1.) A sprinkler head is to be added to the bathroom, it appears to have been missed. X

A six inch electric bell 120V is to be placed on the exterior road side of the structure to indicate water flow. X C
A witnessed hydro test is required, contact the Mason County Fire Marshal Office @ 360-427-9670 ext 273 to schedule a test. X
A witne d cket test is required to ensure adequate pressure and volume. Contact MCFMO @ 360-427-9670 ext 273 to schedule the test.

A sprinkler insulation tenting inspection can be performed simultaneously with the bucket test. X_
A final inspection is required after trim out and includes the bell test. Contact the MCFMO @ 360-427-9670 ext 273. X
With the addition of the above items, the plans are approved as submitted. X�
2.) PURSUANT TO 1997 UNIFORM BUILDING CODE, ALL SITES MUST HAVE APPROVED NUMBERS OR ADDRESSES PROVIDED IN SUCH A
POSITION AS TO BE PLAINLY VISIBLE AND LEGIBLE FROM THE STREET OR ROAD FRONTING THE PROPERTY. MASON COUNTY BUILDING
DEPARTMENT REQUIRES THAT THIS BE COMPLETED PRIOR TO CALLING FOR ANY SITE INSPECTIONS. A REINSPECTION FEE, BASED ON
RATES AS ADOPTED BY THE JURISDICTION AND THE 1997 UNIFORM BUILDING CODE WILL BE ASSESSED IF OWNER/CONTRACTOR FAILS
TO PO�;;ESS ON SITE PRIOR TO REQUESTING INSPECTIONS.

This permit becomes null and void if work or construction authorized is not commenced within 180 days, or if construction or work is suspended for a period of 180 days at any time after
work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of
