RE: Site inspection of March, 2001
409 "H" Street, Shelton
Dear Mr. And Mrs. Jackson
Our office conducted a site inspection on 03/14/01 for possible earthquake
damage at the above location. The inspection was a result of a referral that you made to
the Mason County Department of Emergency Services. At the time of the inspection, the
inspector noted cracks in the slab, substantial cracks and damage to the masonry fireplace
and some sheetrock cracking. During the inspection, you spoke with the inspector
regarding the necessity for a thorough inspection by a Washington State licensed
Structural Engineer. Please contact the Building Department at (360) 427-9670 Ext. 284
regarding permitting requirements once the engineered report and recommendations have
been completed, if they include provisions for new construction or repair of the chimney.
The file records indicate that the inspector did not find cause to post the structure
for non-occupancy and therefore, the file regarding the earthquake damage has been
closed.

JIONTS AND THERE IS A
CRACK IN THE SLAB UNDER
THE KITCHEN.THE CRACK
DOES NOT APPEAR TO HAVE
CAUSE ANY STRUCTURAL
DAMAGE BUT IT NEEDS TO
BE SEALED TO PREVENT
ANY FUTURE WATER
DAMAGE.
