A re-pfrtilacation report for the fire sprinkler head replacement is required. X 2) Contracto registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance Division. X COM2011-00046 Page 2 of 4 5) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a M County Building Inspector shall be made prior to requesting additional inspections. X 6) All builds g permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non->c� pliant with Mason County ordinances and building regulations. X 7) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of th ermit holder have prevented action from being taken. BELFOR Construction,Attention: Ted&Steve Date of Inspection: July 28,2011 Inspecting Engineer: Jack A. Swardz,P.E. INTRODUCTION The purpose of this inspection was to evaluate fire damage,to the extent that a reasonable amount of time and conditions permit. The initial presumption is that all elements are adequate and proper. Statements,representations or conclusions to the contrary are made only when the inspecting engineer sees evidence which, in his judgment, indicates the existence of significant deficiencies. The inspection is visual only and does not involve the opening or exposure of elements concealed from view. Wherever a deficiency of any kind is identified in this report,there is always a possibility that it is associated with a larger,more serious, or more extensive problem that is hidden from view or is beyond the scope of this inspection. Consequently, you should take whatever action is necessary to ensure that the full extent and implications of identified deficiencies are known. To fully understand the importance of any deficiency,you should obtain bids for its correction. Bids for repairs vary widely. You should always obtain at least three competitive bids. Certain limitations apply to any inspection, including those diligently performed by a qualified expert. The limitations on this inspection include any noted in the body of the report together with those listed under"Limitations on Inspections" at the end of the report. We consider these limitations unavoidable. If you make any use of the information contained in this report, it is necessary that you understand and accept these limitations. IF THESE LIMITATIONS ARE UNACCEPTABLE TO YOU,YOU SHOULD NOT RELY ON THE INFORMATION IN THIS REPORT FOR ANY PURPOSE WHATSOEVER. I� Engineer's Inspection of the Property at: 111 SR106, Shelton, Washington EVALUATIONS There has been a fire in an upper level bathroom damaging some of the ceiling structure above. The ceiling is adjacent to an attic that supports some rather lightweight air handling equipment. The damaged structure consists of 12 inch TJI type members. While the damage appears somewhat dramatic, it is repairable. I recommend that the damaged TJls be supplemented by infilling the space between the web and the outside edge of the flange with a length of 3/s"plywood, about 9"deep by 8 feet long on both sides of the damaged TJI. An additional piece of 3/4 inch plywood can then be secured to the existing TJI and the infill piece on both sides of the TJI member. The supplement pieces will be about 12"deep by 8 feet long. The supplement pieces shall be secured to the existing flanges of the TJls with construction adhesive and 8d nails on 3 inch centers. The supplemented pieces can also be attached to the TJI webs and the infill pieces with some random nailing as I discussed with Steve and Crew. The existing TJIs on the bathroom side of the problem are supported by joist hangers. The existing TJIs are damaged in the vicinity of the hangers and are no longer able to support the load. A new Simpson beam hanger similar to a U610 can be used to support the overall supplemented member. There may be a need to shim or modify the system to make it fit. LIMITATIONS ON INSPECTIONS The limitations on this inspection include those discussed throughout the report,together with those listed in the following paragraphs. ACCESSIBILITY OF DEFICIENCIES: Statements,representations,or conclusions offered by the engineer who conducts the inspection and/or by Pacific Northwest Inspection Engineers are based solely upon a visual examination of the exposed areas of the structure inspected. Hidden defects could have a significant impact on the visually based conclusions, statements, and representations made by the inspector.
