X 10) Any anges in construction shall be reviewed by engineer of record and submitted in writing to the Mason County Building Department prior to construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are removed, approval will not be granted. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Maso unty Building Inspector shall be made prior to requesting additional inspections. X COM2016-00066 Page 3 of 8 14) New and replaced heating equipment shall meet the requirements of the internation tnergy conservation uoae as amenaea vvasnington jtate Energy Code (WSEC), and applicable sections of the IBC and IMC. All new or replaced heating equipment shall meet the efficiency requirements of WSEC C403.2.3. Those parts of heating systems that are altered or replaced shall comply with the current Washington State Energy Code (WSEC), Section C101.4.3.2. When a space-conditioning system is altered by the installation or replacement of space conditioning equipment (including replacement of the air handler, outdoor condensing unit of a split system air conditioner or heat pump, cooling or heating coil, or the furnace heat exchanger), the duct system that is connected to the new or replacement space-conditioning equipment shall be installed and sealed in accordance with C403.2.7. All new and replaced heating equipment shall comply with WSEC Section C403. Heating and design load calculations for the purpose of sizing HVAC systems are required and shall be calculated in accordance with accepted practice, including infiltration and ventilation. Provide design calculations during inspection. Referencing WSEC C403.2.7 and IMC Chapter 6, all new ducts, air handlers, filter boxes, and building cavities shall be sealed. All joints of duct systems and seams shall be made substantially air tight by means of tapes, mastics, liquid sealants, gasketing or other approved closure systems. Closure systems used with rigid fibrous glass ducts shall comply with UL181A and shall be marked 181A-P for pressure-sensitive tape, 181A-M for mastic or 181 A-H for heat-sensitive tape. Closure systems used with flexible air ducts and flexible air connectors shall comply with UL181 B and shall be marked 181 B-FX for pressure-sensitive tape or 181 B-M for mastic. Duct connections to flanges of air distribution system equipment or sheet metal fittings shall be mechanically fastened. Mechanical fasteners for use with flexible nonmetallic air ducts shall comply with UL 181 B and shall be marked 181 B-C. Crimp joints for round metal ducts shall have a contact lap of at least 1-1/2 inches (38 mm) and shall be mechanically fastened by means of at least three sheet-metal screws or rivets equally spaced around the joint. Closure systems used to seal metal ductwork shall be installed in accordance with the manufacturer's installation instructions. Duct tape is NOT permitted as a sealant on any new duct systems. When new ducts are located in unheated spaces the ducts shall be insulated to R-8 X 15) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliaD th Mason County ordinances and building regulations. X 16) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permi der have prevented action from being taken. No more than one extension may be granted. In addition a final inspection report, prepared by a representative of the geologist, their authorized representative, or engineer of record shall be required to verify that erosion control, drainage and final work is completed in accordance to the Geotechnical report or assessment recommendations. Reports shall be submitted to the Mason County Building Dept., PO Box 186, Shelton, WA 98584, prior to the footing and final inspections and avail spection. X 18) Pressure treated wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fasteners, co ectors, and flashing. Install metal connectors approved for contact with the new types of pressure treated material. X 19) Recycla aterials & Solid Waste Storage: Space shall be provided for the storage of recycled materials and solid waste. The storage area shall be design eet the needs of the occupancy, efficiency of pick-up, and shall be available to occupants and haulers.X 20) No building or structure shall be used or occupied, and no change in the existing occupancy classification of the building or structure or portion thereof shall be made until the building official has issued a certificate of occupancy as provided herein. Issuance of a certificate of occupancy shall not be construed as an approval of a violation of the provisions of the codes or ordinances of the jurisdiction. Certificates presuming to give authority to violate or cancel the provisions of this code or other ordinances of the jurisdiction shall not be valid. X 21) The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any of the provisions of the building code, or any other code or ordinance of Mason County. Permits presuming to give authority to violate or cancel the provisions of the building code or other ordinances of the jurisdiction shall not be valid. The issuance of a permit based on construction documents and other data shall not prevent the building official from requiring correction of errors in the construction documents and other data. The building official is also authorized to prevent occupancy or use of a structure where in violation of this code or of any other ordinances of this jurisdiction. X X4) 22) A full, NFPA ompliant fire alarm system shall be installed throughout the building under a separate permit issued by Mason County. Complete permit applications shall be submitted with ample time for review and approval, prior to the installation of any component of the required fire protection systems. Fire alarm permit final inspection and approval shall be obtained prior to requesting a final inspection. X 23) In accor e with IBC 703.6, fire barriers shall be effectively and permanently identified with signs or stenciling within the space above the ceiling grid. Extinguishers shall be located not higher than 5 feet above the finished floor. Indoor fire extinguisher cabinets shall not be equipped with locks. Extinguishers shall have a service tag verifying they have been serviced by a professional extinguisher company. If extinguisher is recently purchased, the tag inside the box may be affixed to the extinguisher with "NEW" and the date of purchase. X 25) Third- r y Special Inspection is required in accordance with IBC Sections 1704 & 1705 for the items identified in the Statement of Special Inspections (as required by IBC Section 1704.3). In accordance with IBC Section 1704.4, each contractor responsible for the construction of the work listed in the statement of special inspections shall submit a written statement of responsibility to the building official and the owner prior to the commencement of work on the system or component. The contractor's statement of responsibility shall contain acknowledgment of awareness of the spec I requirements contained in the statement of special inspections. X 26) Plan eview Requirements: And any identified corrections, along with the Energy Code Compliance Worksheet(when applicable) are part of the approved plans and must remain attached thereto. It is the responsibility of the applicant to make the corrections indicated on the plans. Once the plans are marked "APPROVED", they shall not be changed or altered without authorization from the Building Official. All proposed changes to "APPROVED" building plans that affect compliance with the Codes as amended and adopted, or any other Mason County ordinance or regulation, must be reviewed and approved by the Building and Planning Departments prior to construction. Failure to comply and/or removal of attached clocuments,,will result in failure of required building inspections. X 27) Storage racks greater than 69" and up to 60" in height shall be installed in accordance to manufacturer specifications for seismic attachment. When seismic attachment is not provided by the manufacturer an engineered design and attachment details shall be required. Installation specifications or engineer's design shall be available during inspections performed by the Mason County Building Department. Storage racks greater than 8-ft in height shall require an engineer's design addressing seismic conditions and attachment. In addition, storage racks 8-ft tall or more shall require special inspection performed by the design professional or an approved representative. The special inspection report and engineered design shall be provided to the Mason County Building Department for approval prior to the final occupancy inspection. X- /00;5) 28) The stru ure is subject to inspection by the Mason County Fire Marshal. All corrections deemed necessary by the Fire Marshal, to assure the minimum fire and life safety requirements as adopted by Mason County and Washington State may be required. The specified special inspector(s) shall perform the required special inspections during construction of the project, submit all inspection reports to the Building Department and provide a final signed report stating whether the work requiring special inspection was, to the best of the inspector's knowledge, in conformance to the approved plans and specifications and the applicable workmanship provisions of the IBC. Raymond Mow Sig atur Printed Name A Owner A t Date PURPOSE OF SPECIAL INSPECTION Special inspection is the monitoring of materials and workmanship which are critical to the integrity of the building structure to assure that the approved plans and specifications are being followed and that relevant codes are being observed. Special inspections are in addition to those conducted by the county building inspector and by the engineer or architect of record. Special inspectors furnish inspection at all times that their presence is required by the code, the plans and specifications, and the enforcing jurisdiction. Good communication between the special inspector and the designer, contractor, and building department is essential. DUTIES AND RESPONSIBILITIES OF THE SPECIAL INSPECTOR: Special inspectors are individuals with highly developed, specialized skills who observe those critical building or structural features which they are qualified to inspect. Duties of special inspectors and/or inspection agencies include the following: 1. General Requirements Special inspectors shall review approved plans and specifications for special inspection requirements. Special inspectors shall comply with the special inspection requirements of the enforcing jurisdiction. 2. Signify Presence at Job Site Special inspectors should notify contractor personnel and the enforcing jurisdiction of their presence and responsibilities at the job site. 3. Observe All Work for Which They Are Responsible Special inspectors shall inspect all work and perform or observe all tests requiring inspection and testing for conformance with the building-department-approved (stamped) drawings, specifications and applicable provisions of the code. 4. N'otification of Discrepancies Special inspectors shall bring all nonconforming items to the immediate attention of the contractor. If any such item is not resolved in a timely manner or is about to be incorporated in the work, the engineer'or architect of record and the building official should be notified immediately by telephone or in person, and the item noted in the special inspector's written report. The special inspector shall write a separate report to be posted at the job site regarding noted discrepancies which should contain, as a minimum, the following information about each nonconforming item: • Description and exact location, • Reference to applicable detail of approved plans/specifications, • Name and title of each individual notified and method of notification, and • Resolution or corrective action. 5. Reports A. Inspection reports The special inspector should complete written inspection reports for each inspection visit and provide the reports on a timely basis determined by the building official. The special inspector or inspection agency shall furnish these reports directly to the building official, engineer or architect of record, and others as designated. These reports should be organized on a daily format and may be submitted weekly at the option of the building official. In these reports, special inspectors should: • Describe inspections and tests made with applicable locations; • List all nonconforming items; • Indicate how nonconforming items were resolved; • List unresolved items, parties notified, and time and method of notification; and • Itemize changes authorized by the architect/engineer of record and approved by the building department, if not included in the nonconformance items. B. Final reports Special inspectors or inspection agencies shall submit a final, signed report to the building department stating that all items requiring special inspection and testing were fulfilled and reported and, to the best of their knowledge, in conformance with the approved design drawings, specifications, approved change orders and the applicable provisions of the code. Items not in conformance, unresolved items or any discrepancies in inspection coverage (i.e., missed inspections, periodic inspections when continuous was required, etc.)should be specifically itemized in this report. DUTIES AND RESPONSIBILITIES OF THE CONTRACTOR: The contractor's duties include responsibility for notifying the special inspector or agency regarding individual inspections required by the Building Department, or required by the International Building Code, Chapter 17. Adequate notice shall be provided so that the special inspector shall have appropriate access to the approved plans at the job site prior to the required inspection. The contractor shall not perform any work that requires special inspection without the special inspector present. Completion of this form, and the performance of special inspections does not relieve the contractor's responsibility to schedule required inspections by the Mason County Building Department prior to concealing any work. DUTIES AND RESPONSIBILITIES OF THE ENGINEER OR ARCHITECT OF RECORD: The engineer or architect of record is responsible for identifying which special inspections are necessary, developing a schedule of required special inspections, and assisting the owner in the selection of the special inspector. The engineer or architect or record shall review the special inspection reports to determine that all required special inspections have been performed and that all discrepancies (if any) have been resolved.
