The Geotechnical Report for RONALD COLE has been received and reviewed by the Planning
Department. The report was prepared by Curtis Cushman dated 8/8/2013.
Based on the certification provided by the licensed engineer/geologist, the referenced
Geotechnical Report was prepared in general accordance with the requirements in the Mason
County Resource Ordinance, Landslide Hazard Areas 17.01.100.E.5. Mason County considers the
review valid until such time as scope of project, site conditions, and/or regulations change. Should
the scope of work, site conditions, and/or regulations change after the original review, then an
addendum from the original author of the report may be required to address these changes. The
report would only be re-reviewed if a permit for development were submitted after these changes
occur. Mason County does not certify the quality of the work done in this Geotechnical Report.

Dear Mr. Cole:
As per our conversations, I have reviewed the report for the parcel in question dated November 29, 2007
prepared by Geotechnical Testing Laboratory and addressed to Ron Cole. I have prepared this addendum
to the original geotechnical report to fulfill the requirements of Mason County Planning. This addendum
should be submitted with the original report and the required checklist. This is expected to satisfy their
requirements. I have provided three copies for your review and distribution.
BACKGROUND
A geotechnical report was prepared on November 29, 2007 for the above-mentioned parcel. The report
indicated that the site is suitable for construction provided that all the recommendations are met.
This addendum updates the report,answering questions posed by Mason County Planning.
SITE INVESTIGATION
No revisit was required.
ADDENDA
All information will be referenced to the Mason County checklist.
Section (1) subsection (d): Upslope geomorphology is a broad and shallow plateau, posing no threat to
the construction site.
Section (1) subsection (f): Based on field observations, the CZAW map, and the Shelton quadrangle
map,there is no evidence of recent landslide activity in this area.
Section (8): Some specific vegetation recommendations are located in the report (such as Erosion
Control). These recommendations anq the following are general and maybe modified subject to approval
or do not apply. "No disturb"zones will not be needed,
A revegetation plan should be in place before the end of construction. Native species should be
considered as the primary vegetation in areas away from lawn and ornamental plantings. Shallow rooted
species, such as grass, should be planted closer than 10 feet from underground drainages.
Vegetation should be chosen and planted to minimize excessive watering and any erosion of the bluff.

Densely rooted evergreen shrubs are preferable than tree species on slopes greater than 15% gradient.
Tall trees may become unstable in wet soils under high wind conditions.
Section 10 : No impacts are expected if the recommendations in the report are followed along with

standard building practices and Mason County requirements.
Section (12): The existing cabin will not be removed. There is no structural or other mitigation.
The report is still valid and,with this addendum,fulfils the requirements of Mason County Planning

We have prepared this addendum,which along with the original Geotechnical report dated November 29,
2007, is for the exclusive use of Ron Cole and his authorized agents. This addendum does not specify
setbacks for: line-of-sight setbacks, FWHCA setbacks, eagle tree setbacks,wetland setbacks, or property
line setbacks. Within the limitations of scope, schedule and budget, our services have been executed in
accordance with generally accepted practices in the field of geotechnical engineering in this area at the
time this report was prepared. No warranty or other conditions, expressed or implied, should be
understood.

This checklist is intended to assist Staff in the review of a Geotechnical Report. The Geotechnical Report
is reviewed for completeness with respect to the Resource Ordinance. If an item is found to be not
applicable, the Report should explain the basis for the conclusion. The Report is also reviewed for clarity
and consistency. If the drawings, discussion, or recommendations are not understandable, they should be
clarified. If they do not appear internally consistent or consistent with the application or observations on
site, this needs to be corrected or explained. If resolution is not achieved with the author, staff should
