X 2) Water qual' is not to be degraded to the detriment of the aquatic environment as a result of this project. X 3) Recreational pier�11 e no higher than 11 feet above mean higher high water. Piers and docks shall have at least an eight foot span between pilings. X� 4) The surface of floating structures shall be a minimum of eight inches above the surface of the water. X 5) Pressure k9at d wood must be completely dry prior to placement in water. Use of creosote or pentachlorophenol is prohibited. X Ix 6) All new treated piling to be used for this project shall recieve a post treatment vacuum of two hours at not less than 22 inches of mercury. Subsequent to releakpof the vacuum, the piling shall be steamed for a two hour period, followed by a second vacuum of four hours at 22 inches of mercury. X 7) Dock facilities must be maintained in a safe and sound condition. X 8) A Department of the Army permit shall be obtained prior to the performance of any activities waterward of the mean higher high water mark or ordinary high wa ark or in wetlands. Please contact the US Army Corps of Engineers, Regulatory Branch, P.O. Box 3755, Seattle, WA 98124. X 9) All changes to "approved" building plans that effect compliance with the Uniform Codes as amended and adopted, or any other Mason County ordinanc r regulation, must be reviewed and approved by Mason County prior to construction. X 10) The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the Uniform Codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building Inspectors be made prior to requesting additional inspections. X -. BLD2004-00437 Please refer to the following pages for conditions of this permit. 2 of 3 11) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The ' failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-comply t with Mason County ordinances and building regulations. X -7- 12) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit h?yc7 have prevented action from being taken. No more than one extension may be granted. X 13) All floatinptTctures shall include intermittent supports which run parallel to the shoreline to keep structures off the tidelands at low tide. X 14) A Hydraulic Pboject Approval from the Washington State Department of Fish + Wildlife must be granted prior to construction. X c 15) All construction and demolition debris must be removed from the beach after project completion. Prop disposal of construction debris must be on land in such a manner that debris cannot enter or cause water quality degredation of State waters. X 16) All piling shall be driven to a point of refusal . X 17) The proposed project must be consistent Sri II applicable policies and other provisions of the Shoreline Management Act, its rules, and the Mason County Shoreline Master Program.X 18) All construction must comply as approved and conditioned under Shoreline Permit No. SHR2003-00003.X This permit becomes null and void if work or construction authorized is not commenced within 180 days, or if construction or work is suspended for a period of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of continuation of work is by means of a progress inspection.The owner or the agent on the owners behalf, represents that the information provided is accurate and grants employees of Mason County access to the,above described property end structure for review and inspection.
