2) SCOPE OF PROJECT
This permit is approved to repair foundation settlement using a push pier foundation system installed in accordance with manufacturer's
specifications or engineer's requirements.
Each pier shall have a bearing capacity of not less than 12 kips and be founded on very dense hardpan or at a depth in accordance with
manufacturers hydraulic capacity. Provide written reports prepared and signed by a Washington State licensed design professional or
authorized manufacturer representative. The report shall identify installation specifications and be submitted to the Mason County building
Department,ATTN parcel 32224-50-00083,426 West Cedar Street, Shelton, WA 98584.

3) All construction must meet or exceed all local ordinances and the intemational codes requirements as adopted and amended by Mason County and the
State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in

4) The demolition and disposal of debris must meet the regulations of Mason County and Olympic Region Clean Air Agency(ORCAA).
It is unlawful for any person to cause or allow the demolition (or major renovation)of any structure unless all asbestos containing materials have been
identified and removed from the area to be demolished. Work shall not commence on an asbestos project or demolition project unless the owner or

5) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure
to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with
Mas 'y ordinances and building regulations.

6) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for
action fora enod not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit

Dear Mr.Galando,
Envirotech Engineering (Envirotech) has completed this Foundation assessment;for the above
referenced property. Specifically, this assessment was prepared in order to substantiate the

settlement repair of the existing single faintly residence_ The existing residence has experienced
structural :settlement, and it is planned to be lifted and stabilized with a push pier foundation
system.
,
A site visit was conducted by Michael Staten.with Envirotech on June 4,2014 in order to observe
surface conditions- and assess general subsurface conditions near the residence. Differential
settlements of the residence appear to be due to excessive unconsolidated fill.beneath the
foundation,especially on.the south end(downslope side)of the structure.This is common in the
area, and movements usually occur during an earthquake or during times of abundant, shallow =
groundwater. Excessive groundwater was observed by the seepage and water plants located both
upslope and to the west of the residence.
The parent geological unit in this area is glacial till,a very dense soil(hardpan)underlying about
4 to 5 feet of looser overburden.This is confirmed by geological mapping,and several subsurface
.investigations by Envirotech in the area. Due to the.fill material,the depth to hardpan from the
base of the house is estimated to be approximately 10 feet.
Envirotech concludes that deep foundations are prudent for the repair of the home's foundation. j
A push-pier system is proposed,and will be adequate with the pier's end bearing located on the
very dense hardpan soils believed to be about 10 feet below the existing ground surface. We
believe this depth may vary 5 feet or so where foundation repair is necessary due to natural
undulations of the glacial till surface that are sometimes encountered. The push pier system.is
proprietary,and as such,installation shall follow manufacturer's and/or the structural engineer's
requirements. For bearing capacity, each pier shall have a capacity of at least 12 laps, and;
founded on the very dense hardpan or at a depth where the manufacturer's.hydraulic capacity is
reached during installation.
