Smith & Associates, Inc. Minimum Requirements of the DOE 2005 Stormwater Manual The Stormwater checklist identifies the minimum requirements of the DOE 2005 Stormwater Manual. The Checklist is intended to identify the locations within the plan that addresses the minimum requirements. Mason County will not perform a technical evaluation of the submittal. Rather,the checklist provides a guide to allow Mason County to review the submittal and determine if the applicant has addressed the minimum features that make up a Stormwater plan. It is incumbent upon the applicant and his/her engineer to fulfill all the applicable requirements of the 2005 Stormwater Plan as it relates to the proposed project. Review by Mason County is intended to determine if the plan has addressed the minimum requirements. Applicant's engineer shall be responsible for the technical accuracy of the submitted Stormwater plan. During construction of the project,the stormwater plan engineer of record or his/her authorized representative shall inspect the site to ensure the stormwater plan is being implemented as designed. Upon completion of the project, the engineer or his authorized representative shall be required to certify that the stormwater plan has been implemented as designed. Failure to meet the minimum requirements could result in delay or rejection of the application until the deficiencies are corrected. Section I—Construction SWPPP Narrative 1. ENVIRONMENTAL CHECKLIST Purpose of checklist: The State Environmental Policy Act(SEPA),chapter 43.21C RCW,requires all governmental agencies to consider the environmental impacts of a proposal before making decisions. An environmental impact statement(EIS)must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from your proposal(and to reduce or avoid impacts from the proposal,if it can be done)and to help the agency decide whether an EIS is required. Instructions for applicants: This environmental checklist asks you to describe some basic information about your proposal. Governmental agencies use this checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. The proposed project is identified in the City of Shelton 2010 Water System Plan and would aid in offsetting current pressure and storage deficiencies within the existing system and also provide needed fire flow and water pressure to the Washington State Patrol Academy. The project would occur in two phases as follows: Phase IA, (See Figure 1 and 2). Phase IA work includes construction of one or two elevated water storage tanks, approximately 2,000 linear feet of 16-inch diameter water main,and a connection to the Mason County Property to the north. The City intends to construct at least one elevated tank as part of this project. An additive alternative bid item will be included to construct the second tank if funding and budget allows. The plan is to construct at least one of the tanks and have it available(operational)by July 2015 to accommodate the Washington State Patrol Academy(a water line was run from the City to Washington State Patrol in 2012 but does not have enough pressure adequately to serve the site. The proposed water tanks will provide needed pressure). To connect the tanks to the existing water distribution system approximately 2,000 linear feet of new 16-inch diameter water main would be installed from West Dayton Airport Road North along West Public Works Drive to the tank site. The elevated tanks are proposed to be multi-column elevated steel reservoir tanks with a capacity of 400,000 gallons each(total of 800,000 gallons). The tanks would each stand approximately 140'tall above existing grade at the site. 2 Phase 1B(See Figures 1 and 3). Phase 1B improvements include the retrofit of the City's existing Well Number 3 with a new pump, reconfiguration of well yard piping, construction of a new pressure-reducing valve station (PRV), and construction of approximately 3,500 linear feet of new 12-inch diameter water main from the well site to a connection to the existing water system located near the intersection of Shelton Springs Road and Tarragon Avenue. Well number 3 currently pumps to the existing Mountain View Pressure Zone. As part of this project,it will be connected directly to the Upper Mountain View Pressure Zone. The well pump at Well 3 must be replaced to accommodate new head conditions. No change in pumping capacity is proposed and no new sources of water are being developed or added as part of this project. Phase 2 (see figures 4 and 5)improvements include wellhead treatments at(existing)well number 1,a one million gallon ground level water storage tank.a new 1500 gallon per minute booster pump station adiacent to the new storage tank, new water transmission mains to connect to the existing water system,and a pressure reducing valve station to service the Sprinefield loop neighborhood. 12. Location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project,including a street address,if any,and section,township,and range,if known. If a proposal would occur over a range of area,provide the range or boundaries of the site(s). Provide a legal description,site plan,vicinity map,and topographic map,if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit applications related to this checklist. The Phase 1A project site is located immediately south of the existing Mason County Public Works Facility located at 100 Public Works Drive just north of Shelton City Limits. The site is located north of Sanderson Field Airport, west of Highway 101,North of Dayton Airport Road. Assessor's Parcel Number 42002-10-00020.See attached site location map and site plan. The Phase 1B project site is located immediately north of the intersection of Shelton Springs Road and Wallace Kneeland Boulevard in and around the existing City wellhouse complex. The site is located North of Wallace Kneeland Boulevard, East of Shelton Springs Road, and West of Brockdale Road on the City of Shelton owned "watershed" property. Assessor's Parcel Number 32007-30-60000. The Phase 2 proiect site is located on the west side of North 13&Street,south of Shelton Springs Road,north of K Street, and east of Olympic Middle School on City of Shelton owned"watershed"property,Oust north of existing Wellhouse 1 (located at the intersection of north 13&/NorthcHfVK Street). Assessor's Parcel Number 32018-25-60010. B. EwiRoNMENTAL ELEMENTS 1. Earth a. General description of the site(circle one): Flat,rolling,hilly,steep slopes,mountainous, other...... Both sites are,generally,flat. b. What is the steepest slope on the site(approximate percent slope)?5%. c. What general types of soils are found on the site(for example,clay,sand,gravel,peat, muck)? If you know the classification of agricultural soils,specify them and note any prime farmland. The soils at each site are generally gravelly with good drainage and minimal organic toplayer. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. None noted. e. Describe the purpose,type,and approximate quantities of any filling or grading proposed Indicate source of fill. No fill is proposed Elevated tank foundations will be dug below ground surface in the range of 8 feet. All proposed water main piping will be conventional depths below grade and covered 3 approporiately with native cover. The access roadway to the tank site will be minimally graded to allow for drainage. £ Could erosion occur as a result of clearing,construction,or use? If so,generally describe. Doubtful. The construction areas are generally flat with gravelly,well draining,soils. That said,best management practices for control of erosion will he practiced throughout all phases of construction. Denuded areas will be replanted with appropriate grasses following construction. g.About what percent of the site will be covered with impervious surfaces after project construction(for example,asphalt or buildings)? .15 acres(6,534 square feet). This amount includes the water tower and access road for the tower. Other areas would be reseeded after construction and result in no increase in impervious coverage. Phase II improvements have not been designed though impervious surfaces will be the minimum amount necessary for placement of the project and all stormwater is required to be handled pursuant to the Department of Ecology Stormwater Mangement Manual for Western Washington. h. Proposed measures to reduce or control erosion,or other impacts to the earth,if any: Best management practices will be utilized during all phases of construction(silt fences and straw waddles as necessary)and all denuded areas will be revegetated following construction. 2. Air a. What types of emissions to the air would result from the proposal(i.e.,dust,automobile, odors,industrial wood smoke)during construction and when the project is completed? If any,generally describe and give approximate quantities if known. Emissions would result from equipment utilized for construction as well as workers vehicles traveling to and from the site. Periodic emissions would emanate from maintenance workers accessing the site to maintain the tanks and/or facility over time. b. Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. None. c. Proposed measures to reduce or control emissions or other impacts to air,if any: None proposed. 3. Water a. Surface: 1) Is there any surface water body on or in the immediate vicinity of the site(including year-round and seasonal streams,saltwater,lakes,ponds,wetlands)? If yes,describe type and provide names. If appropriate,state what stream or river it flows into. A portion of Phase 1B work includes work in an existing maintained access road which crosses upper Shelton Creek. Shelton Creek is as seasonal creek in this location. 2)Will the project require any work over,in,or adjacent to(within 200 feet)the described waters? If yes,please describe and attach available plans. Yes. See attached plans. Work would include installation of a new 12"Watermain to be installed from existing well house 3,due west,to Shelton Springs Road. The water main would be installed along and existing maintained access road for the water system. Work would be performed during the"dry"season and no water is expected to be present on the surface during construction. 3)Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. None. 4 4)Will the proposal require surface water withdrawals or diversions? Give general description,purpose,and approximate quantities if known. Will this water flow into other waters? If so,describe. Nearly all work associated with the proposal would occur on existing road rights of way or within existing maintained access driveways. The only exception is the construction work directly associated installation of the water towers(Phase 1A). That work includes installation of a gravel access road as well as impervious coverage associated with the footings for each one of the tower legs. This water will be directed to an on-site detention/treatment facility. Phase II also includes installation of a eravel access road in addition to the ground level water tank. 2) Could waste materials enter ground or surface waters? If so,generally describe. No. Runoff from these areas will be clean. d. The Phase lI proiect site has been surveyed(drive by)for presents of Mazama Pocket Gopher. None have been noted at this time. Mitigation proposed for the facility include a Mazama Pocket Gopher survey be performed prior to the initiation of construction activities for the review and approval of the Washington State Department of Fish and Wildlife. c. Is the site part of a migration route? If so, explain. The site is located within the Western Flyway used by migratory birds(a majority of the west coast is). d. Proposed measures to preserve or enhance wildlife,if any: A Habitat Management Plan for the Mazama Pocket Gopher has been developed. A mitigation ratio of 3:1 was utilized and the project proposes that.45 acres of the Phase 1A site would be enhanced through planting of native prairie grasses. The areas would be mowed a minimum of twice per year to discourage the growth of woody vegetation. Compliance with requirements of a survey for impacts to Mazama Pocket Gooher on the Phase H site. 6. There would be minimal lighting associated with the facility for security purposes(all lighting will be downcast and shielded)and the top of the tank would be lit pursuant to FAA Advisory Circular 70/7460-1 K 8 Change 2,Obstruction Marking and Lighting,a med-dual system—Chapters 4,8(M-Dual),& 12(per FAA letter dated 3/312014,attached). b. The existing Mason County Public Works facility is north of the project 1A site. d. The Ridge MotorSports park lies to the east and Sanderson Field(a General Aviation Airport)lies to the south of the Phase I site. b. The assessment concludes that"Pedestrian survey and subsurface testing did not identify any evidence of archaeological sites within the (construction areas). Under this work task, the project manager and design team leaders will coordinate all design development effort. This task includes all project communication and coordination with the City staff, subconsultants, and other project stakeholders as necessary to execute the work and maintain schedule. Included as the first order of work under this subtask is the completion of a detailed project schedule showing the duration and milestones associated with all work tasks. This schedule will reflect a substantial completion date for Schedule A elements as January 1, 2015. Items performed as part of this subtask include: • Monitor progress against schedule, scope of service, and budget, including earned value assessments of design documents. Administer monthly invoicing to City. • Provide regular communications on project status to the City, including progress meetings as necessary. Anticipate schedule adjustments needed to achieve City provision of service deadlines and budgetary limitations. • Subconsultant coordination. • Project documentation. Deliverables: 1. Monthly invoicing and progress reports. SubtaskA.1.2 Project Meetings This subtask includes all project meetings for Schedule A elements to include: • Project Startup Meeting: Immediately following contract execution, a startup meeting will be held to introduce project team leaders to City staff, review scope, budget and schedule, discuss the preferred method of communication protocols and establish priorities. • Site and Facility Investigation Review Meeting. Conduct up to two (2) Site and Facility investigation meetings to aid in detailed design activities. • Final Design Milestone Review Meetings: At the 30, 60 and 90 percent design completion levels, meetings will be held to review contract document package deliverables (plans, specifications, and cost estimates—PS&E) and incorporate City comments. Comments will be documented through the course of design with an electronic review comment log indicating design team responses and changes implemented. • Pre Bid and Bid Opening Meetings: Following advertisement of the general construction contract and schedules of work, the MSA team will attend both a pre-bid meeting and site walk-through for prospective contractors, as well as attend the public bid opening. • Project Status and Design Coordination Calls: A weekly half hour time slot will be established to conduct brief project status and coordination calls. Deliverables: 1. Meeting agenda and minutes. 2. Design milestone review comment log. Task A.2—Schedule A Preliminary Design Concurrent to the start of design package development, a series of site and facility investigations will be performed. Work in this task includes performing design surveys, defining project environmental permitting needs, and completing geotechnical subsurface investigations. As appropriate, documents and findings developed under this task will be provided as appendices within bid packages. Subtask A.Z] Site Surveys. Work under this subtask includes the completion of property, topographical and utility location surveys for Schedule A elements. Work will include preliminary utility and records review and completion of field surveys with adequate coverage to allow for element designs to be completed. Electronic copies of field data will be downloaded and used to establish design base mapping for all Schedule A elements. Assumptions made as part of this Task include: • Base mapping will reflect review of all available utility documentation and facilities marked in the field. No subsurface detection technologies are included to locate unmarked utilities. Deliverables: 1. Base mapping work products will be incorporated into and delivered as part of Task A.3 final designs. Subtask A.2.2 Geotechnical Investigations for Schedule A Elements Work under this task includes the completion of geotechnical investigations necessary for the design and construction of the project elements. Included in this subtask is a site reconnaissance which will be followed by a subsurface exploration program consisting of a test pits and borings. DRAFT Scope of Work MURRAY,SMITH&ASSOCIATES,INC. Subtask elements include the following: 1. Review available geologic maps for the element sites. 2. Explore the subsurface soil and groundwater conditions in the area of the planned elevated reservoir and adjacent structures. 3. Perform one day of test pit explorations and 2 borings. It is assumed that all borings will be performed for element A, the elevated reservoir. 4. Evaluate pertinent physical and engineering characteristics of the soils resulting from the boring and test pit explorations. 5. Prepare a geotechnical report documenting the results of subsurface explorations, and conclusions and recommendations for geotechnical design elements of the project. The report will include: • Description of the geologic materials encountered. • Description of depth to groundwater, if encountered. • Discussion of seismicity at the site along with seismic design parameters including Site Class and site coefficients based on current International Building Code (IBC) criteria. • Subsurface investigations necessary for foundation support recommendations. Assumptions made as part of this Task include: • The elevated reservoir is expected to have higher overturning moments and the need for foundations to resist uplift loads will have to be provided once a preliminary design is obtained. For budgeting purposes 100 feet of drilling is assumed with two 50 foot borings. • Subsurface exploration for elements 2, 3 and 4 will include up to 8 test pits. • City equipment and staff will be used to dig all test pits. Deliverables: 1. Draft and final geotechnical report, including boring logs. Subtask A.2.3 Schedule A Site Telemetry/SCADA Integration Study. The identified property location for the elevated reservoir to serve the new Upper Mountain View pressure zone is geographically isolated from other existing water utility facility locations currently linked by radio communications. Communications with this new facility,preferably using the City's current radio frequency,must be established for system operations and remote monitoring of the reservoir. Draft and final technical memorandum Subtask A.2.4 Environmental Review/Permitting Needs. Work under this task includes the preparation of a SEPA environmental checklist for each project element in accordance with WAC 197-11. An inventory of various permitting needs anticipated for the project elements will be prepared. Work will include a site visit to review the sites for the presence of wetlands and streams. Work elements and assumptions are listed by schedule: Schedule A Element 1: Upper Mountain View Elevated Reservoir. Scoping assumptions for this schedule are as follows: • Based on aerial photo review, we do not anticipate impacting any wetlands, streams, or buffers. Should critical areas be unavoidable, additional scope and budget would be required. • Permitting requirements are anticipated to be limited to City of Shelton fill and grade permit, Mason County Utility permit for location of utilities in public right- of-way, and Mason County grading permit. Schedule A Elements 2, 3 and 4: Scoping assumptions for this schedule are as follows: • Based upon aerial photo review,we do not anticipate impacting any wetlands, streams, or buffers. Transmission main and distribution piping is anticipated to be constructed in existing developed road right of way. Should critical areas or buffers be unavoidable, additional scope and budget would be required to prepare appropriate documentation. • Permitting requirements are anticipated to be limited to City of Shelton fill and grade permit, Mason County Utility permit for location of utilities in public right of way, and Mason County grading permit. It is also anticipated that a Mason County conditional use permit will be required and that the City will secure and pay for this permit. The MSA project team will provide supporting technical documentation for identified permits. Any additional permitting requirements will be identified as part of this task, with appropriate responsibilities being further established with City staff. Deliverables: 1. Electronic draft SEPA environmental checklist for review and comment by City/Team. 2. Electronic version of final SEPA checklist for publication and distribution by the City. 3. List of required project permits for Schedule A elements. Task A.3—Final Design Following completion and incorporation of the work performed under Task 2.2, final plans, specifications and estimates (PS&E) will be completed. Final plans will incorporate survey base mapping, design and schedule modifications recommended under Task 2.2, and the requirements of various anticipated permits and City plan reviews. Plans will be presented for the 30 percent, 60 percent, 90 percent, and final design completion level for City review, comment and approval. Final plans will incorporate various comments as may be appropriate. As previously described two separate design, bidding and award and construction packages will be developed for the Schedule A improvement elements as follows: Schedule A Bid Package 1: Element 1 —Upper Mountain View Elevated Tank Schedule A Bid Package 2: Element 2, 3 and 4—Well No. 3 Upgrades, Well No. 4 Connection Piping Improvements Pressure Reducing Valve Improvements MSA will coordinate the various work activities of the project team, monitor the work of individual design elements and guide activities relative to overall project design requirements. The various specific work efforts will be managed relative to the agreed design assumptions and scope and fee structures to ensure proper and continuous integration between various designelements. Specialty subconsultants will develop the architectural P tY P structural, electrical and instrumentation and control documents. The construction contract documents will be prepared based on the following assumptions: A. Technical Specifications—Technical specifications, complementing project plans will be prepared for all schedules of the work. Specifications will be prepared in accordance with Construction Standards Institute (CSI) formats and will include references to appropriate industry standards. Measurement and payment provisions will be coordinated with the construction proposal schedule of prices. B. Bidding and Construction Contract Documents—Bidding and construction contract documents will be prepared based on Engineers Joint Construction Documents Committee (EJCDC) standard formats, including custom schedule of prices for the project, General Conditions and Supplementary General Conditions which recognize any unique City contracting interests relative to this work. Draft bidding and construction contract documents will be provided to the City as part of the 60 percent design completion submittal for review by the City and the City's legal counsel. C. Opinions of Probable Construction Cost—Opinions of probable costs will be prepared and submitted at all design milestone review periods. The estimates will be subject to levels of accuracy and contingency percentages recommended by the former Association for the Advancement of Cost Engineering (AACE) for different design development levels. Technical specifications and construction bidding and contract documents will be prepared for bidding under public contracting procedures. Fee estimates assume that one bidding package will be developed. Draft documents will be reviewed by the City staff prior to final revisions being made. Final documents will be provided to the Builders Exchange of Washington for printing and distribution. Subtasks include: Subtask A.33—30 Percent Design Completion Plans,Specifications and Estimate (PSE). This subtask includes the preparation of 30 percent design completion plans and specifications based on the preliminary design work and analysis completed in Task 2.2 and the City's review and acceptance of recommended design concepts. Preliminary plans completed to the 30 percent design completion level will be prepared for all schedules. Construction cost estimates will be developed for proposed improvements based on the 30 percent completed designs and a judgment and analysis of current construction market conditions. Work will include the preparation of specification table of contents and draft text for relevant technical specification sections, as well as a limited plan set that includes symbols and legends, general notes, schematics and plan views. Subtask A.3.2—60 Percent Design Completion Plans,Specifications and Estimate (PSE). Under this subtask the preliminary designs completed and reviewed by the City as part of Subtask 2.3.1 will be further developed to the 60 percent design completion level. Plans and specifications will be prepared for all key project components and include civil, structural, geotechnical, mechanical, electrical and SCADA features. As part of this task a 60 percent design completion level set of full pipeline alignment plans, plans and elevations for all storage,pumping and treatment facilities, and full draft specifications will be provided to the City for review and comment. This submittal will also include a draft set of 60 percent design completion level contract documents and an updated project schedule and construction cost estimate. Under this subtask the 60 percent designs completed and reviewed by the City will be further developed to the 90 percent design completion level, including the addition of all draft details. As part of this subtask a 90 percent design completion level set of plans, specifications and contract documents will be provided to the City for review and comment. This submittal will include an updated project schedule and construction cost estimate. Following the 90 percent complete design review meeting,this design package will be submitted to DOH for approval to proceed with construction. Subtask A.3.4—Final Completion Plans,Specifications and Estimate(PSE). Under this subtask the 90 percent designs completed and reviewed by the City will be finalized for bidding and three (3) complete final sets of plans, specifications and contract documents will be provided to City staff, with an additional ten(10) sets for distribution to prospective bidders. Assumptions made as part of this subtask include: • Level of effort and fee estimates for the development of construction contract PS&E documents are based on a preliminary drawing set sheet list and anticipated technical specifications. • Plans will be developed in AutoCAD 2012, but saved and delivered to the City in another acceptable AutoCAD format. Deliverables: 1. Three (3) hardcopies and one electronic copy of 30 percent, 60 percent, and 90 percentcompletion design level draft PS&E document submittals. 2. Three (3)hardcopies and one electronic copy of 100 percent stamped PS&E for City staff record. Plans are assumed to be delivered at 11"x 17" half-scale, but can be produced at full scale upon request. Electronic deliverables will be in PDF format, as well as in original Word, Excel, or AutoCAD file format. 3. Ten(10)hardcopies of 100 percent PS&E for City retention and distribution to prospective bidders. Plan set copies will be produced at full scale. Task A.4—Permits and Approvals �I Under this task, coordination with regulatory and various environmental and local permitting agencies for the preparation and submittal of required documents will be performed, based on the stated assumptions for each schedule in subtask A.2.4. It is understood all permitting fees will be paid directly by the City, and that construction plans will be used extensively for permitting applications figures. Review comments will be addressed and incorporated, as necessary, to obtain approvals. Subtask A.4.1 Update BODR. Provide the final facility configuration and design criteria, including the 90% design drawings. Submit the updated Draft BODR to the City in electronic form for review and comment. Incoporate comments into the Final BODR. Prepare 5 hard copies and 1 electronic copy of the Final BODR for submission to the City. Assumptions • City will submit the updated BODR to the Department of Health(DOH). • Completed 90 percent design plans and the completion level opinion of probable construction cost for the project will be completed under Task 1.3. • The City will review the Draft BODR and comments will be incorporated into the Final BODR. • No additional alternatives analysis or hydraulic modeling will be required. Deliverables: • A draft and final copy of the BODR for the selected alternative will be submitted in electronic form for review. • Following the City's review,prepare 5 hard copies and 1 electronic copy for submission to the City. The BODR will include 90 percent design completion level design drawings, estimates of construction cost and anticipated project schedule. Subtask A.4.2 Soormwater Pollution Prevention Plan (SWPPP). This subtask includes the preparation of a SWPPP for all project sites in compliance with Washington State Department of Ecology(DOE), City and Mason County requirements. Work includes preparation of the plan forms, narrative, illustrations and associated figures using the 90 percent complete design documents. The document will be prepared in compliance with DOE and County S WPPP guidelines and will be prepared as a separate document. Construction site stormwater protection/monitoring requirements will be identified and described as a compliance requirement of the construction contractor. Deliverables: 1. Draft and final S WPPP. Subtask A.4.3 Local Agency Permuting. Based on aerial image review and assumptions stated in subtask A.2.4, no critical areas are anticipated to be impacted and no additional environmental permitting beyond SEPA checklist preparation is scoped. Work under this task includes coordinating the applications of the local agency permitting associated with construction of the Schedule A facilities within public rights-of-way, as well as permits associated with above grade buildings and structures that are subject to City building department codes. Assumptions made as part of this subtask include: • All permit applications will be approved and will be non-contested. • Local construction and building permits will be obtained by the construction contractor Deliverables: 1. Mason County Utility Permit application 2. Construction permitting support documentation provided as part of final designs Task A.5—Bidding and Award Under this task, assistance will be provided to the City for the two Schedule A bidding packages. It is anticipated that Element 1,the elevated reservoir will be completed first and will be advertised first. Schedule A elements 2, 3 and 4 will be combined into the second bid package. The contract documents will be made available to the Builders Exchange of Washington for online posting and for advertisement targeted at the area's construction contractors. It is understood that the City will lead this task with MSA providing technical support through the subtasks outline below. Subtask A.S.1 Advertisement. Prepare advertisements for public bidding and arrange for publication in the Seattle Daily Journal of Commerce (DJC)and the Builders Exchange of Washington website. It is assumed that the Builders Exchange of Washington will make the documents available for distribution to prospective bidders, subcontractors, equipment suppliers and other vendors upon request. Advertisements and posting costs are assumed to be paid by the City. It is also assumed that Builders Exchange of Washington will maintain a list of planholders for distribution upon request. Deliverables.- 1. Electronic and hardcopy of advertisements and final contract documents to Builders Exchange of Washington for the two Schedule A bidding packages. Subtask A.5.2 Bidders Inquiries and Addenda. Work under this subtask includes documentation of and responding to questions from bidders, subcontractors, equipment suppliers and other vendors regarding the project and the plans and specifications. Included in this subtask is reviewing and acting upon any submittals received as allowed by any pre- bid submittal process. It is assumed that there will be a pre-bid submittal and approval process for the following items: steel reservoir supplier/constructor and painting contractor for the new reservoir. Also included in this task is preparing for and issuing addenda as DRAFT Scope of Work MURRAY,SMITH&ASSOCIATES,INC. Subtask A.5.3 Bid Tabulation and Award Recommendation. Under this subtask assistance will be provided to the City for evaluation of the bids and preparation of a written bid tabulation summary document. Following a review of qualifications and references, letters of recommendation for award of the construction contracts will be provided. For budgeting purposes it is assumed that the bid opening and award process will be non-contested. Assumptions made as part of this subtask include: • Construction contract selection and award will be non-contested. Deliverables: 1. Bid tabulation and letter of recommendation for construction contract award. Task A.6—Construction Support Services It is understood that the City will lead all construction management activities and that MSA will provide engineering and technical support throughout project construction activities. Engineering fee estimates assume a total construction contract period of 12 months with project substantial completion by January 1, 2015. It is anticipated that the Schedule A improvements will be constructed as two separate projects. Work in this task is provided as in office engineering support and field engineering support as follows: Task A.6.1 —In Office Engineering Support Under this task, MSA will provide in office engineering support to the City. Subtask elements, including any obligated construction correspondence materials, are itemized as follows: A.6.1.1 Preconstruction Conference—Participate of two engineering team members in a preconstruction conference, for which agenda and minutes will be prepared. A.6.1.2 Clarifications—Provide clarification of the contract documents to the construction contractor based on the contractor's written requests for clarifications, verbal requests or as the need otherwise arises. Prepare written responses and drawings or sketches as necessary for any clarifications of the contract documents. DRAFT Scope of Work MURRAY,SMITH&ASSOCIATES,INC. A.6.1.3 Process Contract Modifications—Provide services related to contract modifications. These include preparation of contract modification proposal descriptions and justification documentation, assistance with construction contractor negotiations, making recommendations to the City regarding any contract modifications, and processing the formal contract amendment documents between the City and the construction contractor. For budgeting purposes it is assumed that up to 10 total contract modifications will be provided. II A.6.1.4 Project Manager/Assistant Project Manager Site Visits—Conduct periodic li site visits by MSA's project manager or assistant project manager when significant construction is occurring. The purpose of these visits is to answer questions regarding the contract documents,review the progress of the work, assist with resolving project difficulties and review the construction work to confirm that it is proceeding in accordance with the requirements of the contract documents. For budgeting purposes it is assumed that up to 10 visits will be provided for during the active construction period. A.6.1.S Miscellaneous Project Related Correspondence—Prepare various miscellaneous correspondence communiqu6s related to the project. Such correspondence includes but is not limited to letters, faxes, a-mails and memos to the City, permitting agencies, and the contractor. A budgetary provision of 80 hours is allocated. A.6.1.6 Schedule Review—Review the project schedule, the schedule of shop drawingsubmittals and schedule of values prepared and updated b contractor on a P P P Y monthly basis. A.6.1.7 Shop g Drawin and Submittal Reviews: Under this subtask receive review and process shop drawings and other technical submittals. Consider and evaluate any alternatives or substitutions proposed by the contractor. Receive and review other submittals of the contractor including construction schedules, shop drawings/submittal schedules, lump sum price breakdowns, and other submittals required b the contract documents. 1 d file will be developed q y o ents. A.6.1.10 Record Drawings: Record drawings of the project will be prepared, based on the construction records of the construction contractor and the Engineer's on-site engineering/observation representative. Record drawings will be provided to the City in AutoCAD electronic file format. A.6.1.11 DOH Construction Completion Report: Following final acceptance of the construction contract, a construction completion report will be completed and submitted for record. Signing the report as the Engineer of Record will be contingent on adequate field observation and daily report documentation being filed by City staff and offered to MSA for review. Task A.6.2—Field Engineering Support Under this task, MSA will provide limited field engineering support to the City. Subtask elements, including any obligated construction correspondence materials, are itemized as follows: A.6.2.1 Site Meetings and Project Milestone Meetings—Attend meetings with the contractor, such as progress meetings,job conferences and other project related meetings, and prepare and circulate copies of meeting summaries. Site meetings will occur over the course of project construction activities. Included in this subtask element are the following formal construction phase milestone meetings: • Bi monthly construction meetings • Facility Start-up • Construction Completion A.6.2.2 Facility Start-Up and Project Close-Out: Under this task, assistance will be provided in the start-up, testing and City operator training schedule periods for the new reservoir and pump station facilities. MSA will provide overall engineering coordination with the general contractor, City operations staff, the City's SCADA system integrator,the City's project representative and the construction contractor's technical representatives for specialty equipment, all as required to attain proper long-term operation of constructed facilities in full accordance with design plans. With the City's concurrence and successful project completion, conduct various activities to effectively close-out the project. The City's SCADA system integrator will be responsible for all start-up activities related to that system. A.6.2.3 Anniversary/Warranty Inspection: Under this task, assistance will be provided for an anniversary/warranty inspection at the end of the construction contractor's warranty period.
