Proposed timing or schedule (including phasing, if applicable). THE PROJECT INVOLVES MINING SAND AND GRAVEL ON A 41.18 ACRE SITE. ECONOMIC CLIMATE WILL DICTATE THE LIFE OF THE MINE. THE PROJECTED LIFE OF THE MINE IS 14 YEARS. MINING WILL BE CONTINUOUS AND RECLAMATION WILL OCCUR ON A ANNUAL BASIS IN THE MINED AREA. 7. Do you have any plans for future additions,expansions,or further activity related to or connected with this proposal? If yes,explain. THE MINE SITE WILL ALSO BE UTILIZED AS WOOD WASTE RECYCLING AREA. S. List any environmental information you know about that has been prepared, directly related to this proposal. NONE 9. Do you know whether applications are pending for governmental approvals of other proposals directly affecting the property covered by your proposal? If yes,explain, 10. Give a brief, complete description of your proposal, including the proposed uses and the size of the project and site. THE PROJECT INVOLVES MILTING SAND AND GRAVEL FROM THE 40 ACRE SITE LOCATED APPROXIMATELY 2.5 MILES NORTH OF BELFAIR ON THE OLD BELFAIR HIGHWAY. THE SITE IS AN EXISTING SAND AND GRAVEL PIT AND THE MINING PERMIT IS FOR THE CONTINUATION OF THE MINING ACTIVITY. THE M%aNG OPERATION IS PROJECTED TO TAKE 14 YEARS UNTIL THE SITE IS EXHAUSTED. A RECLAMATION PLAN IS PROPOSED WHICH WILL ALLOW FOR THE CONTINUOUS RESTORATION. DURING THE ACTIVE LIFE OF THE MINE, APPROXIMATELY 5 ACRES OF THE SITE WILL BE UTILIZED AS A REPOSITORY FOR STUMPS AND BRUSH FROM THE LOCAL AREA WHICH WILL BE GROUND AND UTILIZED IN THE CREATION OF A BLENDED TOPSOIL MIX. THE TOPSOIL WILL BE SOLD AS A COMMODITY SINIILAR TO THE SAND GRAVEL. SOME OF THE MIX WILL BE UTILIZED IN THE RECLAMATION PROCESS. 12. Location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project, including a street address, if any, and section, township, and range, in known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if reasonable available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit applications related to this checklist. THE PROJECT IS LOCATED AT NE 2493 OLD BELFAIR HIGHWAY, BELFAIR, WASHINGTON 98528. THE SITE LIES IN THE NE '/4, NE '/4, SEC. 17, T 23 N, R 1 W, W.M. MASON COUNTY WASHINGTON. 2 TO BE COMPLETED BY APPLICANT EVALUATION FOR AGENCY USE ONLY AGREE / DISAGREE / MITIGATE B. ENVIRONMENTAL ELEMENTS 1. Earth a. General description of the site (underline/circle one): Flat, rolling, hilly, steep, slopes, mountainous, other. HILLY WITH OPEN PIT IN MINED AREAS b. What is the steepest slope on the site (approximate percent slope)? APPROXIMATELY 20% ON NATIVE SITE. THE MINED AREAS ARE AT OR NEAR 1 '/2 : 1. C. SURFACE SOILS ARE CLASSIFIED AS EVERETT GRAVELLY LOAMY SAND. SUBSURFACE SOILS ARE SAND AND GRAVEL. d. Are there any surface indications or history of unstable soils in the immediate vicinity? If so, describe. NO e. Describe the purpose, type, and approximate quantities of any filling or grading proposed. Indicate source of fill. NONE L Could erosion occur as a result of clearing, construction or use? If so, generally describe. YES, IN ACTIVELY MINED AREAS SUBSOILS ARE EXPOSED AND EROSION MAY OCCUR. RUNOFF WILL BE DIRECTED ALONG THE MINE FLOOR TO THE STORM WATER DETENTION AND TREATMENT FACILITIES WITHIN THE MINE. CONTAINMENT BERMS AND ROCK LINED CHANNELS WILL BE INSTALLED AND MAINTAINED TO CONTROL RUN OFF. 3 TO BE COMPLETED BY APPLICANT EVALUATION FOR AGENCY USE ONLY AGREE / DISAGREE / MITIGATE g. Proposed measures to reduce or control erosion, or other impacts to the earth, if any. THE MINE CURRENTLY HAS STORM WATER FACILITIES FOR HANDLING RUNOFF FROM PRECIPITATION FALLING ON THE MINE SITE. THE STORM WATER SYSTEM CONSIST OF A SERIES OF PONDS AND BERMS LOCATED ON THE VARIOUS TERRACES WITHIN THE MINE. THE SYSTEM IS INTENDED TO CAPTURE RUNOFF AND DISCHARGE IN ACCORDANCE WITH THE N.P.D.E.E.S. PERMIT AND THE WDNR MINING PERMIT. 2. Air a. What types of emissions to the air would result from the proposal (i.e. dust, automobile, odors, industrial wood smoke) during construction and when the project is completed? If any, generally describe and give approximate quantities if known. DURING THE ACTIVE MINING OPERATION EXHAUST FUMES FROM THE EQUIPMENT WILL BE EMITTED AND DUST FROM THE CRUSHING, MILLING, WASHING AND GRADING OPERATION. UPON COMPLETION OF THE MINING ACTIVITY THE SITE WILL BE RECLAIMED AND NO EMISSIONS ARE EXPECTED. b. Are there any off site sources of emissions or odor that may affect your proposal? Is there any surface water on or in the immediate vicinity of the site (including year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe type and provide names. If appropriate, state what stream or river it flows into. A CLASS III STREAM LIES FLOWS ALONG THE WEST LINE OF THE SITE. THE STREAM HAS A STEEP GRADIENT. THE STREAM IS UNNAMED. THE STREAM IS TRIBUTARY TO THE UNION RIVER WHICH FLOWS INTO HOOD CANAL. 2. Will the project require any work over, in, or adjacent to (within 200 feet) the described waters? If yes, please attach available plans. NO, THE MINE WILL MAINTAIN THE 65' BUFFER (MASON COUNTY) AWAY FROM THE STREAM. A EARTH RIDGE DEFINING THE DRAINAGE BASINS WILL BE MAINTAINED SO ITS VERY UNLIKELY RUNOFF FROM THE MINE CAN REACH THE STREAM. 3. Estimate the amount of fill and dredge that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. NONE 5 TO BE COMPLETED BY APPLICANT EVALUATION FOR AGENCY USE ONLY AGREE / DISAGREE / MITIGATE 4. Will the proposal require surface water withdrawals or diversions? Give general description, purpose, and approximate quantities, if known. STORM WATER COLLECTED ONSITE IS USED IN THE CRUSHING AND WASHING OPERATIONS. QUANTITIES ARE UNKNOWN. THE SYSTEM IS SET UP TO RECLAIM PROCESS WATER WHICH IS CLEANED AND REUSED AGAIN IN THE WASH OPERATION. 5. Does the proposal lie within a 100 year floodplain? If so, note location on the site plan. NO 6. Does the proposal involve any discharges of waste materials to surface waters? If so, describe the type of waste and anticipated volume of discharge. NO b. Ground 1. Will ground water be withdrawn, or will water be discharged to ground water? Give general description, purpose, and approximate quantities, if known. DOMESTIC SEWAGE FROM THE OFFICE IS ROUTED TO A 10,000 GALLON SEPTIC TANK WITH A DRAINFIELD LOCATED WITHIN THE PIT. A PROPOSED DRAINFIELD FOR WOOD WASTE RECYCLING AREA. LOCATION WILL BE IN THE PIT AND SIZE UNKNOWN. C. Water Runoff(including storm water): 7 TO BE COMPLETED BY APPLICANT EVALUATION FOR AGENCY USE ONLY AGREE / DISAGREE / MITIGATE 1. Describe the source of runoff(including storm water) and method of collection and disposal, if any (include quantities, if known). Where will this water flow? Will this water flow into other waters? If so, describe. DURING THE ACTIVE MINING OF THE SITE RAINFALL FALLING ON THE MINE SITE WILL BE ROUTED THRU EXISTING STORM WATER FACILITIES. THE STORM WATER SYSTEM CONSIST OF A SERIES OF PONDS AND BERMS LOCATED ON THE VARIOUS TERRACES WITHIN THE MINE. THE MAJORITY OF THE WATER ENDS UP IN A POND AT THE SOUTH EAST CORNER OF THE SITE WHERE IT IS PUMPED BACK UP THE HILL SPRAYED ON UPPER AREAS OF THE MINE. IT THEN ENTERS A FRENCH DRAIN AND IS DISCHARGED TO A RAVINE TO THE SOUTH WHICH DRAINS TO THE UNNAMED CLASS III STREAM. THE NORTH EASTERLY PORTION OF THE MINE DRAINS TO A RAVINE ON THE EAST LINE OF THE SITE. WHEN ACTIVE MINING OCCURS IN THIS AREA A DETENTION AND WATER QUALITY FACILITY WILL BE IMPLEMENTED. WHEN THE MINE SITE IS EXHAUSTED AND THE SITE RECLAIMED STORMWATER FROM THE ENTIRE MINE SITE WILL BE ROUTED TO THE POND IN THE SOUTH EAST CORNER. THE POND WILL INCORPORATE A DETENTION AND WATER QUALITY FACILITY. THE POND DISCHARGES TO A DITCH WHICH ULTIMATELY REACHES THE UNION RIVER. SOME OF THE STORM WATER COLLECTED ONSITE IS USED IN THE CRUSHING AND WASHING OPERATIONS. 8 TO BE COMPLETED BY APPLICANT EVALUATION FOR AGENCY USE ONLY AGREE / DISAGREE / MITIGATE 2. Could waste materials enter ground or surface waters? If so, generally describe. YES THRU INFILTRATION. 3. Proposed measures to reduce or control surface, ground, and water impacts, if any.
