_PTI representative on site as requested for special inspection of concrete reinforcing steel and soil
compaction.
Per the contractors request PTI verified/witnessed that the area along the east wall, north of the west main
entrance porch was being compacted with a roller vibrating compactor as it was being filled in. This is
going to be a landscaped area continuing around the east side to the north side of the structure. Full time
observation was not requested. PTI was able to use a T-probe to verify firm unyielding soil for an area
from the porch to another approximate 20' going north. Per the contractor the same method of
compaction will continue for this entire landscape area.
Concrete reinforcing steel was placed for the front (west) entrance footing per detail 8/S3.0 of plan. PTI
verified resteel size, grade, spacing, lap and clearances. No deficiencies were noted.
To the best of my knowledge, the above was performed in accordance with the approved plans,
specifications, and regulatory requirements.

Observed placement and compaction (with an MBW R40 smooth drum vibratory roller with rear rubber
tires, 3700 lbs. name plate weight) of west wall backfill, south half. Backfill was placed in 6" lifts and
appeared compacted to a firm and unyielding condition, approximately 12" of fill was placed over top
of fabric covering drain rock.
Note: Unable to perform density tests due to over 30% of fill material retained on 3/4" sieve; see sample
1187 on 10-19-10 and geotechnical report by Bradley-Noble dated 4-6-10.
Note: Unable to proof roll backfill due to proximity of concrete wall and existing slope.

Per the contractors request an inspection was made of the nailing of wood sheeting for the first 4' at the
bottom of walls from the start of wall at grid C/3 wrapping the entire front perimeter to H/2.5 with special
note to shear walls at grids 2/G - G.2 and G.4/1 - 1.2 and G.4/2 - 2.1 and 1/G.4 - G.5 and 1/G.8 - H. Wall
sheeting and shear wall sections were nailed per general notes shown and SW-schedule on sheet S1.0
of plan. PTI verified nailing size and spacing at boundaries, edges and field. Hold downs and anchor
bolts were also in place per plan. Straps at window openings and headers to top plates were also in place
per plan.
No deficiencies were noted.

PTI representative arrived on site as schedWed for resteel inspectton -and concrete placement
inspection.
_Walls at exterior range up to 15'. Due to forms being in place steel from grids A - H, lines 3 - 4, could not
be verified for lap, clearance, and placement. Laps at top are not staggered. 5' of wall at grid G.8, line .5,
has (3) #5 bars epoxied into footing. No inspection of epoxy and bar placement was done.
Interior column pads and interior wall footings meet details of plans S3.1 for size, grade, lap, and
clearances.
Engineer and county inspectors were on site on 12-9-10 and inspected wall steel. Deficiencies were
noted.
I spoke with engineer and he will send a letter for wall steel signing it off.
Observed placement of 45 yards of mix #0645AF6. (1) set of 4" x 8" samples taken for lab compression
testing.
Observed placement of wet set sill plate bolts per plans.
To the best of my knowledge, the above WAS NOT performed in accordance with the approved plans,
specifications, and regulatory requirements.

Inspector performed thirty IPD-soil tests for compaction at six different locations of the building site (five
tests per site). Tests #1, 6, 11, 16, and 21 did not meet required 95% compaction. Tests #5, 10, 15,
20, and 25 were for same location at same base grade and met required compaction. All remaining tests
met required compaction for structural backfill.

PTI representative on site as requested for special inspection of soil compaction.
Imported fill, (crushed approx. 1"minus, certifications and supplier information on file at McBride job trailer
provided for review.) was placed per the contractor in 8" lifts with a depth ranging from 8" on the east end
of excavation to 3' in places in the middle to west end of excavation. Material was compacted with a roller
compactor (approximately 25,000 lb.). Per geotec:h report"proof rolling" compaction verification was
sufficient to confirm in place density. Since the site was excavated down into a 10' below topsoil grade, it
was not possible to roll a loaded dump truck for traditional "proof rolling". The contractor had PTI witness
compaction of the top lift with the roller compactor to reveal any deflection of material as the machine
passed over. It was noted that the material was firm and unyielding as the machine vibrated and passed
-ver the fill area. There were 2 questionable spots when the machine initially rolled over it, but with
Aditional passes with the machine these areas came to a firm condition. A 5/8" stake was driven with a
mallet in various areas throught the site and met strong resistance in less than 2-4". Work appears to be
visually acceptable.
To the best of my knowledge, the above WAS performed in accordance with the approved
plans, specifications, and regulatory requirements.
