1) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance Division.

3) The owner or the owner's agent shall employ one or more special inspectors who shall provide inspections during construction on the types of work
specified by the design professional. The special inspectors duties & responsibilities shall be as specified in Chapter 17.
Special inspections and structural observation shall be performed in accordance to specifications listed on sheet S1, Section 3.0. Structural observation
shall be performed by the engineer of record, Dale Kvamme, or his authorized representative. Structural observation inspections shall be performed to
verify proper installation of shearwalls, holdowns, concrete construction, masonry construction, and structural steel construction, as needed. in IBC,
Section 109, the owner, the engineer or architect of record acting as the owner's agent shall employ one or more special inspectors who shall provide
inspections during construction on the types of work listed under Chapter 17 and as specified by the design professional. The special inspectors duties &
responsibilities shall be as specified in Chapter 17.
Special inspection reports shall be submitted to the Mason County Building Department, PO Box 186, Shelton WA 98584 and available for inspection.
Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing and final occupancy
inspections.

7) The plan review check list and corrections are part of the approved plans and must remain thereto. It is the responsibility of the applicant to make the
corrections indicated on the plans. Once the plans are marked "APPROVED", they shall not be changed or altered without authorization from the Building
Official. The permit holder is responsible to retain the complete approved set of plans on site for the duration of the project. Failure to comply and/or
removal of appro d documents will result in failure of required building inspections.

8) The "approved" site plan is required to be on-site for inspection purposes. If an inspection is requested and the"approved" site plan is not on site, then

9) Washington State Energy Code Compliance has been approved using the following:
Heat Type: LP Furnace, Compliance Method: Prescriptive option IV, Window (Max U-Factor):0.35, Skylight(Max U-Factor):0.58, Doors (Type/Max
U-Factor):0.20 or less, Wall insulation R-21, Floor insulation R-30, Ceiling Insulation R-38, Vault Insulation R-38 (see exception), Slab Insulation R-10.
Exception: R-30 insulation may be installed, up to 500 sq. ft., in single rafter or joist vaulted ceilings where the distance of the top of the ceiling and the
underside of the nof sheathing is less than 12-inches and there is 1-inch vented airspace above the insulation.

10) Per 2003 IRC - SECTION 1609 -WIND LOADS- 1609.1 Applications. Buildings, structures and parts thereof shall be designed to withstand the
minimum wind loads prescribed herein. Decreases in wind load shall not be made for the effect of shielding by other structures. Per FIGURE 1609
BASIC WILD EED (3-SECOND GUST) the wind speed for Mason County is 85 MPH.

11) Per IRC -SECTION R905 - REQUIREMENTS FOR ROOF COVERINGS - R905.1 Roof covering application. Roof coverings shall be applied in
accordance with the applicable provisions of this section and the manufacturer's installation instructions.

12) A Mason County Stormwater Management Worksheet was completed and signed as part of this building permit application. Design, sizing, placement,
inspection and maintenance of stormwater management systems shall be the responsibility of the owner/agent of the developed parcel. It is the
owner/agent/contractor's responsibility to ensure that Mason County Department of Public Works has approved the stormwater site plan for this parcel
prior to the commencement of any development activities. *NOTE if Stormwater Management option "A" was selected on the Small Parcel Stormwater
Management Application/Worksheet the document entitled "Managing Storm Drainage on Small Lots, The Small Parcel Stormwater Site Plan" constitutes
an approved plan based on the criteria listed on the application/worksheet. If the development has, or will have, a septic/drainfield system you are
responsible for contacting Mason County Division of Environmental Health to ensure that the stormwater system will not adversely affect the septic system
of this, or any other, parcel. You may also wish to consult with the septic design professional involved with the project. By calling for a final inspection of
the building permit the owner/agent/contractor is a W
edging that all components of the stormwater management system have been installed as
approved on the stormwater site plan. X
13) Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to the Mason County Building
Department prior to construction. All engineering and/or architectural documents are a part of the approved set of plans and shall remain attached
thereto. If documents are removed, approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be

14) All construction must meet or exceed all local ordinances and the international codes requirements as adopted and amended by Mason County and the
State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in
permit revocation

r flammable and combustible liquids in excess of 10 gallons is not allowed without the approval of
the Mason County Fire Marshal. X
16) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely impact
adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements of the
stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose. For further
information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or access
connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450. For any construction which
is proposed to be located within 25' of a Mason County road right of way, it is suggested to contact that office to review future planned work which may
affect your project.

18) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED
BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance
with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building
Inspector shall be mada prior to requesting additional inspections.

19) All propane tanks must be installed in accordance with the International Fire Code and all applicable Mason County ordinances. All propane tanks must
meet the installation ments and minimum setbacks as listed in the Mason County Fire Marshal's Standards for the Installation of Propane Tanks.

20) All property lines shall be clearly identified at the time of foundation inspection. X
21) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure
to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with
Mason County di n es and building regulations.

22) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for
action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit
holder have prevent action from being taken. No more than one extension may be granted.

shall be prepared by the engineer of record, Kvamme Consulting, responsible for the structural design as evidenced by the Special Inspection
Authorization form attached to the approved building plans.
Reports shall be submitted to the Mason County Building Department within 2 weeks following each structural observation. A final observation report shall
be submitted prior to the final inspection of the structure. Written statements shall indicate the permit number, type of inspection, and identify any
reported deficiencies.
Structural observation is defined as the visual observation of the structural system by a registered design professional for general conformance to the
approved confor ance to the approved construction documents at significant construction stages and at the completion of the structural system.

27) The basement of this structure is approved as un-heated space. If at any time this building is to be used for anything other than what it is approved for, a
change of use erm hall be applied for, reviewed and approved prior to the change.

This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at any time after work is
commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of continuation of

In addition to the inspections required by International Building Code (IBC) Section 109, the owner or the engineer
or architect of record acting as the owner's agent shall employ one or more special inspectors who shall provide
special inspections during construction on the types of work listed under IBC Section 1704.
This form must be completed by the owner or the engineer or architect of record, prior to permit issuance, indicating
who will perform the required special inspections for this particular project. The specified special inspector(s) shall
perform the required special inspections during construction of the project, submit all inspection reports to the
Building Department and provide a final signed report stating whether the work requiring special inspection was, to
the best of the inspector's knowledge, in conformance to the approved plans and specifications and the applicable
workmanship provisions of the IBC. The following checked items require special inspection:
1. Concrete. During the taking of test specimens and placing of reinforced concrete. (IBC 1704.4)
2. Bolts installed in concrete. Prior to and during the placement of concrete around bolts when stress
increases permitted by Table 1912.2 and section 1912.5 are utilized. (IBC 1704.4)
3. Erection of precast concrete members. Periodic inspection per ACI 318: Ch 16. (IBC 1704.4)
4. Shotcrete. Continuous inspection per ACI 318: 5.9. 5.10 and IBC Chapter 19. (IBC 1704.4)
5. Reinforcing steel and prestressing steel tendons. Inspection of reinforcing steel including prestressing
tendons. (IBC 1704.4)
6. Welding of reinforcing steel. Inspection of the welding of reinforcing steel according to AWS D1.4, ACI
318: 3.5.2, and IBC 1903.5.2. (IBC 1704.4)
7. Structural welding. Welding inspection shall be in compliance with AWS D1.1. (IBC 1704.3.1)
8. High-strength Bolting. The installation of high-strength bolts shall be periodically inspected in accordance
with AISC LRFD Section M2.5. (IBC 1704.3.3)
9. Structural Masonry. Masonry construction shall be inspected and evaluated with the requirements of IBC
section 1704.5.
10. Soils. Inspections for fills 12 inches or greater to be performed in accordance with IBC Section 1704.7.
11. Pile foundations. Inspector to be present when pile foundations are being installed and during tests. (IBC
1704.8)
12. Pier foundations. Special inspection of pier foundations for buildings are required in seismic Zone D. (IBC
1704.9)
13. Spray-applied fire resistant materials. Inspections shall be in accordance with sections 1704.11.1
through 1704.11.5.
14. Exterior insulation finish system(EIFS). Special inspection shall be required for all EIFS systems. (IBC
1704.12)

15. Smoke-control system. Smoke control systems shall be tested by a special inspector per IBC Section
1704.14.
16. Special cases. Special inspection shall be required for proposed work that is, in the option of the building
official, unusual in its nature such as alternative methods and material of construction, unusual design applications,
and materials and systems required to be installed in accordance with additional manufacturer's instructions that
prescribe requirements not contained in the IBC or referenced standards.

PURPOSE OF SPECIAL INSPECTION FOR INSPEC,110N
Special inspection is the monitoring of materials and workmanship which are critical to the integrity of the building
structure to assure that the approved plans and specifications are being followed and that relevant codes are being
observed. Special inspections are in addition to those conducted by the county building inspector and by the
engineer or architect of record. Special inspectors furnish inspection at all times that their presence is required by
the code, the plans and specifications, and the enforcing jurisdiction. Good communication between the special
inspector and the designer, contractor, and building department is essential.
DUTIES AND RESPONSIBILITIES OF THE SPECIAL INSPECTOR:
Special inspectors are individuals with highly developed. specialized skills who observe those critical building or
structural features which they are qualified to inspect. Duties of special inspectors and/or inspection agencies
include the following:
1. General Requirements
Special inspectors shall review approved plans and specifications for special inspection requirements.
Special inspectors shall comply with the special inspection requirements of the enforcing jurisdiction.
2. Signify Presence at Job Site
Special inspectors should notify contractor personnel and the enforcing jurisdiction of their presence and
responsibilities at the job site.
3. Observe All Work for Which They Are Responsible
Special inspectors shall inspect all work and perform or observe all tests requiring inspection and testing for
conformance with the building-department-approved (stamped) drawings, specifications and applicable
provisions of the code.

Dear Mr. Pinkney,
Thank you for submitting the building permit application referenced above. In order to complete
the building department review additional information will be needed which is listed below.
PLAN REVIEW REQUIREMENT COMMENTS:
1. The building permit application references two parcel numbers that the structure will be placed on.
In order to approve the building permit all property lines must be shown and setbacks shall be
measured from the parcel property line. If the proposed structure will be placed over an existing
property line that property line must be removed, moved or adjusted in order to comply with
Mason County requirements. Please contact Mr. Michael MacSems, Mason County Planning
Department at (360)427-9670 ext. 571 to discuss what will be required .
2. The following comments are related to the design calculations and"S"pages prepared and
stamped by Kvamme Consulting, Inc. Sheet S 1 lists special inspection requirements and
structural observation completed by the engineer of record. Please complete the special inspection
authorization form enclosed designating and acknowledging the special inspector and structural
observer as required by the engineer of record. During plan review staff reviews plans to verify
that all specifications within the design calculations are placed on the plans. While reviewing the
design calculations I was not able to verify that all structural members are noted on the plans. In
addition the lateral design and building plans designate a shear wall "3"but I was not able to
locate the specification for shear wall 3 on the shear wall schedule. Please provide specifications.
3. I did not locate framing details for the fireplace. Sheet 15 shows that the fireplace will be faced
with stone veneer that appears to exceed the allowable referenced in the 2006 International
Residential Code (IRC), Section R703.7 for seismic zone D2. Please review the referenced code
section and verify compliance. If prescriptive compliance cannot be obtained please arrange for a
design prepared by the engineer of record. A copy of Section R703.7 is enclosed.
4. The Washington State Energy Code(WSEC)compliance form designates the basement as
unheated space however it appears on sheet 20 that the basement will be heated. Please verify.
When you have compiled the requested information please submit it to the Mason County Building
Department, attention D. Coker. Please respond to comments,in writing, in the order listed. Revisions,
required as a result of this letter must be transferred onto proposed building plans. Where required,please
return two sets of revised plans and documents along with two sets of calculations. If you have questions
