Description of Proposal: The proposal is for the construction of 2 new pipelines in a Critical
Aquifer Recharge Area as part of the Shelton Regional Water and
Wastewater System. The pipelines will convey wastewater and
reclaimed water between the Washington Corrections Center, the
Washington State Patrol Academy, and the proposed Shelton
Water Reclamation Plant.

1) Water quality is not to be degraded to the detriment of the aquatic environment as a result

Construction of the proposed pipelines is scheduled to begin in mid-2007. The project will take
approximately 6 to 9 months to construct.
7. Do you have any plans for future additions,expansion,or further activity related to or connected with

Yes. The proposed project is described in the "Shelton Regional Sewer Plan — Wastewater Facility
Plan" (Cosmopolitan Engineering Group, April 2006). The proposed pipelines will convey
wastewater and reclaimed water between the Washington Corrections Center (WCC), the
Washington State Patrol (WSP) Academy, and the proposed Shelton Water Reclamation Plant
(WRP). Construction of the WRP and related improvements at WCC and WSP will be performed as a
separate project with a separate environmental review process with the City of Shelton acting as the
lead agency.
8. List any environmental information you know about that has been prepared,or will be prepared,
directly related to this proposal.
An Environmental Report was prepared for the Shelton Regional Water and Wastewater System
("Environmental Report for the Shelton Regional Water and Wastewater Plan"—Parametrix, Inc.,
May 2005). This Environmental Report covered both the proposed regional water and wastewater
systems. EPA (April 20, 2006)and USDA—Rural Development(October 5, 2005) have both issued a

Finding of No Significant Impact(FONSI)for the portion of the Environmental Report that describes
the proposed regional wastewater system. Copies of the FONSI determinations are attached to this
checklist. (A determination for the proposed regional water system has not yet been issued by the
federal agencies)
9. Do you know whether applications are pending for governmental approvals of other proposals
directly affecting the property covered by your proposal? If yes,explain.
The City is in the process of securing franchise agreements from WSDOT to allow the installation of
the pipelines within WSDOT right-of-way along SR-102. Easements for the pipeline alignment
through Port of Shelton property are also in process.
10. List any government approvals or permits that will be needed for your proposal,if known.
Washington State Department of Ecology and Health approvals for the contract documents for the
construction of the WRP will be required. Health's approval of the Facility Plan for the project is
pending (as of April 19, 2007). Franchise agreements with WSDOT will need to be secured for the
pipelines.A Mason County Environmental Permit will be required.
11. Give a brief,complete description of your proposal,including the proposed uses and the size of the
project and site. There are several questions later in this checklist that ask you to describe certain
aspects of your proposal.You do not need to repeat those answers on this page. (Lead agencies may
modify this form to include additional specific information on project description.)
The proposed project consists of two pipelines:
1. A raw sewage pipeline will be constructed to convey wastewater from WCC and WSP to the
proposed Shelton Water Reclamation Plant. The pipeline will consist of approximately 9,200 feet
of 10-inch force main and 11,900 feet of IS-inch and 18-inch gravity sanitary sewer.
2. A reclaimed water transmission main will convey reclaimed water from the WRP to WSP and
WCC for beneficial reuse. Reclaimed water that is not reused will be conveyed to the existing
WCC sprayfield in order to implement the land application of Class A reclaimed water for
groundwater recharge. The pipeline will consist of approximately 24,300 feet of 12-inch HDPE
transmission main. Construction of the reclaimed water pipeline and its connection to the
existing WCC sprayfield will result in the existing sprayfield remaining in service once the WCC
treatment plant is taken off line.
The pipelines will be installed in easements secured from the Port of Shelton and in public right-
of-way along SR-102. Pipelines will be constructed inside the Port of Shelton to extend the
gravity sewer and reclaimed water pipelines towards Shelton Springs Road. The last portion of
these pipes would cross a privately-owned parcel in an easement that has yet to be secured.
12. Location of the proposal. Give sufficient information for a person to understand the precise location
of your proposed project,including a street address,if any,and section,township,and range,if
known. If a proposal would occur over a range of an area,provide the range or boundaries of the
site(s). Provide a legal description,site plan,vicinity map,and topographic map if reasonably
available. While you should submit any plans required by the agency,you are not required to
duplicate maps or detailed plans submitted with any permit applications related to this checklist.
See the attached Figure I (vicinity map) and Figure 2 (topographic map). Preliminary construction
drawings for the pipeline alignment are attached.
The pipelines will be constructed along easements and public right-of-way in Sections 2, 3, 4, 9, 11,
and 12 of Township 20N,Range 4 West, W.M.

B. ENVIRONMENT ELEMENTS
1. Earth
a. General description of the site(circle one): flat, olling,hilly,steep
slopes,mountainous,other.
b. What is the steepest slope on the site(approximate percent slope)?
The pipeline alignments are along ground that is generally flat. Total
elevation gain over the entire alignment (more than 20,000 feet) is
approximately 45 feet.
c. What general types of soils are found on the site(for example,clay,sand,
gravel,peat,muck)? If you know the classification of agricultural soils,
specify them and note any prime farmland.
A soils report has been prepared for the project. Summary findings of
the report are as follows: Project site is generally underlain by Vashon
Recessional Outwash. A portion of the project site within the Port of
Shelton is underlain with Vashon Glacial Till. Topsoil typically extends
to a depth of 3 to 6 inches. Horizon B topsoil extends to a depth of about
3 feet below the topsoil and generally consists of a loose mixture of silt
and sand with varying amounts of roots. The Vashon recessional
outwash underlays the topsoil layers and consists of medium dense to
dense sand and gravel with trace amounts of silt.
No agricultural soils were noted in the soils report and the project does
not cross prime farmland.
d. Are there surface indications or history of unstable soils in the immediate
vicinity? If so,describe.
There is no known history of unstable soils.
e. Describe the purpose,type,and approximate quantities of any filling or
grading proposed. Indicate source of fill.
Pipe excavations will be backfilled to existing grade. No additional
filling or grading is proposed other than to restore the ground along the
pipe alignment to the pre-existing condition.

Yes, erosion could occur on cut slopes and stockpile areas if erosion
measures are not taken. The contractor will be required to implement
temporary erosion and sedimentation control measures, consistent with
City of Shelton standards during clearing and construction operations.
g. About what percent of the site will be covered with impervious surfaces
after project construction(for example,asphalt or buildings)?
No new impervious surfaces will be created by the construction of the
project.
h. Proposed measures to reduce or control erosion,or other impacts to the
earth,if any:
Measures to minimize erosion during construction will include covering
stockpile areas, placement of silt fence, and covering of stripped areas
with straw. Disturbed areas will be revegetated as soon as possible after
construction.
2. Air
a. What types of emissions to the air would result from the proposal(for
example: dust,automobile odors,industrial wood smoke)during
construction and when the project is completed? If any,generally
describe and give approximate quantities if known.
During construction, emissions could potentially include dust and
exhaust from equipment.
b. Are there any off-site sources of emissions or odor that may affect your
proposal? If so,generally describe.
No.
c. Proposed measures to reduce or control emissions or other impacts to air,
if any?
Gravity manhole covers along the pipeline will be unvented in order to
minimize the escape offugitive odors from the sanitary sewer.
3. Water

The soils report for the project states that dewatering of trenches to
remove groundwater is not considered necessarily likely. If
excavation dewatering is found to be necessary, water that is
pumped from trenches will be treated and disposed of consistent with
WSDOT, Mason County, Port of Shelton, and City of Shelton
standards.

Class A reclaimed water that is generated at the WRP and conveyed
through the reclaimed water transmission main will be put to
beneficial use at WCC or the WSP Academy or land applied to the
surface at the existing WCC sprayfield to recharge groundwater.
This sprayfield currently is used for disposal of treated wastewater
from the WCC treatment plant, in conformance with the
requirements of the plant discharge permit. Reclaimed water that is
not taken up by trees on the site via evapotranspiration will
ultimately reach the groundwater below the site and then migrate
away from the site. The WRP will be sized to treat a maximum
monthly flow of 400,000 gallons per day. This is the same flow that
the existing sprayfield is permitted to receive per the State Waste
Discharge Permit for the WCC treatment plant issued by the
Department of Ecology. New Reclaimed Water and State Waste
Discharge Permits will be issued by Ecology and Health for the
change in use at the existing sprayfield.
2) Describe waste material that will be discharged into the ground from
septic tanks or other sources,if any(for example: domestic sewage;
industrial,containing the following chemicals: ;
agricultural: ;etc.). Describe the general size of
the system,the number of such systems,the number of houses to be
served(if applicable),or the number of animals or humans the
system(s)are expected to serve.

c. Water Runoff(including storm water):

Trees that are removed along the Port of Shelton pipeline alignment will

According to the Environmental Report, the only listed wildlife species in
the vicinity of the project is the bald eagle. In addition, the Shelton
pocket gopher, which is reported to be a candidate for listing as
threatened or endangered, has been documented to be present at Scotts
Prairie at Sanderson Field.

The original ground surface along the proposed alignment has largely
been disturbed by prior construction activities for the State highways and
Port utilities. A cultural resources survey was prepared as part of the
Environmental Report for the Regional Water and Wastewater System. A
search of files at the Washington State Office of Archaeology and
Historic Preservation showed that no archaeological sites are located
within 2 miles of the project area.
c. Proposed measures to reduce or control impacts,if any:
An Unanticipated Discovery Plan will be prepared and in place prior to
the beginning of construction.As stated in the Environmental Report, `In
the event that ground disturbance or other construction activities result
in the inadvertent discovery of archaeological and/or cultural resources,
work will be halted in the immediate area and contact made with Mason
County officials, the office of the State Historic Preservation Officer
(SHPO), and Squaxin Tribal Cultural Resources Department (CRD). In
the unlikely event of the inadvertent discovery of human remains, work
will be immediately halted in the discovery area, the remains covered
and secured against further disturbance, and communication established
with County administrative and law enforcement personnel, the office of
the SHPO, and representatives of Squaxin Tribal CRD. All work will be
halted until further investigation and appropriate consultations are
concluded."
14. Transportation
a. Identify public streets and highways serving the site,and describe
proposed access to the existing street system. Show on site plans,if any.
The pipeline alignment from the Port of Shelton to WCC will be in
WSDOT public right-of-way along SR-102. Through the Port, much of
the alignment is along existing Port roads.

A portion of the pipeline alignment will cross the area immediately east

will require that the Contractor prepare an Aviation Construction Safety
Plan and that the plan be reviewed and approved by the Port of Shelton
and the City of Shelton.

applicable to new development and redevelopment sites. Per Mason County requirements,the Minimum
Requirements are:

Minimum Requirement#1: Preparation of Stormwater Site Plans
The Stormwater Site Plan for the referenced project is attached. The stormwater plan for the pipeline
project is very simple as the project will not impact stormwater and will not increase stormwater runoff
from the impacted area. The only potential adverse stormwater impacts will be during pipeline
construction,and the BMP's shown on the construction plans and required by the specifications will
mitigate these potential impacts.
Minimum Requirement#2: Construction Stormwater Pollution Prevention
Element#1: Mark Clearing Limits

The marking of clearing limits is not applicable to this project as there is no clearing associated
with the pipeline project. All construction will take place within SR-102 right-of-way or within a
specified easement on Port of Shelton property.
Element#2: Establish Construction Access
Dedicated construction access is not required for this project,as all pipeline construction takes
place adjacent to SR-102 or to the roadway system on the Port of Shelton property.
Element#3: Control Flow Rates
Since all stormwater generated through pipeline construction will be dealt with within the
construction trench,this element is not applicable.
Element#4: Install Sediment Controls
All required sediment controls will be installed as part of the pipeline trench construction and
will be moved as the trench construction progresses.
Element#5: Stabilize Soils
All exposed and unworked soils will be stabilized by application of effective BMP's appropriate
to the time of year required.
Element#6: Protect Slopes
This element is not applicable,as the pipeline construction is taking place in very flat areas of the
SR-102 right-of-way and Port of Shelton property.
Element#7: Protect Drain Inlets
Since there are no drain inlets to be made operable during construction,this element is not
applicable.
Element#8: Stabilize Channels and Outlets
All existing channels and outlets will be protected as required during pipeline construction.
Element#9: Control Pollutants
All pollutants that occur as part of the pipeline construction will be handled and disposed of in a
manner that does not contribute to the contamination of stormwater.
Element#10: Control Dewatering
Based on the site Geo-Technical Report,dewatering is not likely to be required to construct the
project. Should dewatering be needed,all necessary BMP's will be utilized.

All temporary and permanent erosion and sediment control BMP s shall be maintained and
repaired as needed to assure continued performance of their intended function.
Element#12: Manage the Project
The project will be managed and the construction scheduled so as to minimize potential for
erosion and the transport of sediment.
Minimum Requirement#3: Source Control of Pollution
Construction practices and BMP's will be implemented as required to minimize the opportunity for
stormwater to come in contact with pollutants.
Minimum Requirement#4: Preservation of Natural Drainage Systems and Outfalls
All natural drainage systems and outfalls will be preserved as part of the pipeline construction.
Minimum Requirement#5: On-site Stormwater Management
Stormwater management will be dealt with as part of the pipeline construction,but the quantity of
exposed or open soil will be minimal throughout construction.
Minimum Requirement#6: Runoff Treatment
Since there will be no stormwater runoff from the pipeline construction,this requirement is not
applicable.
Minimum Requirement#7: Flow Control
Since no stormwater will leave the pipeline construction site,this requirement is not applicable.
Minimum Requirement#8: Wetlands Protection
Since no wetlands will be impacted during the pipeline construction,this requirement is not applicable.

The requirements of the National Environmental Policy"Act(NEPA)compliance,pursuant to 40 j
CFR Part 6,Subpart E,for the City of Shelton's(Cit )Regional Wastewater component of the project '
have been completed. On October 5,2005,EPA issued a Finding of No Significant Impact(FONSI)for
the City's Regional Water and Wastewater Project. During the 30-day review and comment period for the
FONSI and Environmental Report,EPA received a letter from the Squaxin Island Tribe,dated October 27,
2005,objecting to EPA's FONSI decision,and raised several significant issues relating to the Regional
Water element of the project.
There have been numerous meetings and discussions since that time about the project amongst
EPA,USDA Rural Development,the Squaxin Island Tribe,and the Regional Partners(City of Shelton,
Washington State Department of Corrections,and the Washington State Patrol).An amendment to the
Environmental Report was completed on March 24,2006 highlighting the changes to the water and
wastewater plans that have occurred since the Environmental Report was originally prepared. A consensus
was reached in late March 2006 to separate the environmental review of the Regional Partner's water and
wastewater systems. While there remain concerns on the part of the Squaxin Island Tribe about certain
aspects of the water system,the Tribe supported moving forward with the wastewater system.
This letter confirms that EPA's FONSI,issued on October 5,2005,is valid for the wastewater
component of the project. Processing of grant funds may proceed for the Regional Wastewater Plan.
However,the Regional Water Plan will require additional environmental review.

The City of Shelton will be going out to bid soon on the Shelton Conveyance/Reclaimed Water Pipelines
that is a part of the Regional Water and Wastewater System. The City, in conducting the construction of
sewer and reclaimed water pipelines within the Mason County identified Critical Aquifer Recharge Area,
will provide required notification and education. This is a requirement defined in Mason County
Resources Ordinance 17.01.080(1)(3.)
The contractor(s)and employees,upon selection and before start of operation,will be notified that the
construction activities are being conducted in a Mason County designated Critical Aquifer Recharge
Area. The contractor(s)and employees will also receive training on an annual basis"regarding all
measures set forth by the BMP"defined within the specification and construction plans.
