEP 2018-00078. _Is SEPA exempt. _DNS X MDNS 2 r Stormwater Management A Stormwater and Erosion Control plan written to the specifications of the 2012 WSDOE Stormwater Manual by , as well as a submittal checklist was reviewed of DCD and he approved a submittal checklist on NOTE: Minimum Stormwater Requirements were addressed by the applicant in a written narrative. Permission from Underlying Property Owners: Yes—Kathy Anderson signed the application. CONCLUSION In conclusion, this application for Class IV General Forest Practices is consistent with the above criteria and that the public uses and Mason County has fulfilled its statutory obligations. ESTABLISH CONSTRUCTION ACCESS Construction access shall be established, and access points shall be stabilized to minimize the tracking of sediment onto public roads. If sediment is tracked off site, public roads shall be cleaned by shoveling or sweeping at the end of the day to prevent sediment from entering surface waters. (Applicant shall describe where the construction access will be, obtain a Road Access Permit if it is a new location, and construct access to minimize sediment tracking.) X * 4. UNIT FLAGGING The corners of this unit shall be flagged with brightly colored ribbon prior to issuance of this FPA. Also, the 40'Vegetation Setback in the SE corner shall be flagged. X * 5. GEO-TECHNICAL A geotechnical report has been prepared by Envirotech Engineering and the submittal checklist has been reviewed and found complete by Michael MacSems of Mason County Community Services Department.The applicant shall follow the recommendations of the report. Cutting shall be setback 40'from the top of the slope.Any future development will need to be setback a minimum of 50'from the top of slope. X * 6. MINIMUM FPA STOMWATER and EROSION CONTROL The boundaries of the clearing limits shown on this plan shall be clearly flagged in the field prior to site development. During the site development period, no disturbance beyond the flagged clearing limits shall be permitted. The flagging shall be maintained by the applicant/contractor for the duration of project. Stabilized construction entrances shall be installed at the beginning of construction and maintained for the duration of the project.Additional measures may be required to ensure that all paved areas are kept clean for the duration of the project. This project has been represented as not disturbing more than .75 acres of ground or creating more than 5,000 square feet of new impervious surfaces. An Erosion Control plan was written by Envirotech Engineering and submitted as Appendix D to the geo-report. The specifications of this plan shall be adhered to by the applicant. Equipment shall not operate in standing water conditions. X * The applicant will be responsible for the removal of debris. The burial of debris will not be allowed. * The conversion of this property will be permitted on the condition that prior to developing, subdividing, clearing, grading, or building of structures of any type, other than what is laid out in Forest Practice Application#09- , and approved drainage/siltation/stormwater control plan will be installed. This drainage/siltation/stormwater control plan will be reviewed and approve by Mason County Public Works. When the plan is to be built, the applicant must notify Mason CountyPermit Assistance Center for any permit(s)and fees that may be needed, Mason County Department of Community Development for permits(s) and fee that may be needed and notify Mason County Public Works to obtain permit(s)and fees to review the construction of the drainage/siltation/stormwater control plan prior to the structure being given final approval. There are two linear wetlands onsite,trending north and south of an existing culvert,and located adjacent to or within the smaller parcels ending in 1001,1004,1006,&1009. The wetland exhibited a dominance of hydrophytic vegetation(slough sedge,OBL;water parsley,OBL;Cooley's hedge nettle,FAC;red alder,FAC;and salmonberry, FAC). Hydric soils were present in both wetlands. The wetland located south of the culvert exhibits seasonal ponding(algae mats,water marks)and is probably less than 500 square feet in size. Hydrology in the wetland north of the culvert appears limited to seasonal surface flows and shallow groundwater saturation. This northern wetland is more than 100 feet long,relatively narrow(—2-3 feet wide)for much of its length,then broadening into a lobe approximately 12-15 feet wide before returning to a narrow drainage again. It is difficult to estimate the size of this feature but it is probably more than 500 square feet in area. Based on my site visit and the inability to follow the north-trending drainage through dense blackberry bushes,I cannot really determine if it is less than 1,000 square feet.
