RECEIVED ENVIRONMENTAL CHECKLIST JUN 10 2013 Purpose of checklist: 426 W. CEDAR ST The State Environmental Policy Act(SEPA),chapter 43.21C RCW,requires all governmental agencies to consider the environmental impacts of a proposal before making decisions. An environmental impact statement(EIS)must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from your proposal(and to reduce or avoid impacts from the proposal, if it can be done)and to help the agency decide whether an EIS is required. Instructions for applicants: This environmental checklist asks you to describe some basic information about your proposal. Governmental agencies use this checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. Answer the questions briefly,with the most precise information known,or give the best description you can. You must answer each question accurately and carefully,to the best of your knowledge. In most cases,you should be able to answer the questions from your own observations or project plans without the need to hire experts. JARPA and SEPA were submitted for similar repair (using logs instead of rocks) in 2-18-1993. Determination of non- significance was issued. 9. There are several questions later in this checklist that ask you to describe certain aspects of your proposal. You do not need to repeat those answers on this page. The 116' bulkhead was constructed circa 1933 by our grandfather EJ Murnen,Sr. It is comprised of 7 horizontal rows of logs each approximately 12 inches in diameter. The bottom 4 tiers of logs are quite sound. The very bottom 2 tiers are under the beach sands. This project entails the replacement of the top 3 tiers of logs that we replaced 20 years ago(under a previous 1993 permit)which due to being out of the water have failed due to freshwater from rains,and sunlight. See PHOTOS 1-7 The top 4.5 tiers of logs are all situated above the mean high tide. (See PHOTO 5)Specifically,the top 3 tiers of logs have rotted and are to be replaced with 1-3 man basalt rock set back appropriately. There will be no footprint changes. The job will be done by Randles Landscape & Design of Tahuya, WA. All of the work is to be done from the bulkhead surface; no work shall be done from the water or from the beach. See SKETCH 1-Aerial and Sketch-2-Elevation. 12. Location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project, including a street address, if any,and section,township,and range, if known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a legal description,site plan,vicinity map,and topographic map, if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit applications related to this checklist. General description of the site(circle one): Flat,rolling,hilly,steep slopes,mountainous, other. .. . .. Flat site but steep slope from Hwy 106 which ivy covered and bordered by trees and shrubs.See PHOTOS#1-7 attached. b. What is the steepest slope on the site(approximate percent slope)? 75%slope from Hwy 106 down to bulkhead surface in spots. Two paths provide pedestrian access to bulkhead. c. What general types of soils are found on the site(for example,clay,sand,gravel,peat, muck)? If you know the classification of agricultural soils,specify them and note any prime farmland. Glacial till sands and gravels. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so,describe. Slide areas on Hwy 106 above road to the southwest(Olympic Vista)in recent years in approximately 2007. Noticeable undermining of Hwy 106 at corner+/-300 feet to the southwest of subject. e. Describe the purpose,type,and approximate quantities of any filling or grading proposed. Indicate source of fill. Estimated 25-30 CY washed granular drainage material to be used behind new rock wall constructed of 1-3 man basalt rock(estimate 40 T). Approximately 5 yards of washed quarry spall rock will be used in front of the base row of rocks and behind the top remaining log.See Sketch 2-Elevation f. Could erosion occur as a result of clearing,construction,or use? If so,generally describe. NO. The job will be done from the existing bulkhead surface. The staging area for the receipt of the rocks will be on a grassy pathway which will be protected with soil from the excavation behind the logs which are to be removed. The existing natural vegetation(ivy,grasses,maple trees)will be protected as will our planted trees(i.e.laurels and conifers)as needed. g. About what percent of the site will be covered with impervious surfaces after project construction(for example,asphalt or buildings)? NONE h. Proposed measures to reduce or control erosion,or other impacts to the earth,if any: The mitigation strategy will be to keep our activities on the bulkhead without damaging the natural vegetation on our slopes or the bulkhead that has always served as a buffer strip and BMP for runoff from E SR 106. Specifically: 1) All work is to be done by an experienced excavator and manual labor from the topside of the bulkhead surface(not from the beach or from a barge in the water).The basalt rock(i.e.3-2-1-man rocks,washed granular drainage material and spall rocks)will be delivered onto the bulkhead from E SR 106 by a small(5 CY) dump truck that will dump onto our path(See Photos 6-7)which will have previously been set up as a staging area with appropriate safety measures in place. The dumps will be brief and will not impede traffic.The vegetation will be protected by plywood and tarps as necessary. 2) The damaged logs will be used as a BMP wall to keep native fill soils contained during the excavation phase. A silt fence will be installed on the beach prior to the excavation being started in an attempt to capture any soils or wood debris which may fall during excavation and log removal. 3) The soils removed will be used to reduce the slope of the existing path leading onto the bulkhead from the highway. 3 4) The affected areas will be allowed to naturally seed with native grasses that are drought and salt water tolerant. 5) Any loose excavated soils that are stock piled prior to use on the path will be tarped in the event of rains, until used or removed from the site(if necessary). 6) No activities will take place in the water. 7) The logs being removed from the bulkhead will be taken away from the property. 2. Air a. What types of emissions to the air would result from the proposal(i.e.,dust,automobile, odors,industrial wood smoke)during construction and when the project is completed? If any,generally describe and give approximate quantities if known. Only emissions coming from the dump truck and excavator machine's exhaust pipe. b. Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. NONE c. Proposed measures to reduce or control emissions or other impacts to air,if any: NONE REQUIRED 3. Water a. Surface: 1) Is there any surface water body on or in the immediate vicinity of the site(including year-round and seasonal streams,saltwater,lakes,ponds,wetlands)? If yes,describe type and provide names. If appropriate,state what stream or river it flows into. The bulkhead,originally finished in 1933,is located along the shoreline of Hood Canal. There is a stream located 50 feet to the East and another stream located 200 feet to the West. 2) Will the project require any work over,in,or adjacent to(within 200 feet)the described waters? If yes,please describe and attach available plans. Yes,as the bulkhead is on the shoreline. 3) Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. NONE placed or removed. 4) Will the proposal require surface water withdrawals or diversions? Give general description,purpose,and approximate quantities if known. NO 5) Does the proposal lie within a 100-year floodplain? If so,note location on the site plan. We presume so as it is on the shoreline of Hood Canal. 6) Does the proposal involve any discharges of waste materials to surface waters? Proposed measures to reduce or control surface,ground,and runoff water impacts,if any: The 5-day project is to be done in the Summer of 2013 during a dry period.We will protect all ivy, shrubs and trees during the project as is reasonable,tarp over all soils removed during the project(if there is a threat of rain)until they are restored to their final destination on the bulkhead. The damaged logs will be used as a BMP wall to keep native fill soils contained during the excavation phase.A silt fence will be installed on the beach prior to the excavation being started in an attempt to capture any soils or wood debris which may fall during excavation and log removal. The end result of this project should result in a better filtration system than what exists at present. 4. Plants a. Small dump trucks and a Trac-hoe excavator utilizing diesel fuel will be used in the delivery of the new basalt rock,washed granular drainage material,and quarry spall rock,and excavation of soils/logs and placement of new rock. b. Would your project affect the potential use of solar energy by adjacent properties? If so,generally describe. NO c. What kinds of energy conservation features are included in the plans of this proposal? List other proposed measures to reduce or control energy impacts,if any: The use of equipment sized for the job instead of oversized equipment will save energy. 7. Environmental health a. Are there any environmental health hazards, including exposure to toxic chemicals,risk of fire and explosion,spill,or hazardous waste,that could occur as a result of this proposal? If so,describe. There are no environmental health hazards. 1) Describe special emergency services that might be required. 6 First aid kits will be on hand; vehicles for access to hospital will be available and cell phones for the dispatching of an ambulance will be available. 2) Proposed measures to reduce or control environmental health hazards,if any: Professional grade machinery (small 5 CY dump truck and Trac-hoe excavator) will be operated by licensed, bonded and insured operator. A silt fence will be installed on the beach to capture any loose soils and wood debris that may fall during construction. Tarps will also be used to cover excavated soils in the event of rains. b. Noise 1) What types of noise exist in the area which may affect your project(for example: traffic,equipment,operation,other)? NONE 2) What types and levels of noise would be created by or associated with the project on a short-term or a long-term basis(for example: traffic,construction,operation,other)?Indi- cate what hours noise would come from the site. Job hours will be M-F 7:30-5. Occasional small dump truck loads(5 CY capacity X 6-8 loads)will not cause any traffic disruptions. The Trac-hoe excavator is a late model variety and is reasonably quiet. 3) Proposed measures to reduce or control noise impacts,if any: NONE 8. Land and shoreline use a. What is the current use of the site and adjacent properties? This 80 year old north facing bulkhead serves as a recreational area for the benefit of the second through fourth generations of our large family for family fellowship. It provides a safe,dry area for our recreation equipment and general seating at higher tides. It is the most used area of the property and provides a wonderful platform for reunions and life-guarding our young swimmers. We rarely see anyone on the beach to the East,and occasionally see neighbors to the West. b. Has the site been used for agriculture? If so,describe. NO I c. Describe any structures on the site. NONE I d. Will any structures be demolished? If so,what? NO I e. What is the current zoning classification of the site? Rural residential 5 f. Proposed measures to reduce or control impacts,if any: None other than maintaining this grand property as best we can to honor past stewards and promote its value for future generations to come. 14. Transportation a. Identify public streets and highways serving the site,and describe proposed access to the existing street system. Show on site plans,if any. The small(5CY)dump truck will make approximately 6-8 trips using E SR 106 to unload the new basalt rock for the wall,the washed granular drainage material for behind the wall and the quarry spall to be place in front of the wall. Each dump should take less than 1 minute. (See Map,Sketch 1-Aerial and Photos 6-7) b. Is site currently served by public transit? If not,what is the approximate distance to the nearest transit stop? No,however a Mason County Transit Authority bus goes by the house on weekdays around 7:15 AM towards Belfair,and 9:40 AM towards Union. So,rock dumps taking place between 7:30 and 9:30 will not impact bus service. c. How many parking spaces would the completed project have? How many would the project eliminate? Not Applicable d. The vintage 1933 bulkhead is part of the ambience that which is the south shore of Hood Canal. Our proposal is for the repair of this prominent structure which is a perfect complement to the 1933 log cabin across the street. Proposed measures to avoid or reduce shoreline and land use impacts are: The proposed use of 1-4 man rock instead of using replacement logs while not increasing the original footprint would seem to be a recipe for responsible management of the shoreline and land. 6.
