Present at site: Nicolas Afeiche, Structural Engineer of Record
Jim Fountaine, Superintendent
On site at 1:30 p.m. to perform construction observation of new continuous footings at the primary and
intermediate restrooms, and to observe injection of epoxy and installation of reinforcing dowels.
Cleaning out of holes with pressurized air, injection of epoxy and installation of all reinforcing dowels
were performed in my presence. Installation of epoxied reinforcing dowels is acceptable.
Size and reinforcing of continuous footings are in compliance with the approved construction documents.
Note: In order to remove existing damaged ledger at the primary rstroom, Contractor had to sawcut
existing expansion bolts. Contractor may provide either expansion bolts or epoxied threaded rods with a
6-inch minimum offset from existing holes. Epoxied threaded rods shall be used where edge of concrete
is within 6 inches from bolt location. Contractor shall refer to Detail(6 / S2) for size and embedment of
connector.
I left the site at 2:15 p.m.
NOTE: General Contractor shall forward copies of Field Report to job superintendent and subcontractors
as appropriate. Items noted of concern or as deficient shall be considered as notification for the Contractor
to address and/or resolve as requested. Any corrections, additions or objections to this report must be filed
with the Structural Engineer of Record in writing within 7 calendar days of the report date.

On site at 2:00 p.m. to perform construction observation of the new structural framing at the Primary,
Intermediate and Health Restrooms,prior to placing plywood floor sheathing.
Primary Restroom:New plumbing chase walls, ledger at concrete wall, floor posts, beams and joists are
in place. Structural framing is in compliance with the approved construction documents.
• I reminded Jim F. that the floor area under the janitor's room shall be treated, as indicated on the
contract drawings.
Intermediate Restroom:New plumbing chase walls and perimeter edge blocking are in place. Structural
framing is in compliance with the approved construction documents.
• Due to the presence of ducts near the ceiling, the plumbing chase walls were not extended to the
roof decking, as shown on DETAIL 5 /S2. I asked Jim F. to provide(2)additional 2x6 horizontal
braces. Nail braces to top of plates of new walls and to bottom chords of adjacent existing girder
trusses.
• I told Jim F.that existing floor joists, within the new plumbing chase,do not have to be removed.
Health Restroom:New minor floor framing and blocking are in place. Structural framing is acceptable.
I left the site at 3:10 p.m.
