1) 1. A stormwater site plan with low impact development features such as pervious paving, rain garden infiltration and native vegetation, will be
` created and implemented prior to development, clearing, grading or construction of any portion of the proposed development.
2. The proposal shall be required to control erosion during construction. Removal of vegetation should be minimized and any areas disturbed
should be restored to prevent erosion and other environmental impacts.
3. Site is within a portion of the county designated as Class II Critical Aquifer Recharge Area, under Critical Aquifer Recharge Area Chapter
17.01.080 of the Mason County Resource Ordinance. All provisions of the CARA chapter shall be followed including obtaining a Mason
Environmental Permit, application of Best Management Practices on site, and an engineered stormwater site plan in compliance with Ecology's
stormwater, water quality, Hazardous Waste, wetland and solid waste program BMP, and BMPs from the departments of Health, Agriculture,
Transportation and State Conservation District Office.
4. Project applicant shall develop in compliance with Geotechnical report completed for this site by Aspect Consulting dated January 22, 2007 and
updated March 26, 2007.
5. In accordance with discussion between Squaxin Tribe and Mason County at meeting on April 9 2007, Mason County and the Public Works
department propose to work with the Squaxin Indian Tribe to craft a Memorandum of Understanding between the two governments, relating to
proposed project measures, which further reduce impact to the aquifer beyond that of usual development impact through a series of additional
measures. These proposed measures and the memorandum are being developed, but would include such measures as low impact development,
and minimizing water withdrawal through conservation measures, pervious pavement, stormwater infiltration, rain gardens and native vegetation,
and proposing to h oil up.to City of Shelton water when it becomes available. Although this proposed Memorandum of Understanding is still under
way as of this da it
iie understanding of the parties that it shall be completed and in place prior to the issuance of building permits for this
project.x

8) Any cha' ges in construction shall be reviewed by engineer of record and submitted in writing to the Mason County Building Department prior to
construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are
removed, ap o al will not be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and

9) CONS UCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE
ADOPTED BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in
conform ith the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a
Mason ui g Inspector shall be made prior to requesting additional inspections.

10) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The
failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being
non-com n with M son County ordinances and building regulations.

13) This project is approved with a fire-sprinkler and monitored alarm system installed in accordance to Fire Marshal approval. The sprinkler system

14) The approval of this project is subject to the recommendations and specifications outlined in the attached geotechnical report or assessment.
Structures and /or land modifications (grading, cuts, fills, etc.) required in the geotechnical report/assessment, may require a seperate permit.
The geotechincal report/assessment shall remain attached to the approved building plans.
Site develop t shall comply with the recommendations outlined in the geotechnical report completed for the site, prepared by Aspect Consulting
dated Janua 2, W7 and updated March 26, 2007. Inspection report(s), verifying that site development, has been completed in accordance to

15) Provisio4 for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely
impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements
of the stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose.
For further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a
driveway or access co cting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450.

27) Washington tate Energy Code Compliance is approved as follows:
ENVELOPE: Prescriptive Table 13-1, Other fuels
The building envelope shall be insulated as follows: Wall insulation R-19, Ceiling insulation R-30, slab insulation min R-10, windows U-.45 or less,
doors U-.60 or less. All insulation shall be installed to the full depth of required R-value and shall not be compressed. Evidence of u-factor for
windows and doors shall be available for inspection. R-10 slab insulation shall be installed around the entire perimeter of the heated space and
shall be installed so that insulation begins at the top of the slab and extends downward to the top of footing or 24-inches.

28) The mechanical system for which energy consumption, performance, or mode of operation are regulated by the 2001 Washington State Energy
Code (WSEC) and Ventilation And Indoor Air Quality Code (VIAQ) shall be tested to ensure that control devices, equipment and systems are
calibrated, adjusted and operate in accordance with approved plans and specifications. Drawings and notes shall require commissioning in
accordance with WSEC section 1416.4.
Lighting controls shall 0,tested to ensure that control devices, components, equipment and systems are calibrated, adjusted and operate in
accordance with the p ved plan and specifications. A complete report of test procedures and results shall be prepared and filed with the

We have specified a fixed in place, fold down seat for the Accessible
Shower in the men's and in the women's Shower Rooms and are asking if a
portable shower seat is allowed to substitute for this fold down unit
(B-5181 by Bobrick)?

Are portable seats that you could find in Healthcare Service Retail f�.
Sales allowed as a substitute for the fold down permanent seats? The
reason that we are looking to substitute is because the permanent wall
mounted fold down seats are difficult to clean, can not be positioned to
best suite the transfer and we have had one situation where an
individual slid off the fixed seat and as a result, never used it again.
Let me know if a substitution is allowed.
