PRE-APPLICATION CONFERENCE REQUEST FORM
The purpose of the pre-submission conference is to identify and/or eliminate as many potential problems regarding a
proposal. Representatives from the Fire Marshal's office, Building Department, Environmental Health, Planning, and
Public Works Departments may attend the meeting to discuss rules and regulations applicable to the proposed project.
Topics covered during the meeting may include the comprehensive plan, land divisions, shoreline program, zoning,
availability of sewer and potable water, development concepts, building construction, fire protection and life safety
concerns of the proposed project. The pre-submission conference is intended as an informative meeting. The intent is to
provide detailed information to the applicant in regards to codes and process.
Incomplete applications will not be accepted. If application is deemed incomplete it will be returned.
All Conferences are on Mondays (except holidays)
To schedule a meeting you must complete the requirements for submittal including the required six (6)
copies of detailed site plan(max, size 1 I"x17").
To submit either by mail or in person at: Mason County Permit Assistance Center at 615 W Alder
Street, Shelton WA 98584 by 3:00 PM on the Monday two weeks prior the proposed meeting.

RE: MCPW overall project site work
Earlier this month Mason County Public Works requested a change to the
entry road that may require a copy or the original stamped and signed
drawings be resubmitted for your review. Please let me know your
procedure for this resubmittal. On Wednesday I will be meeting with
County Public Works until 11 AM or so and afterwards will stop by your
office to check on the status of the project overall.
The drawings indicate a minor realignment of the roadway within the
project property and elimination of the roundabout for a Y intersection
with sweeping curves. The contractor's initial response was that it was
minor and would not add cost to the project. The drawings are on Helix
ftp site and can be viewed there if needed. A hard copy with original
signature and stamp is anticipated to be delivered to you for review if
needed.

RE: Pre-Application Request Form for Road Reconstruction/Realignment Options for West
Mackinaw Road
Dear Sir and/or Madam,
Please find the attached materials required for submittal in requesting a pre-application meeting with Mason
County Staff in regard to realignment/re-establishment options as it pertains to West Mackinaw Road on the
West of Lake Cushman. This easement roadway was constructed many years ago, with the intention of serving
water front properties at the time of development around Lake Cushman. These lots were sold and marketed as
rustic lots,off of the grid, with no formal water, sewer or other utilities to serve these lots. The establishment
easement roadway was constructed very informally and due to shoreline erosion factors, several areas along the
access roadway have failed and Lake Cushman Maintenance Company are pursuing options in regard to
realignment and/or re-establishment of sections of this easement roadway in order to serve the existing lots in
this area.
Attached are the required materials needed in order to request a pre-application meeting with Mason County
Staff, Lake Cushman Maintenance Company, Lake Cushman Company and LCMC's Consultants. Please let me
know at your earliest convenience when we might be able to schedule this pre-application meeting so that I can
coordinate Staff and others on our end in order to make the day and time established for this meeting. Once we
are able to learn more from Mason County on feasible options, as it pertains to West Mackinaw Road, work will
happen quickly in order finalize a design and begin permitting associated with this work. I am eager to hear back
from Mason County and appreciate the opportunity to submit these materials for your consideration.

Narrative
The plan shows one proposed road realignment location ("Alternative 1") for W. Mackinaw
Road, with several potential options that are being considered as adjustments to the final road
location. Alternative 1 would begin to deviate away from the existing road on Lot 30, owned by
the City of Tacoma, and would begin to gain elevation and create greater separation from Lake
Cushman.The intent is to provide a stable access road for all lots and minimize potential future
failures into the lake.
- Options 'A' and 'B' are potential simple adjustments to segments of the Alternative 1
location.
- Option 'C' would involve repairing the first section (Approx. 1,500LF) of the existing W.
Mackinaw Road alignment before being relocated further upslope and away from the lake.
As noted on the plans, at least one repair segment of the existing road would require more
significant geotechnical review and design. A geotechnical engineer is already on the project
team and has some preliminary design options for such repairs.
- Option V would include revising access for Lot 7-9B to come from Cushman Ridge Road
as opposed to W. Mackinaw Road.This would be similar access utilized for Lot 10.
The main Alternative 1 alignment and all options are technically still within 200-feet of the
shoreline and extend across erosion hazard areas so avoidance of these critical areas are
unavoidable. Removal of trees within the project area is unavoidable as well since this will be
necessary to construct any new segment of road. Road crossings of seasonal streams is also
unavoidable as the stream(s) bisect the access route. Construction activities directly within the
shoreline will try to be avoided or minimized to the fullest extent practicable to construct any
potential improvements to the existing W. Mackinaw Road, such as if option 'C' is selected.

Washington Department of Natural Resources (DNR).
