CONDITIONS FOR
GRD2015-00015
1) Approved per sjtp plan and topographic cross-section portrayed by ESA for the Division 14 plans and profiles.

2) All construction and demolition debris must be removed from the site area after project completion. Proper disposal of construction debris must be on
land in such a manner that debris cannot enter or cause water quality degradation of State waters. X
3) Temporary erosion control measures must be implemented to prevent water quality degradation of adjacent waters or properties. Silt fencing, straw, or
surface matting mupe installed and maintained until upland vegetation has become established.

4) The approval of this project is subject to the recommendations and specifications outlined in the attached geotechnical report or assessment. All
applicable recommendations and specifications shall be applied to the development on this site. Any deviation requires stamped written approval from
the registered design professional responsible for the report/assessment, and may require special inspection by same. Structures and/or land
modifications (grading, cuts, fills, etc.) required in the geotechnical report/assessment, may require a separate permit. The geotechnical report/
assessment hall remain attached to the approved building plans.

11) Compliance with an approved Storm Water Plan shall be subject to an on site inspection, or inspections, by the engineer of record or an authorized
representative. Said inspection(s) shall be performed in accordance with the approved plan, and prior to any modification that would make a
determination of compliance impossible.
Inspection reports shall be submitted to verify all engineering requirements have been completed in accordance with the approved storm water plan.
Reports shall be submitted to the Mason County Department of Community Development prior to each corresponding inspection and final permit
approva Copies of all special inspection reports shall be made available at time of inspection.
12) Retaining walls needed to support a surcharge such as structures, roads, or to support slopes, shall require a separate building permit and approval prior
to construction of the retaining wall. X to
13) A pre-construction conference shall be held prior to begining any work on site. The project engineer, contractor(s), utility representatives, and Mason
County will be in attendance. Please contact Terry Conley, Mason County Utility Specialist, to schedule the pre-construction conference. Terry will

Prior to working within existing county right-of-way(E Soderberg Rd), the contractor shall obtain a Road use and/or Utility Permit from Mason County.

The project engineer, or engineer's representative, shall maintain adequate records during construction to prepare as-built documents. Prior to fianl plat
approval, certified road, wastewater and stormwater as-built documents shall be submitted in electronic and paper formats.

Mason County Public Works will inspect certain phases of the project and/or require testing results prior to the contractor moving to the next phase of
work (i.e. covering utility trenches, paving, etc.) Inspections are to be coordinated through Terry Conley.

14) Prior to initialing grading, temporary erosion and sediment control measures are to be in place as specified on plan sheet 13/13 dated 5-1-15 prepared
by Engineering Services Associates. It is the owner and contractor's responsibility to ensure that the measures are maintained, and adequate for the site.
Additional temporary measures may be necessary should conditions warrents it.

Prior to Final plat approval, Road A, Road A Court, and Road B Court shall be constructed or bonded per sheet 6/13 dated 5-1-15 prepared by

It is anticipated that a portion of Road B (11+00 to intersection of Road A)will also be constructed under this permit to install a portion of the gravity
sewer collection system, force main and pump station#4. Please coordinate with Fred Perryman to determine the length of Road B to be constructed
and what surfacing will be required for the phase of work to maintain the system prior to Division 15 construction.

16) Prior to final plat approval, the stormwater components shall be installed per plan sheets 3,4, and 8/13 dated 6-5-06, 6-5-15 and 4-18-15 prepared by
Engineering Services Associates.

17) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will
not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building
Department prio ,to any further inspections being performed or approvals granted.

18) All changes to"approved" building plans that effect compliance with the international codes as amended and adopted, or any other Mason County
ordinance or
gulation, must be reviewed and approved by Mason County prior to construction.
19) The slope of cut surfaces shall be no steeper than is safe for the intended use and shall be no steeper than two horizontal to one vertical unless the
permittee furnishes a soils engineering or an engineering geology report, or both, stating that the site has been investigated and giving an opinion that a
cut at a steeper slope will be stable and not create a hazard to public or private property.

20) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure
to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with
Mason Count ordinances and building regulations.

21) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for
action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit
holder hav�,prevented action from being taken. No more than one extension may be granted.

23) The top of cut slopes shall not be made nearer to a site boundary line than one-fifth of the vertical height of cut with a minimum of two feet and a
maximum of ten feet. The setback may need to be increased for any required interceptor drains. The top of the cut slope shall be finished as a rounded
or bald head slope.

24) Setback dimensions shall be horizontal distances measured perpendicular to the site boundary.
Toe of Fill Slope. The toe of fill slope shall be made not nearer to the site boundary line than one-half the height of the slope with a minimum of two feet
and a maximum of twenty feet. Where a fill slope is to be located near the site boundary and the adjacent off-site property is developed, special
precautions shall be incorporated in the work as the official deems necessary to protect the adjoining property from damage as a result of such grading.
These precautions may include but are not limited to:
Additional setbacks,-
Provision for retaining or slough walls;
Mechanical or chemical treatment of the fill slope surface to minimize erosion;
Provisions for the control of surface waters.

25) A 20 foot wide fire apparatus access road is required to extend within 150 feet of any portion of the exterior walls of the building. An unobstructed vertical
clearance of 13 feet, 6 inches is required. Dead-end fire apparatus access roads longer than 150 feet are required to have an approved turnaround. The
maximum grade of a fire apparatus access road is 14%. X (::�t _
26) Hydrants are to be in place and serviceable prior to final plat approval. The following code reference for requirements and placement of hydrants.
MCC14.18 fire flow is required at 1,000 GPM minimum at 30-PSI residential pressure and fire hydrants are capable of delivering 1,000 GPM for 2 hours
are required. Maximum spacing between hydrants shall be 500'.
