List any environmental information you know about that has been prepared, or will be prepared, directly related to this proposal. May 1991, Skillings & Chamberlain, Inc. prepared construction drawings for construction of the ecology block basins. Ken Martig, P.E. has been contacted and reported that the 3611 diameter culverts install beneath Highway 106 were intentionally oversized to accommodate potential mud and debris flows. HPA issued Feb. 4, 1991 (Control No. 00- 47990-02) Current project addresses item #4 Page 1 TO BE COMPLETED BY APPLICANT: EVALUATION FOR AGENCY USE ONLY of that HPA here stated; 4 . When sedimentation to the beaches, caused by run- off of uplands, is no longer a threat, then more conventional beach erosion measures will be taken by Mason County (elimination of the stilling basin and complete restoration of the surf smelt beach) . Mr. Niel Rickard of WDFW has visited site and feels removal of the basin would be adequate restoration of the surf smelt beach. 9. Do you know whether applications are pending for governmental approvals of other proposals directly affecting the property covered by your proposal? If yes, explain. None Known. 10. List any government approvals or permits that will be needed for your proposal, if known. Shoreline Substantial Development Permit Hydraulic Project Approval (WDFW) U.S. Army Corp of Engineer 404 Permit 11. Give a brief, complete description of your proposal, including the proposed uses and the size of the project and site. There are several questions later in this checklist that ask you to describe certain aspects of your proposal. You do not need to repeat those answers on this page. (Lead agencies may modify this form to include additional specific information on project description. ) Proposal is to remove ecology block stilling basins from two locations installed in 1991. Monitoring/inspection of the features over the past 5 years indicates that the upper reaches of the basins delivering water to the cross culverts beneath Highway 106, have healed sufficiently to eliminate the need for the stilling basins. WDFW has requested the removal of the ecology block sediment traps/stilling basins. The rows of ecology blocks protecting the road fill embankments are to be left as indicated on the attached plans. Phased construction is proposed with the waterward ecology blocks being removed as the first step. Gravels trapped in the stilling basins will be allowed to distribute themselves over the neighboring beaches. After the beaches have returned to a natural Page 2 TO BE COMPLETED BY APPLICANT: EVALUATION FOR AGENCY USE ONLY condition, a cable-concrete pad is to be installed at each location to provide erosion protection at the pipes' outfalls. At the time of the pad installation, additional rock work is proposed to "dress up" the upper beach/road embankment areas. Also, riprap that has migrated onto the beach will be restored to a high elevation, and three large boulders are proposed to be positioned to provide additional energy dissipation. It is expected that the project can be performed from the road shoulder requiring no equipment operation on the beach. The ecology blocks salvaged from the basin demolition will be disposed of at an upland location. 12 . What is the location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project, including a street address, if any, and section, township, and range, if known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s) . Provide a legal description, site plan, vicinity map, and topographic map, if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit applications related to this checklist. The project location lies in the Southeast Quarter of Section 33, and the Southeast Quarter of Section 34, both in Township 22 North, Range 3 W.W.M. , Mason County, Washington on State Highway 106 on the south shore of Hood Canal. The Smith/Nordstrom Site is at approximate street address E 7292 Hwy 106. The Gabrielson site is at approximate street address E 8040 Hwy 106, Union Washington. vicinity map attached. B. ENVIRONMENTAL ELEMENTS : 1. EARTH: a. General description t e site (circle one) : Flat, rolling,/1hilly, steep slopes, mountainous, other b. What is the steepest slope on the site (approximate percent slope) ? The beach slopes away from the highway fill embankments at an approximate slope of 10%. c. What general types of soils are found on the site (for example, clay, sand, gravel, peat, muck) ? If you know the classification of agricultural soils, specify them and note any prime farmland. The beach at the Smith/Nordstrom site appears to be underlain by a consolidated clay, with a sandy gravel residing on the surface of the beach. The beach at the Gabrielson site appears to be a sandy gravel, composed of soils washed down from the surrounding uplands. The road embankments are a gravelly sand and has the character of reconsolidated glacial till material. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. Natural beach erosion processes appear to be slowly eroding away the toe of the road embankments. e. Describe the purpose, type, and approximate quantities of any filling or grading proposed. Indicate source of fill. Approximately 30 cubic yards of material is present in each of the two ecology block stilling basins. WDFW is requesting that the material present in the basins not be removed from the beach environment, but be allowed to dissipate naturally. To accomplish this, the basins will be disassembled, and the material allowed to dissipate naturally through tidal action. Once the beaches have established their equilibriums, the cable concrete mats will be buried approximately 0 .51 below the anticipated beach level (s) . Project completion will include the placement of three large boulder at the pipes, outfalls to provide energy dissipation during high flow periods. Page 4 TO BE COMPLETED BY APPLICANT: EVALUATION FOR AGENCY USE ONLY f. Could erosion occur as a result of clearing, construction, or use? If so, generally describe. Yes. Unbridled discharges from the outfall pipes represent potential for erosion, and a scour hole to develop. The placement of the cable concrete mats and riprap boulders is expected to minimize erosion at the culverts' outfalls and provide a natural dispersion and velocity reduction device. g. About what percent of the site will be covered with impervious surfaces after project constructions (for example, asphalt or buildings) ? None. h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any: Construction will be done during the dry season of the year to alleviate environmental impacts. Pipe outfall inverts will be fitted with erosion control-cable concrete mats and riprap boulders to provide permanent erosion/scour protection. 2 . AIR: a. What types of emissions to the air would result from the proposal (i.e. dust, automobile, odors, industrial wood smoke) during construction and when the project is completed? If any, generally describe and give approximate quantities, if known. Dust from the construction and fumes from the equipment. b. Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. None known. c. Proposed measures to reduce or control emissions or other impacts to air, if any: None proposed. Page 5 TO BE COMPLETED BY APPLICANT: EVALUATION FOR AGENCY USE ONLY 3 . WATER: a. Surface: 1) Is there any surface water body on or in the immediate vicinity of the site (including year-round and seasonal streams, saltwater, lakes, ponds, wetlands) ? If yes, describe type and provide names. If appropriate, state what stream or river it flows into. Yes. The ecology block stilling basins are sited almost entirely on the upper beach of Hood Canal. The bottom rows of ecology blocks are sited below the ordinary high water elevation of Hood Canal. Only a small portion of the proposal involves work above tidal areas of Hood Canal. Work will be coordinated to avoid tidal conflicts. 2) Will the project require any work over, in, or adjacent to (within 200 feet) the described waters? If yes, please describe and attach available plans. Yes. Plans attached. 3) Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material . Approximately 30 ecology blocks (21x2lx4l ) will be removed from each of the two sites. Removal of the ecology blocks is expected to release approximately 30 cubic yards of accumulated sands and gravels. 4) Will the proposal require surface water withdrawals or diversions? Give general description, purpose, and approximate quantities if known. None anticipated. 5) Does the proposal lie within a 100- year floodplain? If so, note location on the site plan. Equipment maintenance operations will be restricted from within 200 feet of the Hood Canal shoreline. 4 . PLANTS: a. The ecology block stilling basins will be eliminated. Some portions are to remain to provide the highway fill embankment with wave/erosion protection. e. The 1968 Mason County Land Use Plan (Fig.9) shows the proposed activities lie in a Suburban designated area. g. If applicable, what is the current shoreline master program designation of the site?
