In an effort of cooperation between the Mason County Dept.of Community Development/Building
Division and Jerry Vermillian to resolve the below Mason County Code violations,this agreement is

Re-Activate DOCK permit BLD2000-00928 and obtain necessary inspection(s)@$56.80 per
inspection;within 30 days of the date of this notice
Re-Activate FOUNDATION permit BLD2001-00190 and obtain necessary inspection(s)@$56.80 per
inspection;within 30 days of the date of this notice
Re-Activate ROOF STRUCTURE permit MIS98-00684 and obtain necessary inspection(s)@$56.80
per inspection;within 30 days of t116 date of this notice
} Apply for and follow necessary steps to obtain ATF permit for PORCH ENCLOSURE within 30 days
of the date of this notice
Apply for and follow necessary steps to obtain ATF permit for or remove STORAGE CONTAINER

6) Apply for and follow necessary steps to obtain ATF permit for or remove LAUNDRY/STORAGE
ADDITION within 30 days of decision regarding VAR2002-00015

The above is not a complete list of issues pertaining to the subject property. Inspection fees shall be paid
prior to each inspection. Timelines noted are effective beginning the date this document is signed. Failure
to comply with the items as noted will result in further enforcement action including but not limited to the
issuance of civil infractions pursuant to Mason County Ordinance 03-02,Hearings proceedings before the
Mason County Hearings Examiner pursuant to Mason County Development Code,Title 15 and other
remedies available by law.

1) Dock permitBLD2000-00928 Permit was reactivated on February 21, 2003, subsequent
FINAL inspection failed due to lack of construction plans on-site for Inspector. Approved plans must be provided
on—site for Inspector, if approved site copy has been misplaced, make arrangements for file search to determine if
Mason County file copy is still available or provide new plan for re-review. $56.80 re-inspection fee is due prior to
re-inspection.
2) Foundation permit BLD2001-00190 Permit was reactivated on February 21, 2003. Plan revisions
may be required due to the alteration in foundation load(loft area addition). Areas of foundation must be adequately
sized and reinforced for the number of floors supported.
3) Roof Structure permit MIS98-00684 Permit was reactivated on February 21, 2003, subsequent
inspection failed due to structure alterations not included in original permit. A copy of the inspector's log notes are
attached for clarification as to what is required. $56.80 re-inspection fee is due prior to re-inspection.
4) Porch Enclosure permit BLD2003-00198 Permit was applied for on February 24, 2003. Application is
currently being processed and reviewed.
5) Storage Container During our most recent conversation, you indicated that the
container will be/has been removed from the site. A Mason County Building Inspector must confirm that the
container was removed before this issue can be formally resolved.
6) Laundry/Storage Addition A permit will need to be applied for or the addition must be
removed within 30 days of the decision regarding VAR2002-00015.
7) Rockery Bulkhead and Wetland Fill Correspondence with Pam Bennett-Cummings indicates that
you are working towards compliance regarding these issues.
Fence During our recent conversation, you indicated that a small
portion of the fence exceeds 6' in height,but that portion would be cut down to meet the requirements of a structure
exempt from building permit requirements. A Mason County Building Inspector must confirm that the fence height
does not exceed 6'before this issue can by formally resolved.
The above is not a complete list of issues pertaining to the subject property, nor is it implied that the above is a
complete list of all possible requirements for the listed issues. Permits become null and void 180 days after the last
inspection. To avoid permit cancellation, please make sure that inspections for each permit are performed no less
than every 6 months. Failure to comply with the items as noted will result in further enforcement action including,
but not limited to, the issuance of civil infractions pursuant to Mason County Ordinance 03-02, Hearings
proceedings before the Mason County Hearings Examiner pursuant to Mason County Development Code, Title 15,
and othe edies available by law.
