Mason County has reviewed the above referenced application for Class IV General Forest
Practices and finds it in conformance with all applicable County land use regulations in
effect at the time of application.
FINDINGS
The proposed FPA is compliant with the following conditions:
The proponent has paid the prescribed fees.
The proponent has filed a complete, signed application.
A map has been received that complies with the requirements of 11.05.120:
(I) Harvest boundaries and tree retention areas.
(II) The approximate location of any structures.
(III) The location of all existing and proposed streets, right-of-ways, easements, skid
roads, haul roads, and landings within the proposal.
(IV) The location of future land development including stormwater management
facilities and vegetation to be retained for site landscaping, open space,wildlife
habitat, screening, and/or buffers.
(V) Site topography at contour intervals of 40 feet.
(VI) Critical areas and critical area buffers regulated pursuant to the Critical Areas
Ordinance.
(VII) Drainage ways and culverts.
(VIII) Site area targeted for further harvest including proposed timing.
(IX) North arrow and scale shall be shown on all site plans. The scale shall be no
smaller than one inch to 200 feet.
That this FPA is in conformance with the following Mason County ordinances:
11.05
That the proposed FPA involves .75 acres clear cut and an additional 1.25 acres of
additional thinning.
That the notifications required by 11.05.060 have been made if applicable.
Note: Harvest unit is greater than 500 ft from any State Park

_X_That at the issuance of this permit will not result in a violation of the Mason
County Resource Lands and Critical Areas Ordinance. The site was inspected by
Michael MacSems of the Mason County Planning Department who determined
that any critical area buffer and/or steep slope issues have been resolved.

Envirotech Engineering and a submittal checklist was approved as complete by

* 1. ARCHAEOLOGICAL RESOURCES
Archaeological or historical resources, such as ruins, sites, buildings, artifacts, fossils, or other objects of antiquity that may
have significance from an historical or scientific standpoint,which may be encountered by the developer/contractor, shall not
be further disturbed. If the developer/contractor encounters any such artifacts, he/she shall immediately stop work and notify
Mason County and the Washington State Department of Archaeology and Historic Preservation. Failure to do so may result
in civil penalties.
2. STORM WATER& EROSION CONTROL
Escape of silty water or mud from this property shall be considered a violation of County and State storm water management
regulations and could subject the applicant(s)to civil fines or penalties.A stormwater and erosion control Minimum
Requirements 1-5 narrative shall be submitted to Mason County and approved as complete prior issuance of this permit.

* 3. HOURS OF OPERATION
Noise associated with this application shall not exceed what is allowed under Mason County Code 9.36. Harvest operations
shall start no earlier than 7:00 AM and shall end before 8:00 PM on a daily basis.
* 4. SLASH ABATEMENT
This Forest Practice Application indicates that forest debris resulting from the proposed logging operation may create an
extreme fire hazard under RCW 76.04.600 and WAC 332-24-650. An extreme fire hazard could be created if one or more of
the conditions found in WAC 332-24-650 is located within 100 feet of the running surface of a public road; or is within 500 of
an existing structure and/or designated public use area,which is not owned by the owner of the fire hazard area.
The land owner is fully liable in the event of fire, if he or she fails to appropriately abate the hazard. Extreme fire hazards
shall be abated by removing the forest debris, or by other means approved by the Washington State Department of Natural
Resources(DNR).
* 5. STREAM SETBACK
This harvest shall be setback 75'from the Ns-stream on the east side of the subject parcel.
* 6. LANDSLIDE HAZARD SETBACK
All tree cutting will be setback 10'from slopes of 40% or greater.All recommendations of a geotechnical report(#23036)and
attached email dated 6/2/2023, prepared by Envirotech Engineering, shall be observed.
* FLAGGER
As a matter of public safety, temporary"Trucks Entering Highway"signs shall be placed on the shoulder of both the
westbound and eastbound lanes of E Grapeview Loop Road during hours of operation.Additionally, when a log truck is
exiting the subject property, a flagger shall stop traffic.
* ROAD ACCESS PERMIT
No timber shall be removed from the site prior to final approval of Road Access Permit 2023-00067.

NOTE: Copies of all FPA Applications will be forwarded to Mason County Public Works for stormwater and erosion
control review.Projects which disturb more than one acre(regardless of total lot size)will require an engineered
stormwater plan consistent with the Mason County Stormwater Ordinance(Chapter 14.48.)and the 2012 WSDOE
Stormwater Manual.Applicants will be charged for Public Works staff time to review these plans and applications.
Property owners should also be aware that the Washington State Department of Ecology(WSDOE)might ask for a
NPDES(National Pollution Discharge Elimination System)Permit.The threshold for such a permit is equivalent to
Mason County's threshold.WSDOE will notify applicants through SEPA review if a NPDES is required.An NPDES is
not a perquisite for FPA approval.It is the responsibility of the applicant to obtain this permit when required.
