Q/15/,?nln 1AR nn S19nlnnn Total $9,339.45 CASE NOTES FOR COM2010-00077 CONDITIONS FOR COM2010-00077 1) The approval of this project is subject to the recommendations and specifications outlined in the attached geological assessment prepared by Bradley Noble Geotechnical Services, dated August 19, 2010 and April 6, 2010. Structures and/or land modifications (grading, cuts, fills, etc.) required in the geglogi�,al as�ment may require a seperate permit. The geological assessment shall remain attached to the approved building plans. 3X---��o�� 2) Compliance with an approved Storm Water Plan prepared by JW Morrissette &Associates, dated August 2010 shall be subject to an on site inspection, or inspections, by the engineer of record or an authorized representative. Said inspection(s) shall be performed in accordance with the approved plan, and prior to any modification that would make a determination of compliance impossible. Inspection reports shall be submitted to verify all engineering requirements have been completed in accordance with the approved storm water plan. Reports shall be submitted to the Mason County Department of Community Development prior to each corresponding inspection and final permit ap roval. Copies of all special inspection reports shall be made available at time of inspection. v COM2010-00077 2 of 8 i 3) An inspection performed by a representative of the geologist, their authorized representative, or engineer of record, shall be completed on-site prior to the footing pour to verify that earthwork and foundation installation activities comply with the Geotechnical report or assessment recommendations. In addition a final inspection report, prepared by a representative of the geologist, their authorized representative, or engineer of record shall be required to verify that erosion control, drainage and final work is completed in accordance to the Geotechnical report or assessment recommendations. Reports shall be submitted to the Mason County Building Dept., PO Box 186, Shelton, WA 98584, prior to the footing and final inspections and available fo insp�c 4) Owner/Agent is responsible to post the assigned address and/or purchase and post private road signs in accordance with Mason County Title 14.28. X Q Cl 5) ALL ONSTRUCTION MUST MEET OR EXCEED ALL LOCAL CODES AND THE INTERNATIONAL CODE REQUIREMENTS AND OCCUPANCY IS LIMITED TO THE PERMITTED AND APPROVED CLASSIFICATION. ANY CHANGE OF USE OR OCCUPANCY WOULD RESULT IN PERMIT REVOCATION. CHANGE OF USE MUST BE APPROVED PRIOR TO CHANGE. x 6) Changes to approved building plans that affect compliance to the current Washington State Energy Code (WSEC), ventilationrequirements), Buildin Plu�mbigg/Mechanical Codes and/or Mason County Regulations shall be approved prior to construction. X 8) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County PAIdponspector shall be made prior to requesting additional inspections. 9) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non.:,cpliant ith son County ordinances and building regulations. A minimum of 2A10BC fire extinguishers is required, a maximum travel distance of 75 feet in any direction and at least one mounted on every level and mounted no more than 60 inches above Lhe,flo r to the p of the unit. X , An NFPA 72 automatic fire alarm system is required to be installed, a separate permit application is required to be submitted and approved prior to the insta I tion of the system. The system will be required to be fully monitored by a UL certified monitoring company. X �� c��c A knox box is required to be installed per section 506 of the 2009 International Fire code. XG( r��C 11) PER TITLE 14 MASON COUNTY BUILDING CODE-CHAPTER 14.17, STANDARDS FOR FIRE APPARATUS ACCESS ROADS - 14.17.110: A fire apparatus access road in excess of 14% grade and more than 150'to new residential or commercial structures will require an automatic fire sprinkler system installed. Contact the Mason County Fire Marshal at(360)427-9670, extension 352, for further information. x �� 12) All interior finish materials must comply with chapter 8 of the 2009 International Building Code for smoke development and flame spread index. Exit passgaeways and corridors must be a class A with a Flame spread index of 0-25 and a somke development index of 0-450. All rooms and enclosed spaces shall be a class B with a flame spread index of 26-75 and a smoke development index of 0-450 All assemble areas with an occupant load of less than 300 can be a class C with a flame spread index of 76-200 and a smoke development index of 0-450. A copy of the manufactures UL listing showing the flame spread and smoke dev. index is required at the time of the finial inspection for all finish materials. Tom^ 13) Landings and stairs must meet the same setback conditions as any permitted structure; and, must be shown on your site plan. Please check your "Approve Site Plan"to ensure these structures are shown and meet the setback conditions listed. Xn, i,�n 14) All duct work must be constructed and installed per the 2009 International Mechanical Code chapter 6 and the SMACNA( Sheet metal and air conditioning contractors national association) standards set there in. X 15) Posting of occupant load, every room or space that is an assembly occupancy shall have the occupant load of the room or space posted in a conspicuous place, near the main exit or exit access doorway from the room or space. Posted signs shall be of an approved legible permanent design a s II bgjnaintained by the owner or autherized agent. X o r,� c� 16) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the per it hsldave prevented action from being taken. No more than one extension may be granted. X . 17) All approved plans are required to be on-site for inspection purposes. If inspection is called for and plans are not on site, Approval WILL NOT be granted. X o �•. 20) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. X 21) Approved dimsions and setbacks on submitted site plan. Setbacks are measured from the furthest projection of the structure. 22) The approved site plan is required to be on-site for inspection purposes. If inspection is called for and the site plan is not on site, Approval WILL NOT be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and collected by the Mason County Buildin Depa ment prior to any further inspections being performed or approvals granted. X COM2010-00077 5 of 8 I 23) This project is approved for compliance to the 2006 Washington State Energy Code/Ventilation & Indoor Air Quality Code subject to the following: ENVELOPE: Vaulted Ceiling R-21 or better, Wall insulation R-21 or better, Concrete wall insulation R-21 interior placement, slab insulation R-10 or better, All windows, including store door(s) shall have a U-Factor of.55 or less and all exterior doors shall have a u-factor of.60 or less. Lighting installed in accordance to the approved lighting budget. All exterior lighting shall have automatic controls capable of turning off exterior lighting when sufficient daylight is available or when lighting is not required during night time hours. Lighting not designated for dusk-to-dawn operation shall be controlled by either: a)A combination photo-sensor and a time switch; or b)An astronomical time switch. MECHANICAL: The mechanical system is approved as proposed on plan sheet M0.2. Ventilation shall be provided in accordance to the 2009 International Mechanical Code, Chapter 4. X Co COM2010-00077 6 of 8 24) The mechanical system for which energy consumption, performance, or mode of operation are regulated by the Washington State Energy Code (WSEC) shall be tested to ensure that control devices, equipment and systems are calibrated, adjusted and operate in accordance with approved plans and specifications. Drawings and notes shall require commissioning in accordance with WSEC section 1416 . For all mechanical systems, commissioning shall include, as a minimum: a. A Commissioning Plan (See WSEC 1416.2.1), b. System Testing and Balancing (See WSEC 1416.2.2), c. Equipment Functional Performance Testing (See WSEC 1416.2.3), d. Controls Functional Performance Testing (See WSEC 1416.2.3), e. A Preliminary Commissioning Report(See WSEC 1416.2.5), f. Post Construction Documentation (all) (See WSEC 1416.2.4), and g. A Final Commissioning Report (See WSEC 1416.2.5). Buildings or portions thereof, required by this code to comply with this section, shall not be issued a final certificate of occupancy until such time that the building official determines that the preliminary commissioning report required by Section 1416.2.5.1 has been completed. Lighting controls shall be tested to ensure that control devices, components, equipment and systems are calibrated, adjusted and operate in accordance with the approved plans and specifications. A complete report of test procedures and results shall be prepared and filed with the owner. Drawing notes shall require commissioning in accordance with WSEC 1513.7. Comma iOports shall be made available for the Mason County Building Inspector during the final occupancy inspection. X o(o 0 This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied.
