CONDITIONS OF THIS DETERMINATION
1) Proponent shall obtain all necessary state and federal permits prior to the commencement
of any work.
2) Residential developers and individuals shall be required to control erosion during
construction. Removal of vegetation should be minimized and any areas disturbed should
be restored to prevent erosion and other environmental
impacts.X
3) The proposed bulkhead repair-replacement shall not occur waterward of the existing
bulkhead.

Work to be completed ASAP after receiving all required permits.
7. Do you have any plans for future expansion, or further activity related to or
connected with this proposal: if yes, explain. No.

prepared, or will be prepared, directly related to this proposal. None have
been prepared.

approvals of other proposals directly affecting the property covered by
your proposal? If yes, explain. No.
10. List any government approvals or permits that will be needed for your
proposal, if known. A Mason County shoreline exemption permit, a JARPA,
and a Mason County Building Permit, and an HPA from WDFW.
11. Give a brief, complete description of your proposal, including the proposed
uses and the size of the project and site. There are several questions
later in this checklist that ask you to describe certain aspects of your
proposal. You do not need to repeat those answers on this page. (Lead
agencies may modify this form to include additional specific information on
project description).

Fifty-five feet(55') of an existing 110' rock bulkhead will be reset/repaired
using recycled large angular rock. One set of shared inset beach access
steps will be installed on the adjacent property to the north (parcel # 12228-
44-9024). Base and Lower half rock size will be four and five man sized.
Upper half rock will be two man or larger.
The bulkhead will be approx. five feet above grade in height. Base rock to
be keyed into the beach a minimum of 18-24" below the existing beach
grade. Bulkhead to be backed with 6"-8" quarry spalls and filter fabric as
necessary. Quarry spalls are the appropriate drainage rock for a marine
bulkhead as smaller rock or gravel will be sucked through the interstitial
spaces of the bulkhead, causing it(bulkhead) to fail.
Some overhanging vegetation will need to be trimmed to facilitate
construction. Existing large woody debris on the beach will be left on the
beach. All manmade debris on the beach will be removed from the site and
disposed of at a licensed disposal facility following construction. All
activities are within the 100-year flood plain. The nearest waterbody is
North Bay/Case Inlet. Equipment and materials will be transported to the
site by barge.
12. What is the location of the proposal? Give sufficient information for a
person to understand the precise location of your proposed project
including a street address, if any, and section, township, and range, if
known. If a proposal would occur over a range of area, provide the range
or boundaries of the site(s). Provide a legal description, site plan, vicinity
map, and topographic map, if reasonably available. While you should
submit any plans required by the agency, you are not required to duplicate
maps or detailed plans submitted with any permit applications related to
this checklist. The site is located at 5657 E SR 302. Section 28, Township
22N, Range 01W. The legal description is: TR 2-C OF GOVT LOT 4, E OF
R/W PCL 3 BLA#97-04 AF#640530.
B. ENVIRONMENTAL ELEMENTS:
1. EARTH:
a. General description of the site (circle one): Flat, rolling, hilly, steep slopes,
mountainous, other.
b. What is the steepest slope on the site (approx. percent slope)? Greater
than 15 percent from the top of the shoreline bank to the beach toe.

C. What general types of soils are found on the site (for example, clay, sand,
gravel, peat, muck)? If you know classification of agricultural soils, specify
them and note any prime farmland. Sand, gravel, and clay.
d. Are there surface indications of history of unstable soils in the immediate
vicinity: If so, describe. Yes, the stretch of SR 302 that the site is located
on has a history of being unstable, and it was closed last summer for
almost one month while WSDOT conducted repairs.
e. Describe the purpose,type, and approximate quantities of any filling or grading
proposed. Indicate source of fill. To protect the bank toe, an existing rock
bulkhead will be reset and augmented with approx. 45 cu/yds of
large/medium rock and quarry spalls that will be placed along the bluff toe.
f. Could erosion occur as a result of clearing, construction, or use: If so,
generally describe. The HPA provisions and the WDOE's BMPs shall be
followed to prevent erosion during construction.
g. About what percentage of the site will be covered with impervious
surfaces after project constructions (for example, asphalt or buildings)?
None. The rock bulkhead will be pervious to water.
h. Proposed measures to reduce or control erosion, or other impacts to the
earth, if any. The HPA provisions and the WDOE's BMPs will be
implemented during construction.
2. AIR:
a. What types of emissions to the air would result from the proposal (i.e.
duct, automobile, odors, industrial wood smoke) during construction and
when the project is completed? If any, generally describe and give
approximate quantities, if known. N/A.
b. Are there any off-site sources of emissions or odors that may affect your
proposal? If so, generally describe.
NIA.
C. Proposed measures to reduce or control emissions or other impacts to air,
if any: N/A.
3. WATER:
a. Surface:

1) Is there any surface water body on or in the immediate vicinity of
the site (including year-round and seasonal streams, saltwater,
lakes, ponds, wetlands)? If yes, describe type and provide names.
If appropriate, state what stream or river it flows into.
Yes, North Bay/Case Inlet.
2) Will the project require any work over, in, or adjacent to (within 200
feet) the described waters? If yes, please describe and attach
available plans. Yes, approx. 55' of an existing 110-foot rock
bulkhead will be reset/repaired on the shoreline.
3) Estimate the amount of fill and dredge material that would be placed in or
removed from surface water or wetlands and indicate the area of the site
that would be affected. Indicate the source of fill material. To protect
the bank toe, an existing rock bulkhead will be reset and augmented
with approx. 45 cu/yds of largelmedium rock and quarry spalls that
will be placed along the bluff toe.
4) Will the proposal require surface water withdrawals or diversions?
Give general description, purpose, and approximate quantities if
known. No.
5) Does the proposal lie within a 100-year floodplain? If so, note
location on the site plan. Yes, the proposed project is located on
the Puget Sound shoreline.
6) Does the proposal involve any discharges of waste materials to
surface waters? If so, describe the type of waste and anticipated
volume of discharge. No.
b. Ground:
1) Will ground water be withdrawn, or will water be discharged to ground
water? Give general description, purpose, and approximate quantities,
if known. N/A.
2) Describe waste material that will be discharged into the ground from
septic tanks or other sources, if any (for example: domestic sewage,
industrial, containing the following chemicals...,agricultural, etc.).
Describe the general size of the system, the number of such systems,

the number of houses to be served (if applicable), or the number of
animals or humans the system(s) are expected to serve. None will be
discharged.
C. Water runoff (including storm water):

Any runoff would be from precipitation. WDOE's BMPs shall be
adhered to during construction
2) Could waste materials enter ground or surface waters: if so,
generally describe. No.
d. Proposed measures to reduce or control surface, ground, and
runoff water impacts, if any: The HPA provisions and the WDOE's
BMPs will be followed during construction.
4. PLANTS:

The Chinook salmon, the bull trout, the Puget Sound steelhead, the killer
whale, the leatherback turtle, the humpback whale, the marbled murrelet,
the green sturgeon, and the Puget Sound rock fish are some of the ESA
listed threatened or endangered species in the area.
C. Is the site part of a migration route? If so, explain. Fish migrate through
Case Inlet on their way to spawning streams.
d. Proposed measures to preserve or enhance wildlife, if any: None at this
time.
6. ENERGY AND NATURAL RESOURCES:
a. What kinds of energy (electric, natural gas, oil, wood stove, solar) will be
used to meet the completed project's energy needs? Describe whether it
will be used for heating, manufacturing, etc.

b. Would your project affect the potential use of solar energy by adjacent

this proposal? List other proposed measures to reduce or control energy
