The contractor shall add the following interim temporary erosion and sediment control measure to field
report#2,effective immediately:

DISCUSSI O MOBSERVATION
Onsite meeting with John Sliva, Mason County, Gary Larson, GAI.Cu Hngr., Steve Brannon, Jones &c
Roberts, and Keith Tibbets, SECO, to inspect the onsite stormwater wet-pond, infiltration pond and
biofiltration Swale. These facilities are not complete at this time. The wet pond liner is installed and the
wet pond is full of stormwater, but the wet pond berm/gravel filter berm has not been constructed yet,the
bottoms and side slopes of the pond facilities have not been seeded and no vegetation has been
established, and the biofiltration swale has not been finish graded and properly vegetated. Given the
current wet weather conditions and give", we are starting the wet season, it was agreed by all parties
present that it is impractical to attempt to complete the wet pond berm construction and biofiltration
construction at this time,and completion may be deferred until the dry season 2008.
John Sliva, Mason County, indicated that the County would uccept the current, incomplete condition that
the pond facilities and biofiltration facility are in with the provision that written temporary erosion &c
sediment control installation requirements (TESL) and maintenance; requirements be provided to the
County outlining the temporary measures to be put in place at this time in order to prevent wet season
erosion and degradation of the facilities, until such time next year (2008) when these facilities are
completed, during the dry season.
Therefore, the fallowing is an outline of interim requirements the Contractor is required to perlbrm,
efTectively immediately:
(1) Apply temporary erosion control seed ntix and ntinitnum 2"-3" thickness of straw mulch to promote
grass establishment, on all areas between access road and pond. Alternatively,apply seed mix and cover
awith plastic cover to promote grass establishment. (Note: mulch or plastic cover are required until 75%
grass cover is established);

(2) Apply temporary erosion control seed niix and tight-woven,jute netting or matting on all pond side-
slopes to promote grass establishment (Caution - care must be exercised when staking jute
netting/matting so as to not puncture the PVC liner installed with the wet pond side slopes).
Alternatively, apply seed alix and eover with plastic to promote grass establishment. (Note: jute
netting/nlatting or plastic cover are required until 7504.grass cover is established);
(3)Install two rock check-dams,evenly spaced along the biofiltration Swale bottom, perpendicular to the
flow of water. Check darns are to be minimum 12" in height and constructed of crushed rock with slopes
of2.:1 maximum;
(4)Remove all sediment within infiltration pond and scarify bottom to restore infiltration function;
Upon completion of initial installation of the above measures, the contractor shall maintain these
measures throughout the wet season as follows:
(A) Inspect all measures on a weekly basis, and after all runoff-producing storm events between October
1" and April 30", (inspect all measures on a monthly basis (minimum) and within 48 hours of a storm
event, between May 1"and September 30".)
(13) All mulched areas must have mulch added as required to maintain minimum cover thickness until
grass cover is established.
(C)Remove sediment fi-om rock check dams when sediment height equals one-half the check danl height.
(0) Remove sediment from infiltration pond facility immediately upon observing, and maintain a
scarified pond bottom surface. The contractor is to exercise extra care when working within the
infiltration pond so as to not compact the pond bottom and adversely affect its infiltration function.
(E)Maintain all pipe out-fall protections, and remove:sediment buildup as required.
All pond and bioflln•atiorl swale facilities construction shall be completed, inspected and accepted, per
contract documents, within the dry season of 2008, and all permanent erosion and sediment control
provisions will be required to be installed and established prior to the start of the 2009 wet season.
