This checklist is intended to assist Staff in the review of a Stormwater Plan. The Stormwater Plan is
reviewed for completeness with respect to the 2005 Department of Ecology Stormwater Manual. If an
item is found to be not applicable, the Plan should explain the basis for the conclusion. The Plan is also
reviewed for clarity and consistency. If the drawings, discussion, or recommendations are not
understandable, they should be clarified. If they do not appear internally consistent or consistent with the
application or observations on site, this needs to be corrected or explained. If resolution is not achieved
with the author, staff should refer the case to the Planning Manager or Director.

(a). An operation and maintenance schedule shall be provided for all proposed stormwater facilities
'7 and BMPs, and the party (or parties) responsible for maintenance and operation shall be
identified. An operation and maintenance (0 &M) Declaration of Covenant will be required to
cover all privately owned and maintained stormwater facilities. 0& M Declaration of Covenant
forms are available at the Mason County Permit Assistance Center, 426 W. Cedar Street,
Shelton,WA 98584. The proponent shall record a copy of the completed Declaration with the
Mason County Auditors' office. A copy of the recorded document must be submitted to the Permit
Assistance Center together with this completed Checklist.

Butch,
I just finished going through N.L. Olson's stormwater and erosion control plan submittal
checklist and I am not able approve the document as complete. For this to make sense
you need to know that we Mason County planners are not engineers, and we rely
exclusively on the submittal checklist review to determine if an engineered stormwater
and erosion control plan is fully compliant with County requirements. We simply are not
qualified to read through the documents and make a determination unaided.
In this case, I see that the engineer left a large number of items unchecked, particularly in
Section II. I realize that many of these items may not apply to your situation and in that
case the engineer needs to provide a written acknowledgment when a checklist item is not
applicable.
Please have the engineer address all of the following items in the form of an addendum
(if needed)and a completed Submittal Checklist.
Here is the list:
Section I
La through c; I can't not certify that this has been done for all elements.
12-point required elements; Items 3 through 12 have not been addressed. The engineer
needs to direct the reader to the location in the Narrative where the element is discussed
or explain why it is not required.

One other thing: I noticed on the Submittal Checklist that 2,200 cubic yards of earth will
be cut and filled. This requires a Land Modification Permit. I see that one has not been

also modify your SEPA checklist to include this in the project description, as over 500
cubic yards of cut and/or fill is subject to SEPA review.
