The Department of Ecology(Ecology)reviewed the environmental checklist and has the following comment(s): WATER RESOURCES: Vicki Cline(360)407-0278 Any ground water withdrawals in excess of 5,000 gallons per day or for the irrigation of more than one-half acre of lawn or noncommercial garden, requires a water right permit from Ecology. All water wells shall be constructed in accordance with the provisions of Chapter 173-160 WAC by a driller licensed in the State of Washington. Well reports must be submitted to Ecology within 30 days after completion of a well. All water wells that may be drilled must be a minimum of 100 feet from any known, suspected,or potential source of contamination. Wells shall not be located within 1,000 feet of a solid waste landfill. WAC 173-160-171(1)The proposed water well shall be located where it is not subject to ponding and is not in the floodway, except as provided in Chapter 86.16 RCW. (2)It shall be protected from a one hundred year flood and from any surface or subsurface drainage capable of impairing the quality of the ground water supply. The Growth Management Act(RCW 19.27.097)requires an applicant to submit evidence of an adequate water supply before a building permit can be issued for any building requiring potable water. Clearing, grading and/or excavation on sites smaller than one acre that are part of a larger common plan of development or sale, if the common plan of development or sale will ultimately disturb one acre or more and discharge stormwater to surface waters of the State. a) This includes forest practices(including,but not limited to, class IV conversions) that are part of a construction activity that will result in the disturbance of one or more acres, and discharge to surface waters of the State; and 3. Any size construction activity discharging stormwater to waters of the State that Ecology: a) Determines to be a significant contributor of pollutants to waters of the State of Washington. b) Reasonably expects to cause a violation of any water quality standard. An environmental impact statement (EIS) must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. This fall the bridge and driveway are scheduled to be built, next year power, water, sewer and the house would be built. 8. Paved access off of SR302 through WSDOT, single lane steel bridge with pre-cast concrete footings, 12 ft wide gravel driveway, new well, tie-in to county sewer system, power drop from overhead power pole located adjacent to property, and a new 2000 to 2500 sf house. 12. ENVIRONMENTAL ELEMENTS 1. EARTH: a. General description of the site (circle one . Flat, r ling, hilly, steep slopes, mountainous, other(explain). Eastern portion of lot is about elev 2 and slopes down (approximately 500 ft) to top of shore bank at elevation 18 ft b. What is the steepest slope on the site (approximate percent slope)? Approximately 2.5 % c. What general types of soils are found on the site (for example, clay, sand, gravel, peat, muck)? If you know the classification of agricultural soils, specify them and note any prime farmland. Top 6" is generally a peat (decomposing leaves, branches, etc). BMPs will be used to control any potential runoff, and given the loose top soil, infiltration is expected to be high. g. Swales will run on both sides of driveway to promote infiltration. 2. AIR: a. This project is for construction of a new home with driveway, driveway will use poorly graded 3/4" gravel to minimize dust and maximize infiltration. b. Salmonids are salmon, trout and chars, e.g. bull trout. For our purposes, "ESA Listed Salmonids" is defined as fish species listed as endangered, threatened or being considered for listing. Other General Access Connection Information and Potential Additional Document Requirements: 1. The Permit Holder shall be responsible for obtaining all necessary Federal, State, and Local Permits including, but not limited to, Washington State Department of Ecology, Washington State Department of Fish and Wildlife, Washington State Department of Natural Resources, and U.S. Army Corps of Engineers permits or other approvals required prior to beginning any of the work. Copies of such permits shall be provided to the Department office issuing the permit, if requested. 2. To ensure proper compliance with all terms and conditions, a surety amount in the form of a Bond, Assignment of Escrow, Certificate of Deposit, Irrevocable Letter of Credit, Check or Money Order may be required by the Department. If a bond is provided, it shall be written by a surety company authorized to do business in the State of Washington. 3. Generally, the Department requires that no direct access to the state highway be provided if the property has access to another public road. RCW 47.50.010(3)(b); WAC 468-51-030(1)]. Per WAC 468-51-060, the Department encourages adjacent properties to obtain a joint use permit for an access connection to a common point. 4. No work is authorized on the state highway right of way until a permit is approved and fully executed by the Department. RCW 47.32.150. If the Permit Applicant is not the property owner, the property owner's signature is required before the permit application will be processed. 5. Permit holder is responsible for maintaining the access connection and related appurtenances between the state highway edge of shoulder and the state highway right-of-way line, including but not limited to, maintaining sight distance as only authorized by the Department, cleaning of culverts, etc. 6. If the parcel(s) served by this permit do not abut the state highway, it is the Permit Holder's responsibility to ensure that all necessary legal documentation, e.g. access easements, are obtained that provide the parcel(s) to be served by this permit the legal right to have a continuous right of access between this parcel(s) and the state highway.
