X COM2002-00006 2 of 4 6) As indicated on sheet S1.0 or the approved building plans, the structural engineer of record, Jon Dietrich, shall perform inspection of roof diaphragms, drag struts, chord splices, and boundary nailinig prior to application of roof building paper or floor toppings. Inspection of shear wall nailing and holdwon connections shall be made prior to concealment by wall GWB or suspended ceilings. The engineer's report shall be submitted to the Mason County Building Department prior to the final inspection of this project. I X 7) 1997 UBC CHAPTER 17, SECTION 1701: IN ADDITION TO THE INSPECTION REQUIRED BY SECTION 108, THE OWNER OR THE ENGINEER OR ARCHITECT OF RECORD ACTING AS THE OWNER'S AGENT SHALL EMPLOY ONE OR MORE SPECIAL INSPECTORS WHO SHALL PROVIDE INSPECTIONS DURING CONSTRUCTION ON THE TYPES OF WORK LISTED UNDER SECTION 1701.5. THE SPECIAL INSPECTORS DUTIES & RESPONSIBILITIES SHALL BE AS SPECIFIED IN 1701.2 AND 1701.3. (::�_ t� 8) Adhesive anchor installations require special inspection in accordance to UBC section 1701. The special inspector must record the product description, adhesive expiration date and concrete type and strength, anchor diameter and grade steel, show compliance of drill bit type, hole diameter and location, cleanliness of hole and anchor, adhesive application and anchor embedment as specified in section 2.5 of the evaluation report attached to the approved building plans. The report shall contain all other required information listed in the evaluation report and submit to the Mason County Building Department prior to the final inspection of this permit. The first batch of photos illustrates shearwall construction for the north wall in Area 'A'. Shearwall construction appears acceptable as constructed. The next batch of photos verifies the existing anchor bolts on the west wall in Area 'A'. Photos additionally illustrate the installation of the holdowns in the west wall and new epoxy anchor bolts to provide W'diameter anchor bolts at a minimum spacing of 32"o.c. A site visit was made on the morning of May 2. A visual observation was made of the shearwall construction and nailing for the east hallway wall of Area 'A' and the south hallway wall of Area 'B'. Shearvall construction was acceptable as constructed. This report completes structural observation services. All structural elements appear to have been constructed per Contract Documents. There is an existing exhaust fan serving that space which is not operating as the occupants feel it should.
