There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at 1-800-647-0982. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to WA state law. X 010 2) The internatioanl code requires a fire apparatus access road for every facility, building, or portion of a building that is more than 150'from an approved access road. Roads are required to meet the minimum Mason County Fire Marshal standards for Fire Apparatus Access Roads up to the point where such reads connect with a county maintained public road or to another fire apparatus access road which connects to a county maintained public road. xS 3) The proposed project must be consisten ith all applicable policies and other provisions of the Shoreline Management Act, its rules, and the Mason County Shoreline Master Program.X� 4) Xp per dimensions and setbacks on submitted site plan. Setbacks are measured from the furthest projection of the structure. 5) Conditions of Shoreline Substantial Development permit#SHR2004-00003 must be implemented as part of building permit development. X 6) All construction and demolition debris must be removed from the beach after project completion. Proper disposal of construction debris must be on land in such a manner that debris cannot enter or cause water quality degredation of State waters. X 7) W tee r,quality is not to be degraded to the detriment of the aquatic environment as a result of this project. X 8) Floata ion for the structure shall be entirely enclosed and contained to permanently prevent the breakup or loss of the floatation material into the water. X 9) Pres*ure treated wood must be completely dry prior to placement in water. Use of creosote or pentachlorophenol is prohibited. X BLD2005-01263 Please referto the following pages for conditions of this permit. 2 of 5 10) Proper erosion and sediment control practices must be used on the construction site and adjacent areas to prevent upland sediments from entering shoreline waters. Erosion control measures must be in place prior to any clearing, grading, or construction. These control measures must be effective to prevent soil from being carried into surface water by stormwater runoff. Sand, silt, and soil will damage aquatic habitat and are considered pollutants. Any discharge of sediment-laden runoff or other pollutapollutants to waters of the state is in violation of Chapter 90.48 RCW, Water Pollution Control, and WAC 173-201A, Water Quality Standards for Surface Waters of the State of Washington, and is subject to enforcement action. Any work in or adjacent to waterways that will adversely affect water quality must receive specific prior authorization from the Department of Ecology pursuant to WAC 173-201A-110. A short-term water quality standards modification may be issued if the proponent agrees to a number of specific construction practices and techniques designed to minimize water quality impacts. All areas disturbed or newly created by construction activities must be revegetated using bipneering techniques, clean durable riprap, or some other equivalent type of protection against erosion when other measures are not practical. X 11) Erosion control measures must be in place prior to any clearing, grading or construction These control measures must be effective to prevent stormwater runoff from carrying soil and other pollutants into surface water or storm drains that lead to waters of the state. Sand, silt, clay and other particles and soil will damage aquatic habitat and are considered pollutants. Prior to final approval, all upland areas disturbed or newly created by construction activities shall be seeded, vegetated or given an equivalent type of erosion protection (silt fencing or straw matting).X t 12) During construction, all release of oils, hydraulic fluids, fuels, other petroleum products. paints, solvents and other deleterious materials must be contained and removed in a manner that will prevent their discharge to waters and soils of the state. The cleanup of spills should take precedence over other work on the site. X t 13) ReQoviaLof vegetation should be minimized and any areas disturbed should be restored to prevent erosion and other environmental impacts. X 14) Planting Plan on page 28 of Biological Evaluation prepared by Marine Surveys and Assessments dated July 26, 2004 must be implemented within two year o ' suance of shoreline permit. Plantings to be maintained in a healthy condition. X 15) The project shall be consistent with the Biological Evaluation and/or Habitat Management Plan and Best Management Practices as conditioned and apTO 16) All other necessary permits from Mason County, Washington State and/or Federal Agencies that a[g required for this proposed development and construction must be obtained PRIOR TO SAME DEVELOPMENT AND CONSTRUCTION. X 17) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building Dgn ent prior to any further inspections being performed or approvals granted. x BLD2005-01263 Please referto the following pages for conditions of this permit. 3 of 5 18) In accordance with international codes and Title 14, Mason County Building Code, "Standards for Fire Apparatus Access Roads,"all new structures that require an address shall have approved numbers or addresses located at the beginning of long driveways when the address is not clearly visible from the access road. The numbers shall also be plainly visible and legible from the street or road fronting the property and shall contrast with their background. Mason County Building Department requires that this be completed prior to calling for any site inspections. A re-inspection fee based on rates as adopted by the jurisdiction and the international codes will be assessed if the owner and/or contractor fail to post the address on site prior to requesting inspns. X 19) The plan review check list and corrections are part of the approved plans and must remain thereto. It is the responsibility of the applicant to make the corrections indicated on the plans. Once the plans are marked "APPROVED", they shall not be changed or altered without authorization from the Building Official. The permit holder is responsible to retain the complete approved set of plans on site for the duration of the project. Failure to comply and/or rerrgvpf approved documents will result in failure of required building inspections. X 20) The "approved"site plan is required to be on-site for inspection purposes. If an inspection is requested and the"approved"site plan is not on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and shall be collected by the Builg�Department prior to any further inspections being performed or approvals granted. X �-� 21) Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to the Mason County Building Department prior to construction. All engineering and/or architectural documents are a part of the approved set of plans and shall remain attached thereto. If documents are removed, approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be chargeq and shall be collected by the Building Department prior to any further inspections being performed or approvals granted. X -t 22) All construction must meet or exceed all local ordinances and the international codes requirements as adopted and amended by Mason County and the State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in per Wirocation. 23) International Building Code. In addition to the inspections required by Chapter 1 of the International Building Code, the owner or the engineer or architect of record acting as the owner's agent shall employ one or more special inspectors who shall provide inspections during construction on the types of work listed der IBC, section 1704. The special inspectors'duties & responsibilities shall be as specified in Chapter 17. X (}�� 24) All changes to "approved" building plans that effect compliance with the international codes as amended and adopted, or any other Mason County Xrd' or regulation, must be reviewed and approved by Mason County prior to construction. 25) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building XI ec r shall be made prior to requesting additional inspections. X 27) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with MasoQCounty ordinances and building regulations. X 28) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit hold_er�iave prevented action from being taken. No more than one extension may be granted. X 29) Pressure treated wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fasteners, co�tors, and flashing. Install metal connectors approved for contact with the new types of pressure treated material. X This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended fora period of 180 days at anytime after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period.
