HARVESTING AND GROUND DISTURBANCE OF APPROXIMATELY 5.5
TO 6 ACRES OF TIMBER ON A 162.61-ACRE LOT. NO CLEARING IS EXPIRES: 06/24/2026
PROPOSED WITHIN 200 FEET OF THE OHWM OF OAKLAND BAY
(SOUTH PUGET SOUND). THE HARVESTED AREA WILL BE
CONVERTED TO GRAVEL MINING,WITH NO FILLING PROPOSED
OTHER THAN REPLACING TOPSOIL AND SUBSOIL. RECLAMATION
OF THE SITE WILL EVENTUALLY BE PERFORMED IN
CONFORMANCE WITH THE DNR APPROVED REVISED

2. STORM WATER&EROSION CONTROL.Escape of silty water or mud from this property shall be considered a
violation of County and State storm water management regulations and could subject the applicant(s)to civil fines or
penalties. The approval of this project is subject to the recommendations and specifications outlined in the attached
Construction Stormwater Pollution Prevention Plan, Drainage Report, and Maintenance and Operation Manual, all prepared
by Envirotech Engineering PLLC, dated April 1,2024.All applicable recommendations and specifications shall be applied to
the development on this site. During the logging period,the erosion &sediment control facilities shall be upgraded as
needed for unexpected storm events and to ensure that sediment and sediment-laden water do not leave the site.

3. HOURS OF OPERATION. Noise associated with this application shall not exceed what is allowed under Mason County
Code 9.36. Harvest operations shall start no earlier than 7:00 AM and shall end before 8:00 PM on a daily basis.
4. SLASH ABATEMENT. This Forest Practice Application indicates that forest debris resulting from the proposed logging
operation may create an extreme fire hazard under RCW 76.04.600 and WAC 332-24-650.An extreme fire hazard could be
created if one or more of the conditions found in WAC 332-24-650 is located within 100 feet of the running surface of a
public road; or is within 500 of an existing structure and/or designated public use area,which is not owned by the owner of
the fire hazard area. The land owner is fully liable in the event of fire, if he or she fails to appropriately abate the hazard.
Extreme fire hazards shall be abated by removing the forest debris, or by other means approved by the Washington State
Department of Natural Resources(DNR).

5. GEOTECHNICAL REPORT.The approval of this project is subject to the recommendations and specifications outlined
in the attached Geotechnical Report, prepared by Envirotech Engineering PLLC, dated March 23, 2024. All applicable
recommendations and specifications shall be applied to the development on this site.Any deviation requires stamped written
approval from the registered design professional responsible for the report and may require special inspection by same.
Structures and/or land modifications(grading,cuts,fills, etc.)required in the geotechnical report, may require a separate
permit.The Geotechnical Report shall remain attached to the approved Forest Practices Approval permit.
6. CLEARING LIMITS. The boundaries of the clearing limits shown on the submitted site plan shall be clearly flagged in
the field prior to logging. During the logging period, no disturbance beyond the flagged clearing limits shall be permitted.The
flagging shall be maintained by the appiicant/contractor for the duration of'construction.

* 7. MAINTAIN BMP'S.All temporary and permanent erosion &sediment control BMP's shall be maintained and repaired as
needed to assure continued performance of their intended function. All temporary erosion and sediment control BMP's shall
be removed within 30 days after the final site stabilization is achieved or after the temporary BMP's are no longer needed.

The harvest unit is greater than 500 feet from a County or State Park, therefore,
Mason County Parks and Trails and Washington State Parks were not notified.
The harvest unit is located on a site known to contain historic/archaeological
resources, therefore, the Washington Department of Archaeology and Historic
Preservation (DAHP) was notified of the project and sent a copy of the
application on June 3, 2024. No comments from DAHP were received. The
harvest unit is located on a site known to contain cultural resources; therefore,

the Spuoxin Island Tribe was notified of the project and sent a copy of the
application on June 3, 2024.A comment was submitted by Shaun Dinubilo on
June 4, 2024, with a recommendation that a Cultural Resource Survey be
completed prior to any forest practices activities. Manke Family Resources
coordinated with Shaun Dinubilo and shared documentation from past projects
on the site. Upon receiving additional information, Shaun amended his
recommendation to "no concern"on June 19, 2024.
c) Granting project approval will not be detrimental to public health, safety
and general welfare.

improvements adjacent to and in the vicinity of the proposal.

Mason County Resource Ordinance
The issuance of this permit will not result in a violation of the Mason County Resource
Lands and Critical Areas Ordinance. The site was inspected by Luke Viscusi, of the Mason
County Planning Department, who determined that any critical area buffer and/or steep
slope issues have been sufficiently documented in the Geotechnical Report, prepared by
Envirotech Engineering, PLLC, dated March 23, 2024. Almost all project activities are
within the landslide hazard area, which was determined safe for clearing and stumping if
complying with the recommendations of the Geotechnical Report(see below). There is a
stream mapped by the DNR which runs through the proposed harvest unit. No stream or
stream channel was observed on the site and there are no culverts mapped by the WA
Department of Fish and Wildlife in the area around the proposed harvest unit.

Geo-Technical Report/Assessment Review
A Geotechnical Report was prepared by Envirotech Engineering, PLLC, dated March 23,
2024, and a submittal checklist was approved by Luke Viscusi, of the Mason County
Planning Department, as being complete on May 17,2024. The entirety of the clearing
limits are within a Landslide Hazard Area. No buffer from the Landslide Hazard Area was
recommended in the Geotechnical Report

State Environmental Policy Act
This application for FPA2024-00001 has been reviewed for SEPA compliance. It was
determined by Luke Viscusi, of the Mason County Planning Department, that this
proposal had been previously evaluated for environmental impacts though a DEIS
(1991), SETS(1992), FEIS(1995), and DNS(2017). While SEPA review had already been
conducted for this proposal, County staff still notified the tribal and government entities
which would have been notified of a SEPA threshold determination. The following
entities were notified of the project and sent project documentation on June 3, 2024:
Squaxin Island Tribe, WA Department of Natural Resources, Mason County Public
Utilities District 3, WA Department of Fish and Wildlife, WA Department of Archaeology
and Historic Preservation, Olympic Regional Clean Air Agency, WA Department of
Ecology, and Manke Family Resources LLC. The aforementioned entities were also sent a
signed SEPA Adoption Form on June 3, 2024, and County staff adhered to all the
requirements outlined in WAC's 197-11-600, 197-11-630, and 197-11-708, even though
"adoption"is not necessarily outlined as the necessary procedure in cases where a
project action has already been evaluated through SEPA

Stormwater Management
An engineered storm water plan for Mason County Public Works is NIA due to the fact
that the project doesn't convert any disturbed acreage to new/replaced hard surfaces,
lawn, landscape area, or pasture. However, the applicant submitted more than the
required documentation, which included a Construction Stormwater Pollution Prevention
Plan, a Drainage Report, and a Maintenance and Operation Manual, all prepared by
Envirotech Engineering PLLC, dated April 1, 2024. The submitted documentation included
narratives for Minimum Requirements#1 through#5 that meet the standards of the
2019 Stormwater Management Manual for Western Washington.

NOTE: Copies of all FPA Applications will be forwarded to Mason County Public Works for stormwater and erosion
control review. Projects which disturb more than one acre(regardless of total lot size)will require an engineered
stormwater plan consistent with the Mason County Stormwater Ordinance(Chapter 14.48.)and the 2012 WSDOE
Stormwater Manual.Applicants will be charged for Public Works staff time to review these plans and applications.
Property owners should also be aware that the Washington State Department of Ecology(WSDOE)might ask for a
NPDES(National Pollution Discharge Elimination System)Permit. The threshold for such a permit is equivalent to
Mason County's threshold. WSDOE will notify applicants through SEPA review if a NPDES is required.An NPDES is
not a perquisite for FPA approval. It is the responsibility of the applicant to obtain this permit when required.
