concrete (#0360050) was placed by line pump for the topping slab at the lower level. One set of
(4) 6 x 12" cylinders were cast from a sample obtained from the center of the first 10 cubic yard
load. All work was performed for plans and specifications.
To the best of our knowledge, items inspected this date are in accordance with approved plans
and specifications.

MTE inspector arrived on site as requested this day for special inspection of lateral wood
nailing.
Upon arrival, David of Krekow, provided plans for review of structural wood construction for
,clues�thhoous e. Exterior wall panel nailing is completed. Roof sheeting over car deck tongue and
groove material nailing at grids 3-4, A-B was inspected. Field and edge nailing of panels was
done per plan. It was noted that the boundary nailing over bird blocking was not done and that
A35 or LTP clips were not installed around perimeter as required by plan. Contractor is aware
of this and installation will continue.

MTE inspector arrived on site for reinforced concrete special inspection. Resteel was inspected
for grade, size, placement and clearances for the CMU sound wall footing. All resteel was per
plans and specifications. Approximately 29 cubic yards of 4,500 psi concrete mix (#02650) was
placed by pump to the footing. A vibrating stinger was used for consolidation. One set of(4) 6 x
12" cylinders were cast.
Shear wall nailing of the exterior bunkhouse addition wall was inspected and was found per shear
wall schedule for a SW1, detail 12/S4.11.
To the best of our knowledge, items inspected this date are in accordance with approved plans
and specifications.

yards of 4,000 psi mix concrete (product code 02650) was pumped into the cabin stem walls. The
material was mechanically consolidated with a vibrator. A sample of the concrete was obtained
and slump testing was performed (5 112"). One set of (4) 6 x 12" cylinders were cast.
To the best of our knowledge, items inspected this date are in accordance with approved plans
and specifications.

MTE inspector traveled to the project site to conduct a project set up visit. Met with Tim Clark and
Mike Graves with Kerkow Jennings. Reviewed project specifications and special inspection
requirements with contractor. Discussed scheduling for inspections. Dropped off a water cure
tank and heater on site to help facilitate delayed sample pick up.
Also left two MTE project clipboards on site, one for each project. The work on the beach house
will be MTE project#T5265 and the underpass will be a different MTE number, which has not
been assigned yet. Augercast piles for the beach house will start on 10/31/05. MTE is scheduled
to be on site full time during the augercast installation.

placed. There was 135 112 cubic yards of Miles Sand & Gravel Concrete Mix# 15LCD1 placed at

gridline B on 1.5. The test results were a 4" slump, 560 concrete, 400 ambient, and 3.7% air and
(4) 6 x 12" test cylinders were cast. At the engineers request there were (4) 6 x 12" field cure test
cylinders taken at random times and locations.

MTE inspector arrived on site for reinforced concrete inspection for the beach house walls at A-
B/0-3. Verified final clearances and trim steel installation. Minor corrections for clearances at
doorway headers were corrected on the spot. Monitored placement of 40 cubic yards of Miles
Sand & Gravel Concrete Mix#02650 (4,000 psi), which was placed by pump and mechanically
consolidated in the short lifts placed.
One set of 6 x 12" samples were cast representing today's placement. Results of the field tests
are as follows: Slump 5", air temperature 450, concrete temperature 57°. All was found to be in
general conformance with the plans.

MTE inspector arrived on site for structural masonry grouting inspection. Upon arrival, MTE
observed the scheduled inspection is actually for a slab placement at the fireplace chimney grids
2.2 to 2.7 along "B" main level.
Course grout was used and placed for slab at above noted location. Grout manufacturers
specified strength 3000 minimum; slab strength required 5000 psi at 28 days. The engineer was
contacted on this matter and stated approval is dependent on the mix placed achieving 5000 psi
at 28 days.
MTE checked reinforcement and found it to be within reasonable agreement with job
requirements. Sampled the grout slump 8", grout temperature 500, ambient temperature 420, cast
one set of(3) 4 x 8" cylinders for strength verification testing. Approval for this placement of grout
in place of 5000 psi concrete is dependent on the grout achieving 5000 psi at 28 day per the
engineer.

MTE checked reinforcements and found it to appear to be within reasonable agreement with job
requirements.
Observed the placement of 17 cubic yards of Miles Sand & Gravel Concrete Mix#15LCD1 by
pump and was mechanically consolidated. Sampled at 9 cubic yards at a 4 %" slump, concrete
temperature 590, ambient temperature 440, air 3.9%, ticket#120746. Cast one set of(4) 6 x 12"
cylinders for strength verification testing.

MTE inspector arrived on site for the chimney slab at A.8-B.2/2.2-2.8 upper level. Contractor had
completed placement of the 8" CMU, reinforcement and embed plate. Resteel was inspected and
found to conform to plans. Monitored the placement of Design Mix Course Grout, which was
mixed on site. One sample was collected and one set of(3) 8 x 8 x 16" Masonry Prisms were
cast using the grout. (Prisms were laid the day prior). Results of field tests were slump 7", grout
50°F, and air 50°F. All grout was mechanically consolidated.
After placement of grout, contractor batched the concrete for the chimney slab. Design Mix
Course Grout (1 Bag) and Quikrete 5,000 (1 Bag) was mixed on site and placed for the slab.
Concrete was mechanically consolidated and one set of (4) 6 x 12" sampled was cast for analysis.
Results of the field tests were slump 4 112", concrete 50°F and air 50°F.

of (4) cylinders for strength verification testing. Manufacturers mixing directions for the Quikrete
5000 pre mixed bag appeared to be followed within reasonable agreement.

MTE inspector arrived on site as requested this day for special inspection of Lateral Wood
Framing. Upon arrival, David of Krekow, provided plans for review of structural wood
construction requirements. At this point of the project, the basic frame with sheeting at grids (1-
2, A-D) is in progress. MTE reviewed shear walls at grids (A&D, 1.25-1.75) and (A-B & C-D,
1&2) and (C, 1.25-1.5).
MTE inspector Gordon, pointed out to David of Krekow, the plan requirements for boundary
nailing in addition to edge and field nailing, and showed him on the building where this nailing is
intended (Upon inspection this had not been done to this point). MTE is confident that this was
understood and will proceed per plan and will be verified with next nailing inspection. MTE also
noted some points for questioning the engineer regarding his intention on the plans. 1) Are
vertical wood columns at shear wall edges considered the vertical boundary/king stud of the
walls, or is a vertical king stud required on the inside of the column with boundary nailing
applied. 2) Is a double top plate required under horizontal wood beams in shear walls or is a
single top plate sufficient.
MTE was also provided photographs of the installation of Simpson LPT4's for attachment of 3X
bottom plates on top of concrete walls to beams and at top for connection of 2X top plates to
beams. Noted no deficiencies on this other than question regarding double top plate mentioned
above. MTE was only able to provide preliminary review and inspection on this day. Further
construction verification will be requested at a later date.

MTE inspector arrived on site as requested this day for special inspection of Lateral Wood
Framing.
Upon arrival, David of Krekow, provided plans for review of structural wood construction of
primary roof section that is to be placed at grids 1-2, A-D. This section of framing and sheeting
was pre-fabricated on the ground in three sections. MTE visually inspected the nailing of 'h"
plywood panels over tongue and groove wood boards nailed to roof frame. Per plans,
requirements were 6" edge and 12" field nailing. Any missed nails (this was a minimal number)
were immediately placed by a carpenter from Krekow. This was also the case where nails were
sunk too deep; another nail was added next to it. No other deficiencies were noted on these
sections.
At the joints of these three sections, further nailing will be required to join them together. These
will be verified after they are in place.

building perimeter and through middle building (separation section). Stem wall at garage
entrance door was not placed at this time, footing section at east where mechanical passed
through was also placed monolithically with stem wall.
MTE checked reinforcement and found it to be within reasonable agreement with job
requirements. Observed 12 cubic yards of Miles Sand and Gravel Concrete Mix#01555
placed by pump truck and was mechanically consolidated at the above noted locations.
Sampled at 6 cubic yards, slump 5", concrete temperature 70°, ambient temperature 560, ticket
#125465 and cast one set of strength verification test samples.

10". (.131-diameter nail). Nailing pattern appeared to be within reasonable agreement with job
requirements. Main floor deck within grids 1 to 2/outside A to B-2x6 Tongue and Groove. Nail
pattern appears to be within agreement with job requirements. Observed epoxy doweling for
insulated front stairs at grids 0 to 1/A to B for shear dowels per RFI#517 and SSK-06 minimum 4"
embedment, holes cleaned. Simpson AT epoxy with expiration 9/06 lot#467290GG was placed
using supplied mixing tip, smooth dowels were then inserted. Manufacturers recommended
procedures were followed.

structural epoxy.
Upon arrival, David of Krekow, provided plans for review of structural wood construction for
anchored straps per details 5&6/S5.02 and RFI#59. MTE verified that all anchors were installed
per plan and straps were nailed to diaphragm per plan at northwest and northeast
terrace/decks. No deficiencies were noted.
For south stairway number four, smooth #4 dowels were placed vertically in concrete per RFI
#17/3 with 5/8" X 4" embedment. MTE verified correct cleaning of all holes prior to placement
of epoxy per manufacturers instructions. Simpson AT epoxy with an expiration of 05/07 was
used. No deficiencies were noted.
CORRECTIVE ACTIONS TAKEN: Resolution to preliminary report dated 07/13/06: Per RFI #
S 133 regarding boundaries of SW-3 and SW-2 shear-walls, G.L. columns and single top
plates, as is built, is correct.

12" cylinders were cast.
MTE inspected the brick veneer epoxy anchor wall tie system on the north side of the bridge.
Holes were inspected and found to be clean. Clips were epoxied in using Power Fast (lot
#06192011 LC, expiration date 06/08). Work was performed per specifications and manufacturers
recommendations.

MTE inspector arrived onsite for reinforced concrete special inspection. Approximately 90 cubic
yards of 4,000 psi concrete Mix Design #02650, was placed by pump for the tennis court wall
footings. One set of (4) 6 x 12" cylinders were cast. Resteel was inspected and approved by
MTE on 11/15/06. The concrete was mechanically consolidated with a vibrator.
MTE inspected lateral nailing of the exterior shear wall of the beach house. Contractor was
informed of missed nailing in several areas of the west wall. Nails were added to the areas.
Final inspection found the shear wall nailing for the north, south, east and west walls per
schedule 12/S4.11.
To the best of our knowledge, items inspected this date are in accordance with approved plans

placed by pump to the footing. A vibrating stinger was used for consolidation. One set of (4) 6 x
12" cylinders were cast.
Shear wall nailing of the exterior bunkhouse addition wall was inspected and was found per shear
wall schedule for a SW1, detail 12/S4.11.

Upon arrival, David of Krekow, provided plans for review of structural wood construction for
guest house. MTE visually inspected bracing shown on 3/S5.01 at grid 3,A-B and also roof
sheeting and car deck tongue and groove material nailing at grids 3-4,A-B. A preliminary walk
through was also done of exterior walls, which are still in progress with blocking at panel edges
and some field nailing being done. One PHD2 at grid 3.75,A was installed with a 5/8" anchor
bolt using Simpson AT epoxy with an expiration of 03/07. MTE verified hole size, embed and
cleanliness prior to epoxy use. Work was done per RFI #59. No deficiencies were noted.
Angle ledger 4X4X1/4" at floor level around outside fireplace for support of river rock at main
level was installed with '/2" anchors using Powers FastSet epoxy, expiration 06/08. MTE verified
hole size, depth and cleanliness. No deficiencies noted

MTE inspector arrived on site for reinforced concrete special inspection. Resteel for the tennis
court walls was previously inspected and approved. Approximately 36 cubic yards of Miles Sand
and Gravel Concrete Mix# 0750A was placed by pump to the remaining tennis court wall
sections. Slump (4"), and air(5.0%) testing was performed on a sample of the concrete. One set
of(4) 6 x 12" cylinders were cast from the sample.
