Shelton,WA 98584 Permit#:BLD2024-00128 Final Summary Letter: This letter certifies that we have performed an inspection of the above-referenced project with respect to the approved structural plans dated November 1, 2023. Due to field conditions,with the approval of Vista Structural Engineering, LLC all piers with the exception of pier#3 were changed to pin piles. On June 13, 2024, a representative of Vista Structural Engineering, LLC observed the installation of one Foundation Supportworks Incorporated model HP288 helical pier and two PP237 pin piles(periodic inspection per adopted code). In addition,Vista Structural Engineering has examined the job photos, drive logs and pier locations as provided by Terra Firma Foundation Systems and approves of the installation means and methods for this project. During installation a foundation crack was seen between piers 1 and 2 and the foreman was advised to install angle iron to provide additional support. Pier#3, helical, exceeded the minimum specified Ft-Lbs of pressure torquing out at only 6' and was advised to receive a concrete pourback. Piers 4, 6 and 7 achieved refusal short of 7'-6" and were also advised to receive a concrete pourback.All other piers are approved having achieved refusal per plan. Inspection has been provided in an effort to meet the requirements of the building code and does not constitute a guarantee of the installed work. It is the responsibility of the contractor to execute the work in accordance with the approved structural drawings.On-site inspections were not performed by Vista Structural Engineering, LLC.
