1) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance
Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be

2) All approved plans are ed to be on-site for inspection purposes. If inspection is called for and 211ans are not on site, Approval WILL NOT be
granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour w" a harged and collected by the Mason County
Building Department prior to any further inspections being performed or approvals granted. X
3) Owner/Agent is responsible to post the assigned address and/or purchase and post private road signs in accordance with Mason County Title

7) 3000 PSI CONCRETE WILL REQUIRE SPECIAL INSPECTION IF THE QUANTITY EXCEEDS 50 CUBIC YARDS< LESSER AMOUNTS WILL
REQUIRE AN APPROVED CONCRETE SUPPLIER TO PROVIDE YOU WITH A BATCH TICKET THAT WILL BE REQUIRED TO BE SUBMITTED
TO THE SITE INSPECTOR FOR THE VERIFICATION OF MATERIAL USED. X G
8) Any changes in construction shall be reviewed by engineer of record and submitted in " g to the Mason County Building Department prior to
construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are
removed, approval will not be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and
collected by the Mason County Building Department prior to any further inspections being performed or approvals granted.

9) ALL CONS RUCTION MUST MEET OR EXCEED ALL LOCAL CODES AND THE INTERNATIONAL CODE REQUIREMENTS AND OCCUPANCY
IS LIMITED TO THE PERMITTED AND APPROVED CLASSIFICATION. ANY CHANGE OF USE OR OCCUPANCY WOULD RESULT IN PERMIT
REVOCATION. CHANGE OF USE MUST BE APPROVED PRIOR TO CHANGE. x �-
10) Changes to approved building plans that affect compliance to the current Washington Sta a Energy Code (WSEC), ventilationrequirements),
Building/Plumbi /Mechanical Codes and/or Mason County Regulations shall be approved prior to construction.

11) CONSTR44hON PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE
ADOPTED BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in
conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a
Mason County Building Inspector shall be made prior to requesting additional inspections.

12) All buildi6prmits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The
failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being
non-compliant with Mason County ordinances and building regulations.

13) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the
time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control
of the permit holder have prevented action from being taken. No more than one extension may be granted.

14) Pressure d wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal
fasteners, connectors, and flashing. Install metal connectors approved for contact with the new types of pressure treated material.

15) No buildin r structure shall be used or occupied, and no change in the existing occupancy classification of the building or structure or portion
thereof shall be made until the building official has issued a certificate of occupancy as provided herein. Issuance of a certificate of occupancy shall
not be construed as an approval of a violation of the provisions of the codes or ordinances of the jurisdiction. Certificates presuming to give
authority to violate or cancel the provisions of this code or other ordinances of the jurisdiction shall not be valid.

16) Fire AlaraYermit Required. A full, NFPA 72 compliant fire alarm system shall be installed throughout the building under a separate permit issued
by Mason County. Complete permit applications shall be submitted with ample time for review and approval, prior to the installation of any
component of the required fire protection systems. Fire alarm permit final inspection and approval shall be obtained prior to requesting a final
inspection.

17) Electrical Permit Required. The electrical system shall be installed under a separate electrical permit issued by the Washington State Department
of Labor& Industries (L&I). Electrical"final" inspections must be approved by L&I prior to requesting a building final.

18) Fire Extinguis rs: Fire extinguishers are required for this occupancy. Minimum rating of 2-A:10-B:C required. Fire extinguisher shall be located
along a path of egress and spaced such that not more than 75 feet of travel distance exists between any point inside the building and an
extinguisher. Extinguishers shall be located not higher than 5 feet above the finished floor. Indoor fire extinguisher cabinets shall not be equipped
with locks. Extinguishers shall have a service tag verifying they have been serviced by a professional extinguisher company. If extinguisher is
recently,purchased, the tag inside the box may be affixed to the extinguisher with "NEW" and the date of purchase.

19) Speciarin n: IBC Chapter 17, Section 1704. In addition to the inspections required by IBC Section 109, the owner or the engineer or architect
of record acting as the owner's agent shall employ one or more special inspectors who shall provide inspections during construction on the types of
work listed under IBC Section 1705. The special inspectors' duties & responsibilities shall be as specified in 1704. All special inspection reports
shall be provided to the Mason County Building Department in accordance with IBC 1704.2.4. Prior to final inspection, the special inspection
agency shall also provide a final, signed report to the building department stating that all items requiring special inspection and testing were fulfilled
and reported, corrections of any discrepancies has been completed, and, to the best of their knowledge, are in conformance with the approved
design drawings, specifications, and applicable provisions of the code.
20) Special Inspection Contractor's Statement of Responsibility: Third-Party Special Inspection is required in accordance with IBC Sections 1704 &
1705 for the items identified in the Statement of Special Inspections (as required by IBC Section 1704.3). In accordance with IBC Section 1704.4,
each contractor responsible for the construction of the work listed in the statement of special inspections shall submit a written statement of
responsibility to the building official and the owner prior to the commencement of work on the system or component. The contractor's statement of
responsibility shall contain acknowledgment of awareness of the special requirements contained in the statement of special inspections.

21) Approved per dimensions and setbacks on submitted site plan that conforms with the Special Use Permit for the campus renovations. Setbacks
are m6a'syred from the furthest projection of the structure.

22) All property lines shall be clearly identified at the time of foundation inspection. X
23) All construction and demolition debris must be removed from the shore area after project completion. Proper disposal of construction debris must
be on land in such a manner that debris cannot enter or cause water quality degradation of State waters. X
24) Temporary erosion control measures must be implemented to prevent water quality degradation of adjacent waters o roperties. Silt fencing,
straw, or surface matting must be installed and maintained until upland vegetation has become established. XT_
25) Landings and stairs must meet the same setback conditions as any permitted structure; and, must be shown on yotai-site plan. Please check your
"Approve a Plan"to ensure these structures are shown and meet the setback conditions listed.

OWNER / BUILDER acknowledges submission of inaccurate information may result in a stop work order or permit revocation. Acknowledgement of such is by
signature below. I declare that I am the owner, owners legal representative, or contractor. I further declare that I am entitled to receive this permit and to do the
work as proposed. I have obtained permission from all the necessary parties, including any easement holder or parties of interest regarding this project. The
owner or authorized agent represents that the information provided is accurate and grants employees of Mason County access to the above described property
and structure(s) for review and inspection. This permit/application becomes null & void if work or authorized construction is not commenced within 180 days or if
construction work is suspended for a period of 180 days. PROOF OF CONTINUATION OF WORK IS BY MEANS OF INSPECTION. INACTIVITY OF THIS
PERMIT APPLICATION OF 180 DAYS WILL INVALIDATE THE APPLICATION.
