uses and the size of the project and site. There are several questions

agencies may modify this form to include additional specific information on
project description.)
The deck of a floating pier at the site was replaced without being
permitted. The purpose of this project is to obtain an after the fact
permit from Mason County and a new permit from Mason County and
the WDFW to complete required construction necessary to bring the
repaired section of the pier into compliance with WDFW's standards
for overwater structures.
The existing structure, which was permitted in 1994 by Mason
County, consists of a 7' by 106' floating pier and two finger piers (8.5'
x 12' & approx. 14.5' x 12') that are constructed primarily of wood
and encapsulated float material. The unpermitted repair work
consisted of replacing the original decking with Alaska yellow cedar
decking. Both Mason County and the Washington Department of Fish
and Wildlife (WDFW) are requiring that the repaired floating pier be
grated and meet the WDFW's percentage and light passage
standards to the extent that it is possible (i.e. due to the presence of
the float capsules). Approximately 346 sf of pultruded grating with 60
percent light passage will be added. Thirty-two, 12" by 12" metal,
float landing feet will also be added to the pier structure to prevent it
from grounding out during low tides. The landing feet will provide
one foot of seabed clearance. An associated 4' x 16' ramp, two steel
pilings, and a modular plastic 6' x 69' floating pier with several
smaller modular floats were not repaired, nor is any repair work
proposed.
12. What is the location of the proposal? Give sufficient information for a
person to understand the precise location of your proposed project
including a street address, if any, and section, township, and range, if
known. If a proposal would occur over a range of area, provide the range
or boundaries of the site(s). Provide a legal description, site plan, vicinity
map, and topographic map, if reasonably available. While you should
submit any plans required by the agency, you are not required to duplicate
maps or detailed plans submitted with any permit applications related to
this checklist The site is located off Hwy 106, approximately 0.17
miles east of the Alderbrook Lodge, on the south side of the
highway. Section 33, Township 22N, Range 03W. The legal
description is: W1/2 GOVT LOT 1 S 9157 EX TAX 683-C, 669-C.
B. ENVIRONMENTAL ELEMENTS:

1. EARTH:
a. General description of the site (circle one): Flat, rolling, hilly, steep
slopes, mountainous, other.
b. What is the steepest slope on the site (approx. percent slope)? Slopes
along the shoreline, where the property is immediately adjacent to
SR 106, exceed 15 percent.
C. What general types of soils are found on the site (for example, clay, sand,
gravel, peat, muck)? If you know classification of agricultural soils, specify
them and note any prime farmland. Sand and gravel.
d. Are there surface indications of history of unstable soils in the immediate
vicinity: If so, describe. No.
e. Describe the purpose, type, and approximate quantities of any filling or
grading proposed. Indicate source of fill. No filling or grading will be
done.
f. Could erosion occur as a result of clearing, construction, or use: If so,
generally describe. The WDOE's Best Management Practices will be
followed during construction to prevent water pollution.
g. About what percentage of the site will be covered with impervious
surfaces after project constructions (for example, asphalt or buildings)?
The project will not add any new impervious surface to the site.
h. Proposed measures to reduce or control erosion, or other impacts to the
earth, if any: WDOE's Best Management Practices will be followed
during construction, and the WDFW's HPA stipulations shall be
strictly adhered to.
2. AIR:
a. What types of emissions to the air would result from the proposal (i.e.
duct, automobile, odors, industrial wood smoke) during construction and
when the project is completed? If any, generally describe and give
approximate quantities, if known. Emissions from this project will be
minor.

An unnamed Type Np stream and the Hood Canal.
2) Will the project require any work over, in, or adjacent to (within 200
feet) the described waters? If yes, please describe and attach

placed in or removed from surface water or wetlands and indicate

located on the shoreline of Hood Canal.

coriacea) are some of the ESA endangered species in the area.
