' ' Type By Date Amount Receipt Total CASE NOTES FOR COM2005-00069 CONDITIONS FOR COM2005-00069 1) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at 1-8004 -982. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to WA state law. X 2) Prior to final appaval, all upland areas disturbed or ne ated by construction activities shall be seeded,vegetated or given an equivalent type of erosion protection (silt fencing or straw matting). X 3) Temporary erosion control measures must be impl ented to prevent water qual' radation of adjacent waters or wetlands. Silt fencing must be installed and maintained until upland vegetation has become established. X 4) Ap per dimensions and setbacks on submitted site plan. Setbacks ar easured from the furthest projection of the structure. If inspection is called f nd plans are not on site, Approval WILL NOT be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour I charged and collected by the Mason County Building Department prior to any further inspections being performed or approvals granted. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Ma s County Building Inspector shall be made prior to requesting additional inspections. X 9) Aaroperty lines shall be clearly identified at the time of foundation inspection. X 10) All building permits shall have a final inspection performed and approved by the?asonCounty Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non- pliant with Mason County ordinances and building regulations. X 11) M st have State DOH water adequacy prior to temporary/final occupancy. X This permit comes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at anytime after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. TRENCHES Customers must provide all trenches along private roads, right-of-ways, county roads and state highways. Trenches provided by the customer or contractor must meet the following requirements. 1) Customer to call PUD superintendent to schedule job after proposal has been accepted and a work order released. PUD will notify the customer when the work order has been released. (Please provide PUD with a daytime phone number.) 2) Customer to call for UNDERGROUND UTILITY LOCATES AT LEAST 48 HOURS PRIOR TO DIGGING - 1-800-424-5555. Once marked the excavator is responsible for maintaining the markings. 3) Trenches should be adequate width for the installation as determined by the PUD. (See attached ditch specification.) Trench shall be a minimum of 3 feet deep. The bottom of the Trench shall be level and free from all obstructions. 4) Trench should not be dug prior to scheduling with the PUD superintendent. Trenching within two feet of primary cable should be done by hand with PUD crew on site. 5) Backhoe and operator must be on site at time of power installation to backfill ditch when installation complete. 6) The PUD will install the primary cable in the ditch and the customer shall cover the cable with at least 1 foot of select backfill prior to the cable being energized. Backfill will usually be selected from material excavated from ditch. 7) The customer may then backfill the remaining ditch. Clearances between district cables and other conductors, cables, or pipes shall not be less than 1 foot. 8) If the trenching is on county, City of Shelton or state right-of-way, and if the trenching is not provided by a contractor on the enclosed list of pre-qualified small works contractors then special arrangements need to be made with the PUD Engineering Department. Trenching in county, City of Shelton or state right-of-way shall conform to specifications, locations and distances specified in county, City of Shelton or state road permit. Depending on the number of floats for your application, some functions may be omitted or combined. High Level Alarm: This float activates the alarm light(steady)and audible alarm when lifted.The audible alarm may be silenced by pressing the illuminated PUSH TO SILENCE button on the front of the control panel.The alarm light(steady)will remain on until the float is lowered. Lag Pump Enable: This float will activate both pumps when the timer function enters the on cycle.Both pumps will continue to run together until the lag pump enable float lowers. Note:If used this function must be combined with either the High Level Alarm,Override Timer On&Off or both. Override Timer On&Off: This float activates the override timer function when lifted for more than 2 seconds. This timer function controls the pump cycles during high flow conditions.The override timer function will remain active until at least the set minimum number of override cycles have been completed and the float has lowered.When the override timer function has been completed,normal timer operation will resume.
