Authorized Local Government Offi ' I Dale �Cant" D�C�r9 FCC. mow , MITIGATED DETERMINATION OF NONSIGNIFICANCE (WAC 197-11-350) SEP2004-00082 CONDITIONS OF THIS DETERMINATION 1) All upland areas disurbed or newly creted by construction activities shall be seeded, vegetated or given an equivalent type of erosion protection (silt fencing or straw matting). 2) The applicant shall consider the collocation of additional communication antennas on the approved tower upon the request by other communication companies in order to reduce the need for additional cellular tower construction in this area. 3) The applicant shall remove the communications tower within ninety days of the date of notice that the subject facility is abandoned (i.e. taken out of use for all facility users for any period of twelve consecutive months). 4) The tower must be designed for a minimum of three users, per Mason County Ordinance 5-98. 5) A. Towers shall either maintain a galvanized steel finish or, subject to any applicable standards of the FAA, be painted a neutral color, so as to reduce visual obtrusiveness. B. At a tower site, the design of the buildings and related structures shall, to the extent possible, use materials, colors, textures, screening, and landscaping that will blend the tower facilities to the natural setting and built environment. C. If an antenna is installed on a structure other than a tower, the antenna and supporting electrical and mechanical equipment must be of a neutral color that is identical to, or closely compatible with, the color of the supporting structure so as to make the antenna and related equipment as visually unobstructive as possible. D. Towers shall not be artificially lighted, unless required by the FAA or other applicable authority. If lighting is required, the governing authority may review the available lighting alternatives and approve the design that would cause the least disturbance to the surrounding views. 322 3 7 5 ENVIRONMENTAL CHECKLIST A. Cingular proposes to install and operate and maintain an un-staffed 150' steel self- support antenna support structure containing antennas and ground mounted radio equipment adjacent to the base of the tower. The site will be secured by a six (6) foot talk chain link fence and locked gate. No water or sewer service is required. 13. Location of the proposal. Give sufficient information for a person to understand the precise location of our proposed project, including a street address, if any, and section, township, and range. If a proposal would occur over a range or boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit application related to this checklist. a.Address: 100 E. However the balance on the immediate land area slopes to the southeast approximately 5%. C. What general types of soils are found on the site (for example, clay, sand, gravel, peat, muck)? If you know the classification of agricultural soils specify them and note any prime farmland. Unknown at this time. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. The soil appears to be stable. There is no known history of otherwise. e. Describe the purpose, type and approximate quantities of any filling or grade proposed. Indicate source of fill. The excavation will be limited to removing the soil to accommodate the equipment pad and tower foundation. All soil materials are from and will remain onsite. It is estimated the amount of excavation will not exceed 50 cubic yards. f. Could erosion occur as a result of clearing, construction, or use? If so, generally describe. No g. The subject site project area is approximately 2,500 sq. ft., which will contain approximately 1,500 sq. ft. for the equipment cabinets/shelter (both proposed and future collocations), and the balance will be covered with gravel or crushed surfacing. h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any: There are no erosion control hazards. 2. Air a. What types of emissions to the air would result from the proposal (i.e. dust, automobile, odors, industrial wood smoke) during construction and when the project is completed? If any, generally describe and give approximate quantities if known. During construction there may be a little dust due to the excavation of the footing for the equipment pad and pole, however post construction there shall be no dust, odors, or wood smoke. b. Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. The facility is unmanned and will not be affected by such. C. Proposed measures to reduce or control emissions or other impacts to air, if any: Site will be watered during construction if necessary to help diminish the dust. 3. Water a. Surface: 1. If there is any surface water body on or in the immediate vicinity of the site (including year-round and seasonal streams, saltwater, lakes, ponds, and wetlands)? If yes, describe type and provide manes. If appropriate, state what stream or river if flows into. No 2. Are there any environmental health hazards, including exposure to toxic chemicals, risk of fire and explosion, spill, or hazardous waste that could occur as a result of this proposal? What types of noise exist in the area, which may affect your project (for example: traffic, equipment,operation, other)? The site is unmanned and will not be affected by noise. 2. What types and levels of noise would be created by or associated with the project on a short-term or a long-term basis (for example: traffic, construction, operation, other)? Indicate what hours noise would come from the site. During construction and there may be a little noise generated by the excavating equipment. While the facility is in operation, there will be little or no noise generated. 3. Proposed measures to reduce or control noise impacts, if any: Increased noise levels due to construction will be restricted to the hours permitted by ordinance. 8. Land and Shoreline Use a. What is the current use of the site and adjacent properties? The current use of the project site is vacant. Its immediate surroundings consists of vacant land and an existing communications facility. b. Has the site been used for agriculture? If so, describe. Unknown. Not likely. C. Describe any structures on the site. The site is limited to that area dedicated for the proposed wireless facility. There are no structures on the proposed project site, however there is an existing communications facility to the east of the proposed site. d. Will any structures be demolished? If so, what? No. e. What is the current zoning classification? The current zoning classification is RR5. f. What is the current comprehensive plan designation of the site? The current comprehensive plan designation is RR5. g. If applicable, what is the current shoreline master program designation of the site? Not known. h. Has any part of the site been classified as an "environmentally sensitive" or "Critical" area? Not known. i. Approximately how many people would reside or work in the completed project? The proposed facility is unmanned. j. Approximately how may people would the completed project displace? None k. Proposed measures to avoid or reduce displacement impacts, if any: None 1. Proposed measures to ensure the proposal is compatible with existing and projected land uses and plans, if any: Compliance with applicable Mason County codes and standards. The wireless facility will be compatible with the RR5 zoning designation of the area. The facility is unmanned, remotely monitored, produces no pollution and requires approximately 2500 s.f. of space. The equipment area will not displace existing parking, or employment. The facility is being limited to the minimum height necessary to achieve coverage objectives. The tower will be galvanized steel (grayish in color) and equipment is to be placed inside a fenced chain link compound. As Mason County's population increases in this area, the demand for dependable wireless communication services also increases. This proposed facility will provide increased capacity and improved coverage service in this area. 9. Housing a. Approximately how many units would be provided, if any? Indicate whether high, middle or low-income housing. Not applicable b. Approximately how many units,if any,would be eliminated? Indicate whether high, middle, or low-income housing. Not applicable C. Proposed measures to reduce or control housing impacts, if any: Not applicable 10. Aesthetics a. What is the tallest height of any proposed structure (s), not including antennas; what is the principal exterior building material (s)proposed? The proposed wireless tower will be 150' tall. It will be of galvanized steel with a grayish color. Equipment will be contained inside a compound surrounded by a six-foot tall chain link fence for security purposes. b. What views in the immediate vicinity would be altered or obstructed? The location of the tower was carefully chosen to have minimal to no impact on the views enjoyed in the immediate vicinity. Due to the nature of cellular communications (line of site), the pole will be visible from a distance, but the grayish color should allow it to blend in with the typical Washington overcast skies. No views will be obstructed. C. Proposed measures to reduce or control aesthetic impacts, if any: The tower is being limited to the minimum height necessary to achieve coverage objectives. The tower will be galvanized steel (grayish in color). Equipment is to be placed inside a fenced compound for security purposes. 11. Lights and Glare a. What type of glare will the proposal produce? What time of day would it mainly occur? At present, it is believed that the FAA will not require lighting atop the tower. Any lighting requirements are solely under the jurisdiction of the FAA and the applicant has no option but to comply with their requirements should it be required. b.
