COM2005-00135
1) Changes to approved building plans that affect compliance to the current Washington State Energy Code (WSEC), ventilation and Indoor Air
Quality Code (VIAQ), Building/Plumbing/Mechanical Codes and/or Mason County Regulations shall be approved prior to construction.

2) This project is approved without structural changes to the building. Exhaust systems required by the manufacturer of the spray booth shall not
affect structural members and shall maintain required setback from combustible materials as required by the manufacturer installation
specifications.

3) Lighted exit signs are required at all exit doors. Battery back-up emergency lights are required in all storage areas and exit corridors.

4) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE
ADOPTED BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in
conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a
Mason County Building Inspector shall be made prior to requesting additional inspections.

5) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The
failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being
non-compliant with Mason County ordinances and building regulations.

6) The requirements of the International Fire Code chapter 26 "Welding and Other Hot Work" shall be adhered to. A copy of the chapter has been
included with the review. X
An automatic fire alarm system meeting NFPA 72 and Mason County standards and monitored by an UL certificated monitoring company shall be
installed in this building. Information indicates a disconnected system currently exists that may need upgrading. Separate plan and permit is
required. X -
A rapid access "Knox" key box is required for this building. Contact Mason County Fire District# 11 at 360-426-1822 for an application and
mounting instructions. X
Minimum 3A40BC rated fire extinguishers are required to be within 30 feet travel distance in approved locations and mounted no more than 48
inches from the finished floor to the top of the extinguisher. X
Fire apparatus access roads chapter 14.17 of the Mason County Building Code shall be adhered to.X
7) This permit is approved for the UL listed spray booth only. A separate permit is required for the fire suppression system which will be reviewed by
and approved by the Mason County Fire Marshal. The fire suppression system shall be connected to the building alarm system. A modification of
the existing fire alarm system may be required. The UL listed spray booth is not approved for use until the fire suppression system is installed and
tested.

8) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely
impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements
of the stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose.
For further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a
driveway or access connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450.
For any construction which is proposed to be located within 25'of a Mason County road right of way, it is suggested to contact that office to review
future planned work which may affect your project.

9) The applicant understands that they must contact the Olympic Region Clean Air Agency for a permit to operate and annual registration.

10) All approved plans are required to be on-site for inspection purposes. If inspection is called for and plans are not on site, Approval WILL NOT be
granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and collected by the Mason County
Building Department prior to any further inspections being performed or approvals granted. X
11) PURSUANT TO INTERNATIONAL CODE, ALL SITES MUST HAVE APPROVED NUMBERS OR ADDRESSES PROVIDED IN SUCH A
POSITION AS TO BE PLAINLY VISIBLE AND LEGIBLE FROM THE STREET OR ROAD FRONTING THE PROPERTY. MASON COUNTY
BUILDING DEPARTMENT REQUIRES THAT THIS BE COMPLETED PRIOR TO CALLING FOR ANY SITE INSPECTIONS. A REINSPECTION
FEE, BASED ON RATES AS ADOPTED BY THE JURISDICTION AND THE INTERNATIONAL CODE WILL BE ASSESSED IF
OWNER/CONTRACTOR FAILS TO POST ADDRESS ON SITE PRIOR TO REQUESTING INSPECTIONS.

12) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance
Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be
obtained at 1-800-647-0982. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to
WA state law. X

14) Recyclable materials & Solid Waste Storage: Space shall be provided for the storage of recycled materials and solid waste. The storage area
shall be designed to meet the needs of the occupancy, efficiency of pick-up, and shall be available to occupants and
haulers.X
15) Welding, cutting and similar spark-producing operations shall not be conducted in or adjacent to spray areas or dipping or coating operations
unless precautions have been taken to provide safety. Conspicuous signs with the following warning shall be posted in the vicinity of spraying area,
dipping operations and paint storage:

THE USE OF WELDING OR CUTTING EQUIPMENT IN OR NEAR THIS AREA IS DANGEROUS BECAUSE OF FIRE AND EXPLOSION
HAZARDS. WELDING AND CUTTING SHALL BE DONE ONLY UNDER THE SUPERVISION OF THE PERSON IN CHARGE.

16) A new natural gas heater installed per 2003 International Mechanical Code is approved with a thermostat that limits heat to 44 degrees fahrenheit
or less.

This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at any
time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be
occupied. Proof of continuation of work is by means of a progress inspection.The owner or the agent on the owners behalf, represents that the information provided is accurate

Complete the Change in Tenant Application and return with a floor plan,site plan,septic pumper's report,septic records and
fee to the Mason County Permit Center, P.O. Box 186, Shelton,WA 98584. Evaluation of the Change in Tenant Application will involve
staff members from the Building, Fire Marshal, Environmental Health, Planning and Public Works offices who will identify compliance

Pursuant to Mason County Code, Title 14 Building and Construction, a permit and
final inspection for this type of activity is required under the 2006 International
Building Code or the code your permit was issued and your property is currently in
violation status of occupancy and use.
Please contact our office to make the necessary arrangements 21 days from
the date of this letter. Failure to contact our office to make the necessary scheduled
inspections will result in enforcement actions.
To bring your site into compliance, you must schedule an inspection. One (1) $64.00
site investigation fee will need to be paid prior to inspection along with any
outstanding fees currently due on your building permit. For every inspection required
after that, you will be charged $64.00 again, per inspection until final inspection and
conditions are met.

SUBJECT: CHANGE OF TENANT APPLICATION
You will find attached a Change of Tenant Application being submitted by Gary Yando
for Jim Ness of Extreme Auto Body. Mr. Ness is proposing to change the use of the
existing building located on Shelton Springs Road. The existing building has been used
for the past four years for repairing vehicles and also for the storage of antique vehicles
and recreational vehicles.
It is Mr. Ness's intent to use the building for auto body repair work.
You will find attached five copies of the required application, a layout of the building and
the paint booth location. In addition, you will find a blow up of the existing building and
a drawing that shows the locations of the drainage facilities that are already in place. I
might add that there is an approved Stormwater and Erosion and Sediment Control Site
Plan for the site. There will be no additional impact as a result of the proposed use.
You will find enclosed a check in the amount of$127.00(as required by staff).
We are requesting that the necessary steps be taken to process this application in a timely
manner.
