APPLICANT: North Mason School District
APPLICATION DATE: April 23, 2014
Mason County has reviewed the above referenced application for Class IV General Forest
Practices and finds it in conformance with all applicable County land use regulations in
effect at the time of application.
FINDINGS
The proposed FPA is compliant with the following conditions:
The proponent has paid the prescribed fees.
The proponent has filed a complete, signed application.
A map has been received that complies with the requirements of 11.05.120:
(I) Harvest boundaries and tree retention areas.
(II) The approximate location of any structures.
(III) The location of all existing and proposed streets, right-of-ways, easements, skid
roads, haul roads, and landings within the proposal.
(IV) The location of future land development including stormwater management
facilities and vegetation to be retained for site landscaping, open space,wildlife
habitat, screening, and/or buffers.
(V) Site topography at contour intervals of 40 feet.
(VI) Critical areas and critical area buffers regulated pursuant to the Critical Areas
Ordinance.
(VII) Drainage ways and culverts.
(VIII) Site area targeted for further harvest including proposed timing.
(IX) North arrow and scale shall be shown on all site plans. The scale shall be no
smaller than one inch to 200 feet.
That this FPA is in conformance with the following Mason County ordinances:
11.05
That the proposed FPA involves 5.5 acres encompassing two parcels.
That the notifications required by 11.05.060 have been made if applicable.

County Resource Lands and Critical Areas Ordinance. The site was inspected by
Allan Borden of the Mason County Planning Department who determined that
any critical area buffer and/or steep slope issues have been resolved.

CONCLUSION
In conclusion,this application for Class IV General Forest Practices is consistent with the
above criteria and that the public uses and Mason County has fulfilled its statutory
obligations.

I, being the Manager of the Mason County Planning Department, hereby attest to and
certify that appropriate provisions have been made for this Class IV General Forest

1) STORM WATER & EROSION CONTROL
Escape of silty water or mud from this property shall be considered a violation of County and State storm water management regulations, and could
subject the applicant(s)to civil fines or penalties. Conditions and specifications of the storm water and erosion control plan/SWPPP prepared for this
project by Mark Conwell (consistent with 2005 WSDOE Stormwater Manual) shall be fully implemented and observed.

2) SLASH ABATEMENT
This Forest Practice Application indicates that forest debris resulting from the proposed logging operation may create an extreme fire hazard under RCW
76.04.600 and WAC 332-24-650. An extreme fire hazard could be created if one or more of the conditions found in WAC 332-24-650 is located within 100
feet of the running surface of a public road; or is within 500 of an existing structure and/or designated public use area, which is not owned by the owner of
the fire hazard area.
The land owner is fully liable in the event of fire, if he or she fails to appropriately abate the hazard. Extreme fire hazards shall be abated by removing the
forest debris, or by other means approved by the Washington State Department of Natural Resources (DNR).

3) ARCHAEOLOGICAL RESOURCES
Archaeological or historical resources, such as ruins, sites, buildings, artifacts, fossils, or other objects of antiquity that may have significance from an
historical or scientific standpoint, which may be encountered by the developer/contractor, shall not be further disturbed. If the developer/contractor
encounters any such artifacts, he/she shall immediately stop work and notify Mason County and the Washington State Department of Archaeology and
Historic Preservation. Failure to do so may result in civil penalties.

4) HOURS OF OPERATION
Noise associated with this application shall not exceed what is allowed under Mason County Code 9.36. Harvest operations shall start no earlier than 7:00
AM and shall end before 7:00 PM on a daily basis.

5) GEO-TECHNICAL
A geotechnical report has been prepared by Envirotech Engineering and the submittal checklist was approved Michael MacSems of Mason County
Department of Community Development. The applicant shall follow the recommendations of the report.
6) ESTABLISH CONSTRUCTION ACCESS
Construction access shall be established and access points shall be stabilized to minimize the tracking of sediment onto public roads. If sediment is
tracked off site, public roads shall be cleaned by shoveling or sweeping at the end of the day to prevent sediment from entering surface waters. (Applicant
shall describe where the construction access will be, obtain a Road Access Permit if it is a new location, and construct access to minimize sediment
tracking.)

This permit becomes null and void if work or construction authorized is not commenced within 180 days, or if construction or work is suspended for a period of 180 days at any
time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be
occupied. Proof of continuation of work is by means of a progress inspection.The owner or the agent on the owners behalf, represents that the information provided is accurate
and grants employees of Mason County access to the above described property and structure for review and inspection.

Michael,
I wanted to run something by you. We have the FPA all filled out and ready to go,except for the erosion control
plan. It is due in from the engineer any time,but I don't want to hold up the FPA. We will be submitting a Clear/Grade
permit application in the next week or so,which will include all of this work and the clearing portion. My fear is that it
would take less time to process a clear and grade permit than the FPA.
Can we submit the FPA as it is now and supplement it with the TESC drawings once they are ready? We have
everything else in place including the SEPA and the Tax Number on this end. Please let me know if this will work on
your end. If so, I'll drop it off today. Thank you for your consideration.

Purpose of Checklist:
The State Environmental Policy Act(SEPA),chapter 43.21C] RCW,requires all governmental agencies to consider the
environmental impacts of a proposal before making decisions. An environmental impact statement(EIS)must be prepared for all
proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide
information to help you and the agency identify impacts from your proposal(and to reduce or avoid impacts from the proposal,if it
can be done)and to help the agency decide whether an EIS is required.
Instructions for Applicants:
This environmental checklist asks you to describe some basic information about your proposal. Governmental agencies use this
checklist to determine whether the environmental impacts of your proposal are significant,requiring preparation of an EIS. Answer
the questions briefly,with the most precise information known,or give the best description you can.
You must answer each question accurately and carefully,to the best of your knowledge. In most cases,you should be able to
answer the questions from your own observations or project plans without the need to hire experts. If you really do not know the
answer,or if a question does not apply to your proposal,write"do not know"or"does not apply". Complete answers to the
questions now may avoid unnecessary delays later.
Some questions ask about governmental regulations,such as zoning,shoreline,and landmark designations. Answer these questions

6.Proposed timing or schedule(including phasing, if applicable):
The project schedules with estimated construction start and
completion dates are as follows:
• Begin construction of the new high school, new play field, service
loop and northern parking lot July 2014. The high school, parking
lot and service loop are to be completed by December 2015. New
play field to be completed by October 2015.
• Move students from the existing high school to the new
high school in December 2015.
• Place new portables for PACE Academy (Alternative HS)
in August 2015. Students are to occupy the new PACE Academy
in September 2015.
• Begin the modernization of the existing high school (to become
Hawkins MS) in January 2016. The modernization is to be
completed by August 2016. Move all existing staff and students
from the existing Hawkins Middle School to the modernized school
in August 2016. Full occupancy and school to begin September 2016.
• The demolition of the existing Hawkins MS will occur June 2016 and
be totally removed by mid-July 2016. Following the demolition, the
main parking area and bus drive loop are to be replaced. The parking
and bus drive loop replacement is to be completed by September
2016.
• Move District Administration office staff from the existing
building to the existing Annex Building (currently the existing
high school main office building and 6 classrooms) February 2016.
Concurrently with this move, the Boys and Girls Club will move
from their existing portables to the existing Administrative
Building.
• Upon completion of the Boys and Girls Club move into the
existing Administrative Building, the Boys and Girls Club portables, PACE
portables and central kitchen will be demolished.
• Immediately after the portables are demolished, the stadium
project is to begin construction in approximately March 2016.
The stadium project consists of the stadium covered seating, track and field,
football field, and concessions, restroom and locker building. The completion
of the stadium will be August 2016.

the east, adjacent to the existing baseball field. As part of this
project, utilities will be stubbed out on the east side of the new

The proposal will need approval on various phases of the project
from the North Mason School Board of Directors and the State
Superintendent of Public Instruction. Mason County permits for
Special Use, building height variance, building, plumbing, mechanical,
electrical, and site development are required. Mason County Health
Department approval is required for the kitchens.

The proposal includes the construction of a new replacement high
school and central kitchen on the east-central part of the site,
which is currently occupied by an athletic field. The existing middle
school will be removed, and the existing high school will be
converted into a new middle school. Additionally, the existing
central kitchen will be demolished and a new stadium/athletic field
will be built in its current location on the northwest corner of the
site; a new athletic field will be built on the northeast corner of
the site; and new pavements will be constructed to accommodate
the planned improvements. See attached Exhibit I.

Best Management Practices as set forth in the current Mason
County Stormwater Management and Site Development Manual will
be utilized during construction. These practices may include but
are not limited to: stabilized construction entrance, temporary
interceptor ditches and swales, temporary sediment traps/ponds,
and silt fencing as needed. Seasonal limitations of construction
activities will be considered and erosion and sedimentation control
will be provided accordingly to allow construction to continue as
needed during the wet season. All County and State requirements
for wet season work will be adhered to.
2. AIR

Temporary increase of construction vehicle emissions and dust
during construction. Upon completion of the project, minimal
amounts of auto and bus exhaust will occur during operational hours
of the facility. There are no significant adverse impacts to air
quality as a result of the proposed project.

existing infiltration pond and supplemental underground infiltration
facilities in accordance with the 2005 Edition of Ecology's
Stormwater Management Manual for the Puget Sound Basin. Storm
water runoff from rooftops and other non-pollution generating
surfaces will be collected by conventional storm water conveyance
systems consisting of catch basins and underground gravity flow
piping and routed directly to the aforementioned infiltration
facilities.
