Page 1 of 8 ` CASE NOTES FOR BLD2012-00887 CONDITIONS FOR BLD2012-00887 1) By definition, propane tanks and heatpumps are structures, which must meet setback conditions. Please check your"Approved Site Plan"to ensure these structures meet the setback conditions listed. X 2) Owner/Agent751onsible to post the assigned address and/or purchase and post private road signs in accordance with Mason County Title 14.28. X 3) All construction must-meet or exceed all local ordinances and the international codes requirements as adopted and amended by Mason County and the State of Washington: =Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in permit revocation. X 4) All changes to"approved" building plans that effect compliance with the international codes as amended and adopted, or any other Mason County ordinance or regulatj n, must be reviewed and approved by Mason County prior to construction. X 5) All construction an demolition debris must be removed from the site after project completion. Proper dispo al of construction debris must be on land in such a manner that debris cannot enter or cause water quality degradation of State waters. X 6) The demolition and disposal of debris must meet the regulations of Mason County and Olympic Region Clean Air Agency (ORCAA). It is unlawful for any person to cause or allow the demolition (or major renovation) of any structure unless all asbestos containing materials have been identified and removed from the area to be demolished. All engineering and/or architectural documents are a part of the approved set of plans and shall remain attached thereto. If documents are removed, approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and shall ected by the Building Department prior to any further inspections being performed or approvals granted. X 9) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building Inspector shall be a prior to requesting additional inspections. X 10) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with Mason County or finances and building regulations. X 11) Carefully revs w the Mason County Fire Apparatus Access Standards, Mason County Code Title 14.17, attached to the approved construction plans in the plan review checklist and available online at www.co.mason.wa.us. Verify that the entire driveway is accessible and maintained with required pull-outs, vegetation and debris maintained or removed as needed, - an all-weather driving surface is installed from the county road access to the structure, and that hammerhead or other approved turnaround is provided. The international code requires a fire apparatus access road for every facility, building, or portion of a building that is more than 150'from an approved access road. Roads are required to meet the minimum Mason County Fire Marshal standards for Fire Apparatus Access Roads up to the paint where such roads connect with a county maintained public road or to another fire apparatus access road which,connects to a county maintained pulXic road. X - - 12) THE FOUNDATION SYSTEM SHALL BE PLACED ON UNDISTURBED, FIRM-NATIVE SOIL. X 13) The approval of 4is project is subject to the recommendations and specifications outlined in the attached geotechnical report or assessment. All applicable recommendations and specifications shall be applied to the development on this site. Any deviation requires stamped written approval from the registered design professional responsible for the report/assessment, and may require special inspection by same. Structures and/or land modifications (grading, cuts, fills, etc.) required in the geotechnical report/assessment, may require a separate permit. The geotechnical report/assessment shall remain attache he approved building plans. X 14) Landings and stairs must meet the same setback conditions as any permitted structure; and, must be shown on your site plan. Please check your "Approved Site an" to ensure these structures are shown and meet the setback conditions listed. X Bf_D2012-00887 Please refer to the following pages for conditions of this permit. Page 3 of 8 ;6) A minimum of 50 percent of all luminaires shall be high efficacy luminaries unless lighting compliance was approved using the options available in WSEC Section 1520 or 1530. Luminaires providing outdoor lighting and permanently mounted to a residential building or to other buildings on the same lot shall be high efficacy luminaries unless permanently installed outdoor luminaires are controlled by a motion sensor(s) with integral photocontrol photosensor or installed in or around swimming pools or water features. All fluorescent fixtures must be fitted with T-8 or smaller lamps (but not T-10 or T-12 lamps). Reference WS C 505. X 16) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements of the stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose. For further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or access connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450. For any construction which is proposed to be located within 25' of a Mason County road right of way, it is suggested to contact that office to review future planned work which may affect your project. X 17) A permanent certificate, completed by the builder or registered design professional, shall be posted within three feet of the electrical distribution panel. The certificate shall list the predominant R-values of insulation installed in or on ceiling/roof, walls, foundation (slab, basement wall, crawlspace wall and/or floor), and ducts outside the conditioned spaces; U-factors for fenestration; and the solar heat gain coefficient(SHGC) of fenestration. Where there is more than one value for each component, the certificate shall list the value covering the largest area. The certificate shall list the type and efficiency-of heating, cooling, and service-water heating equipment, duct leakage rates including test conditions as specified in WSEC Section 503.10.2, and air leakage results if a blower door test was conducted. Building envelope air leakage control shallbe considered acceptable when tested to have an air leakage,less than 0.00030 Specific Leakage Area (SLA) 1, when tested with a blower door at a press of 50 Pascals (0.2 inch w.g.). Testing shall occur at any time after rough in and after installation of penetrations Y of the building envelope, including penetrations for utilities, plumbing, electrical, ventilation, and combustion appliances and sealing thereof. The blower door test results shall be recorded on the permanent certificate required located near the electrical distrbution panel. Air leakage testing is not required for additions less than 750 square feet. Reference WSEC 105.4 Certificate and 502.4.5 Building Air Le:' ' ge Testing. Compliance certificates are available online at the WSU Energy program website titled, "WSEC 2009 Certificate" and are available in %4 or'/z sheets. The Mason County Permit Center will also have some available. X 18) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit holder have preven ion from being taken. No more than one extension may be granted. X PW 19) The plan review check list and corrections are part of the approved plans and must remain thereto. It is the responsibility of the applicant to make the corrections indicated on the plans. Once the plans are marked "APPROVED", they shall not be changed or altered without authorization from the Building Official. The permit holder is responsible to retain the complete approved set of plans on site for the duration of the project. Failure to comply and/or removal of approve cuments will result in failure of required building inspections. X BLD2012-00887 Please refer to the following pages for conditions of this permit. Page 4 of 8 • 20) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour) will be charged and must be collected by the Building Department prior to any further inspections being performed or approvals granted. X 21) All propane tan(s must be installed in accordance with the International Fire Code and all applicable Mason County ordinances. All propane tanks must meet the installation requirements and minimum setbacks as listed in the Mason County Fire Marshal's Standards for the Installation of Propane Tanks. X 22) All property Qlies shall be clearly identified at the time of foundation inspection. X 23) Contractor registration laws are governed under RCW 18.27 and enforced by the WA tate Dept of Labor and Industries, Contractor Compliance Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at 1-800-647-0982. X 24) Retaining walls needed to support a surcharge such as structures, roads, or to support slopes, shall require a separate building permit and approval prior to construction of the retaining wall. X 25) REQUIREMENTS FOR ROOF COVERINGS. oof coverings shall be applied in accordance with the applicable provisions of the current code and the manufacturer's installation instructions. X 26) WIND LOADS-=.Roof coverings shall be designed and tested to withstand the maximum basic wind speed. The basic wind speed for Mason,County is 85 MPH. X 27) The prop d project must be consistent with all applicable policies and other provisions of the Shoreline Management Act, its rules, and the Mason County Shoreline Master Program.X 28) Approved per dimensions and setbacks on de plan submitted on Dec. 5, 2012. Setbacks are measured from the furthest projection of the proposed structure. X 29) The"approved" site plan is required to be on-site for inspection purposes. If an inspection is requested and the"approved" site plan is not on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and shall be collected by the Building Department prior to any further inspections being performed or approvals granted. Page 5 of 8 • -10) An inspection performed by a representative of the geologist, their authorized representative, or engineer of record, shall be completed on-site prior to the footing pour to verify that earthwork and foundation installation activities comply with the Geotechnical report or assessment recommendations. Reports shall be submitted to the Mason County Building Dept., PO Box 186, Shelton, WA 98584, prior to the footing and final inspections and available for inspection. X 31) A Mason County Stormwater Management Worksheet was completed and signed as part of this building permit application. Design, sizing, placement, inspection and maintenance of stormwater management systems shall be the responsibility of the owner/agent of the developed parcel. It is the owner/agent/contractor's responsibility to ensure that Mason County Department of Public Works has approved the stormwater site plan for this parcel prior to the commencement of any development activities. "NOTE if Stormwater Management option "A"was selected on the Small Parcel Stormwater Management Application/Worksheet the document entitled "Managing Storm Drainage on Small Lots, The Small Parcel Stormwater Site Plan" constitutes an approved plan based on the criteria listed on the application/worksheet. If the development has, or will have, a septic/drainfield system you are responsible for contacting Mason County Division of Environmental Health to ensure that the stormwater system will not adversely affect the septic system of this, or any other, parcel. You may also wish to consult with the septic design professional involved with the project. By calling for a final inspection of the building permit the owner/agent/contractor is ck_owledging that all components of the stormwater management system have been installed as approved on the stormwater site plan. X 32) ALL SURFACE WATER AND POTENTIAL RUNOFF WILL BE CONTROLLED ON SITE AND SHALL NOT ADVERSLY AFFECT ANY ADJACENT PROPERTIES NOR INCREASE THE VELOCITY FLOW ENTERING OR ABUTTING TO ANY STATE OR COUNTY CULVERTING/DITCHING SYSTEM OR ROAD WAY. X 33) Temporary erosion ontrol measures must be implemented to prevent water quality degradation of Ajacent wEers or=properties. Silt fencing or straw ... matting must be installed and maintained until upland vegetation has become established. X 34) Washington State Energy Code Compliance has been approved as follows: Heat Type: Air source heat pump, HSPF 8.5 or better, Compliance Method: Prescriptive option I, Window(Max U-Factor):0.30, Skylight(Max U-Factor):0.50, Doors (Type/Max U-Factor):0.20 or less, Wall insulation R-21, Floor insulation R-30, Ceiling Insulation R-38 advanced or R-49 standard, Vault Insulation R-38, and Slab Insulation R-10. In addition the following credit from Table 9-1 shall be completed: Option 1A: High Efficiiency HVAC Equipment, Heat pump HSPF 8.5 or better. Page 6 of 8 +D 35) Installation of heating equipment in a single-family residence shall meet the requirements of the current Washington State Energy Code, applicable sections of the IRC and IMC. Heating equipment shall be sized in accordance to IRC, Section M1401. Heating and design load calculations for the purpose of sizing HVAC systems are required and shall be calculated in accordance with accepted practice, including infiltration and ventilation. Design calculations shall be available for inspection during inspection. Referencing IRC M1601.4, all ducts, air handlers, filter boxes, and building cavities shall be sealed. All joints of duct systems and seams shall be made substantially air tight by means of tapes, mastics, liquid sealants, gasketing or other approved closure systems. Closure systems used with rigid fibrous glass ducts shall comply with UL181A and shall be marked 181A-P for pressure-sensitive tape, 181A-M for mastic or 181 A-H for heat-sensitive tape. Closure systems used with flexible air ducts and flexible air connectors shall comply with UL181 B and shall be marked 181 B-FX for pressure-sensitive tape or 181 B-M for mastic. Duct connections to flanges of air distribution system equipment or sheet metal fittings shall be mechanically fastened. Mechanical fasteners for use with flexible nonmetallic air ducts shall comply with UL 181 B and shall be marked 181 B-C. Crimp joints for round metal ducts shall have a contact lap of at least 1-1/2 inches (38 mm) and shall be mechanically fastened by means of at least three sheet-metal screws or rivets equally spaced around the joint. Closure systems used to seal metal ductwork shall be installed in accordance with the manufacturer's installation instructions. Duct tape is NOT permitted as a sealant on any ducts. When ducts are located in unheated spaces the ducts hall be insulated to R-8 DUCT TIGHTNESS TESTING shall be conducted by person(s) trained to perform such testing. A signed affidavit documenting test results in accordance to WSEC Section 503.10.3 shall be provided to the Mason County.Building Department prior to the final occupancy inspection.. Affidavit forms are available on at the`WSU-Energy Program website titles, "Duct Leakage Affidavit"or"Duct Leakage Testing Results (Existing Construction)." Duct tightness testing is not required if the air handler and all ducts are located within the heated space. All heating equipment shall meet the requirements of the National Appliance Enefgy Conservation Act(NAECA) and be so labeled. (WSEC503.4) or in accordance to WSEC Section 1411. Contact the Mason County Building department to reuest the additional inspctions at (360)427-9670 ext. 352 SHEAR WALL INSPECTION A separate inspection in addition to the inspections required in IBC, Section 109 shall be required in accordance with the 2009 IBC, Section 1707.3. As specified by the engineer of record Mason County building inspector or other approved special inspector shall verify that sheathing type and thickness and nailing pattern at all high load diaphragms and shear walls per section 1704.6.1. For this project, this may requirement applies to the floor diaphragm at the upper floor, and to shear wall at the main floor listed as W3, W2, and 2W3 of sheet S2.1. Windows in High Exposure Area All windows and doors shall be installed and flashed in accordance with the manufacturers written installation instructions and shall be available during inspections. In addition to other inspection(s), an inspection of the flashing shall be required for projects located in an area with a wind exposure of C or D, reference IRC R301.2.1.4 & IBC 1609.4. The design criteria for this project is established at 85 mph wind speed, exposure C. The required inspection may be performed by the Mason County Building Department, a WABO certified inspector, certified to inspect as specified, the engineer of record(EOR), or an authorized representive of the EOR. The special inspectors duties & responsibilities shall be as specified in Chapter 17. When a,third party inspector is used to perform the inspection special inspection reports shall be submitted to the Mason County Building Department, PO Box 186, Shelton WA 98584 and also available for-inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing inspections. X K) In addition to Qthinspections required in IBC, Section J09, the owner, the engineer or architect of record acting as the owner's agent shall employ one oore special inspectors who shall provide inspections during construction on the types of work listed under Chapter 17 and as specified by the design professional. The special inspectors duties &responsibilities shall be as specified in Chapter 17. Welding performed on-site shall be performed by a WASO certified special inspector. Welding performed off-site shall be completed at a WABO certified fabrication shop. when required ppecial inspection reports shall be submitted to the Mason County Building Department, PO Box 279, Shelton WA 98584 and available for inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing and final occupancy inspections. X This permit becomes null and void if work or construction authorized is not commenced within 180 days, or if construction or work is suspended for a period of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Gr055 Structural Engineers giL Re: Bobcat Residence Garage shear walls (project #10223G.7) I am writing this letter to summarize the approved revisions to the shear walls on the west side of the garage at the Bobcat Residence project in Allyn, WA. In March of 2013 the shear walls on either side of the garage doors were revised to eliminate the center shear wall between the two garage doors. This revision was shown on the Construction Set documents issued on March 27, 2013. The revision included increasing the length of the two end piers to 2'-5", increasing the shear wall type to a 2W2 shear wall on each side, and increasing the holdowns at the ends of the walls from HDUSs to HDU11s. I have included a partial copy of the revised plan with this letter for reference. We were able get up on ladders and with a flashlight confirm the roof sheathing was cut back in all areas except one. This allows our roof calcs that I sent over previously at 1/300 to apply. The area in question that does not have the sheathing cut back is to the west of Crow's Nest with a low roof ending in the wall of the Crow's Nest. Al is having the roofer come back and install the roof to wall venting in this area. Let us know if there is anything else we need to do to resolve this issue. The site visit was conducted on March 29, 2013 in order to observe the soil conditions, and assess the suitability of the founding soils for use as bearing strata. Information pertaining to the project was provided by Mr. Fujita, owner's representative of the referenced property. Information collected from site observations, and field besting mas completed by Michael Staten with Envirotech. It is reported that garage footings have been structurally designed to range from 16 inches and 24 inches wide. house footings are to be 18 inches to 22 inches in width. The foundation baring soils were competent at the time of our site visit per the attached sketch. Relative density was assessed by measuring the resistance of hand tools within several locations of the planned building pad. It is concluded that the existing bearing soils and editions are suitable for a structural pressnne of no more than 15M psf for varying foundation widths per the existing shwctu al design.The foundation systems may undergo a maximum of 1.0 inch total settkmwzt,and a maximum differential settlement of 0.75 inch. The conclusions presented above were derived from engineering judgment using geotechnical standards in conjunction with the information collected from the site visit and owner of the Project.
