1) This application is subject to Buffer and Landscaping requirements as established under Mason County
Ordinance 1.03.036.X r �
2) The use, handling and storage of hazardous materials or flammable and combustible liquids in excess of 10
gallons is not allowed without the approval of the Mason County Fire Marshal.

3) Provisions for surface/subsurface drainage control must be implemented with new construction or development
on site and MUST NOT adversely impact adjacent parcels. Under the requirements of Mason County
Stormwater Ordinance, either private ditches and drains will meet requirements of the stormwater ordinance or
prior approval will be granted to use an existing utility and drainage easement dedicated for that specific
purpose. For further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS
PERMIT for the installation/construction of a driveway or access connecting from a Mason County Road,
Contact the Mason County Public Works Department prior to construction at Ext 450. For any construction
which is proposed to be located within 25' of a Mason County road right of way, it is suggested to contact that
office to review future planned work which may affect your project.

4) Proposed structure or any portion thereof greater than 30" in height from grade line, must maintain a minimum
of 5'setback from all property lines, easements and 10'from all County and State Road right of ways.

5) All upland areas disturbed or newly created by construction activities shall be seeded, vegetated or given an
equivalent type of erosion protection (silt fencing or straw matting). X "L40r
6) Parking shall be sufficient for 55 normal parking stalls (9 feet by 20 feet) and 3 handicap parking stalls (12.5 feet
by 20 feet)with sufficient maneuvering aisles. Handicap stalls shall be of a smooth surface at level or ramped
to entry, located closest to the building entry, and shall be signed with the International Symbol of Access.
Screening from djacent residential properties is required (per SCB pre-sub requirements).
7) 1. Install a fire alarm system throughout the addition.
2. Separate the addition from the existing building with a 2-hour fire wall and divide the addition in half with
another 2-hour fire wall.
3. Install lighted exit signs at all required exits.
4. Install 2A 10BC rated 5 lb fire extinguishers at all exit doors.
NOTE: ANY FUTURE ADDITION TO THIS BUILDING, BEYOND THE ADDITION COVERED UNDER THIS
PERMIT, WILL REQUIRE THE INSTALLATION OF AN AUTOMATIC FIRE SPRINKLER SYSTEM
THROUGHOUT THE EXISTING BUILDING AND ANY FUTURE ADDITIONS.

however, propane tanks are subject to Planning Department regulations. Such regulations primarily consist of
setbacks from shorelines and features considered to be critical areas (streams, wetlands, slopes, etc.) If you
think such features exist on or nearby your property, please contact the Planning Department so that exact

15) If the tank size is between 125 and 500 gallons you must follow these guidelines: 1. Tank is to be 10 feet from
any buidling, public way or property line. 2. If the tank is exposed to probable vehicular damage, provide
protective bollards. 3. All weeds, grass, brush, trash and other combustible material shall be kept a minimum of

108, THE OWNER OR THE ENGINEER OR ARCHITECT OF RECORD ACTING AS THE OWNER'S AGENT
SHALL EMPLOY ONE OR MORE SPECIAL INSPECTORS WHO SHALL PROVIDE INSPECTIONS DURING
CONSTRUCTION ON THE TYPES OF WORK LISTED UNDER SECTION 1701.5. THE SPECIAL
INSPECTORS DUTIES & RESPONSIBILITIES SHALL BE AS SPECIFIED IN 1701.2 AND 1701,3.

is not on site, Approval WILL NOT be granted. In addition, a Re-Inspection fee in the amount of$42.00 per

receptor." Answer. The church is doing warming and serving of pre prepared foods and is not a "Food And
Beverage Handling Establishment"as this section would lend to in section 801.2.
Item#12.Section 1014—"The project will require to have an approved grease trap.....". Answer.Has$rouck
Design Services(426-4370) designed the septic system and 1200 gallon tank for the new septic system. in order
for the work to be done, a grease interceptor was to be provided to protect the septic system. The grease
interceptor shall be JR Smith#8007. Please see the letter to Mason County Enviornmental Health dated
12115100,
Item#13. "Alan - Sheet M2-1 does not reflect a dishwasher drain receptacle....". Answer: The waste is not
shown but we will provide a detail so that the contractor-knows how to connect the waste to the sink drain. This
is a residential type dishwasher.
Items# 14. "If the church wants to have a certified kitchen,you will need to contact ... Food ffandling
Department." Answer. The church wants to have an approved kitchen for warming only but not a food handling
permit as required by a "Food And Beverage Handling Establishment". The Owner and Architect will
coordinate with the Food Handling Department.
Item#15. "Provide strapping information for the water heater." Answer: We will provide direction on the
plans to comply with seismic criteria for the entire project for zone #3. This will include equipment andpiping
bracing. 1f there are any other local requirements that we are not aware of,please let me know at(956-0384)as
soon as you can.
Item#16. "Please indicate how the combustion air will be delivered to the hot water heating unit located in
room 206. Answer: The water heater will be moved to the attic area—the water heater will have sealed

The following list contains the additional site measurements requested by Kelly Mayo
during our meeting on February 261''. I have also attached copies of the site drawing,
which has already been submitted, with the additional measurements added. Please note
the change in the distance from the front of the existing building to Shelton Springs Road.
My original measurement was incorrect.
The additional measurements are as follows:
- 129.5 ft. from the front of the existing church building to the easement
- 142.5 ft. from the northwest corner of the new addition to the easement
- 37 ft. from the northwest corner of the new addition to the adjacent
property line
- 37 ft. from the southwest corner of the new addition to the adjacent
property line
- 138 ft. from the northeast corner of the existing church building to the
adjacent property line at its closest point
- 150 ft. from the east side of the existing church building to the adjacent
property line, parallel to Shelton Springs Road
The measurement of the distance from the front of the existing church building to Shelton
Springs Road needs to be changed from 132 ft.to 162.5 ft.

I have discussed the use of the new classroom areas with Pastor Willis Barney and
he indicates that their use will be for Sunday school classrooms and no daycare will be
provided in the new area. The entire new area will be recognized as a Group A-3
occupancy.
UNIFORM BUIDING CODE
1. Section 504.6. Area separation walls are required to extend from floor surface to the
underside of the roof sheathing or to terminate with a parapet that extends 30 inches
above the roof surface as stated in Section 504.6.4. The interior area separation wall
that divides the multi purpose room from the kitchen will need to be extended to the
underside of the roof sheathing. The one-hour fire resistive assembly for the ceiling
will need to extend a distance of five foot on both sides of this wall as specified in
Section 504.6.4 item #2.1. The area separation wall that separates room 200 from the
existing church area will need to comply with Section 504.6.4 or with Section 504.6.6
item #2. This section requires that there either be a parapet wall or the existing
church will need to have the entire ceiling replaced with a one-hour-fire resistive
assembly. Please indicate your choice and provide details demonstrating compliance.
2. Please review all the detail references on the construction plans. I find that they are
not accurate. Plan sheet A-6 building section A-A indicates room 205 and room 206
please check the assigned room numbers, it appears that these should be room 207
and room 208. Building section B-B indicates room 209 should this be room 211 or
room 214? Please check these call-outs and revise.
3. Section 1003.3.1.6/ 1103.2.2. Door# 8 on the West Side of the addition does not
appear to have an accessible route of travel back to the parking area. Please revise
plans to reflect.
4. WAC Section 1003.3. Please provide a stair riser detail.
5. Section 1004.3.4.3.2.1. Doors 2, 6, 7, 9, 10, 12, 19, 19,20, 21 are required to have
smoke and draft gaskets. Please add this to the door schedule.
6. Section 1004.3.4. Provide a corridor detail showing the construction of the wall and
ceiling for room 207 Detail 12/A-10 only indicates wall construction.
7. Section 1003.3.3.9. The interior of room 206 is required to be of one-hour fire-
resistive construction. The please revise the finish schedule to reflect.
8. WAC Section 2903.4.1. / 1106.13.1. A water fountain is required to be placed in the
addition area. Please revise.
9. Section 709.7. Plan sheet E-2.1 shows electrical switches located adjacent to each
other in the area separation wall that divides room 200 and room 205. Membrane
penetrations in fire resistive walls will need to be separated by a minimum of 24
inches. Please review plans and revise. I have no plans for the existing wall that
separates the existing area. Please review the existing wall to see if the same
condition exists, also indicate the type of electrical boxes to be used. They are
required to be a rated assembly.
10. Section 709.6 / 714. All penetrations through fire resistive assemblies shall be
protected as required in these sections. Please identify how this will be
accomplished.
UNIFORM PLUMBING CODE
11. Section 801.2.3 /804. Food preparation sinks shall drain indirectly through an air-
gap to an approved receptor. Please add this information to the construction plans.
12. Section 1014. The project will be required to have an approved grease trap. Please
indicate size and area on the plumbing plans.
13. Plan sheet M2-1 does not reflect a dishwasher drain receptacle. Please indicate on the
construction plans.
14. If the church intends to have a certified kitchen you will need to contact Mr. Jess
Mosley at extension 361, Food Handling Department.
15. Section 510.5. Provide strapping information for the water heater

Please make the requested corrections and submit two sets of plans showing the
revisions. If only specific pages are affected in the revisions, only those pages need
to be re-submitted. Voided sheets shall be removed from the collated sets. Be sure to
include any additional changes that have been made since the plans were originally
submitted. All corrections and changes must be detailed in a transmittal letter and
clearly marked on the plans (clouded). The re-submittal letter and the plan revision
numbers must follow the same order as the plan review letter. Re-submittal will not
be reviewed if these items are not in the called out order.
Prior to the re-submittal you must call the Plans Examining Department to make
arrangements for re-submittal. If a meeting is needed or if you have any questions
regarding this review you may contact me at (360) 427-9670 extension 595. Please
notify the department that the re-submittal is being sent by mail. This will aid in a
timely review service. Meetings and the intake of re-submittal documents without
prior arrangements will be denied.
The issuance or granting of a building permit or approval of plans, specifications
and computations shall not be constructed to be a permit for, or an approval of, any
violation of any of the provisions or ordinances of this jurisdiction, per the Uniform
Building Code, Section 106.4.3.
Re-submitted plans are normally reviewed within five working days after the
revisions are received in our office. During the review of the re-submitted plans
additional information may be required.

In response to your letter of June 18, 2001, we are re-submitting the building plans (attached)with revisions
that address several items. The following comments describe our responses:
UNIFORM BUILDING CODE
1. The detail for the interior area separation wall between the Multi-purpose Room and the Kitchen has
been revised (see Detail 8/A10). The new detail shows more clearly the exact situation and is consistent
with the discussion we had related to Footnote 14 of Table 7-C. The detail for the area separation wall
between the existing building and the new structure has not been revised, consistent with Footnote 14
of Table 7-C. We have investigated the existing ceiling and the existing gwb is 5/8" Type "X". I have
enclosed a copy of a label that was removed from one of the panels, and though it is blurry, it is evident
it is UL classified fire resistive and gives clear design numbers that can be confirmed.
2. You are correct that the detail references were not accurate. They have been revised.
3. The Site Plan, Sheet A2, has been revised to show a sidewalk providing access from the exit to the
main front walk.
4. A stair riser detail has been added; see Detail 1/A5.
5. A note has been added indicating that all rated doors are to have smoke and draft gaskets. See Sheet
A5.
6. A detail has been added; see Detail 12/A10.
7. The Room Finish Schedule has been revised; see Sheet A5.
8. A water fountain has been added near the new restrooms; see Sheet A3.
9. This item is addressed in the comments from the M/E engineer; see attached.
10. A note has been added to Sheet A3.
