1) Approved perl dimensions and setbacks on submitted site plan. Setbacks are measured from the furthest projection of the structure.

2) The proposal shall be In full compliance with the Mason County Cell Tower Ordinance#5-98 (Revised by Ord. No. 53-04).

The design of the equipment buildings and related structures shall, to the extent possible, use materials, colors, textures, screening and
landscaping that will blend the tower facilities to the natural setting and built environment.X W
3) All construction and demolition debris must be removed from the shore area after project completion. Proper disposal of construction debris must
be on land in such a manner that debris cannot enter or cause water quality degradation of State waters.
4) Temporary erosion control measures must be implemented to prevent water quality degradation of adjacent waters or properties. Silt fencing or
straw must be installed and maintained until upland vegetation has become established. X rNlrS
5) All other necessary permits from Mason County,Washington State and/or Federal Agencies that are required for this proposed development and

6) The following special conditions should be made a part of the building permit:
1. Electrical Permit Required. The electrical system shall be installed under a separate electrical permit issued by the Washington State
Department of Labor& Industries (L&i). Electrical"final"inspections must be approved by L&I prior to requesting a building final.

2. Special Inspection: IBC Chapter 17, Section 1704. In addition to the inspections required by IBC Section 109, the owner or the engineer or
architect of record acting as the owner's agent shall employ one or more special inspectors who shall provide inspections during construction on
the types of work listed under IBC Section 1705. The special inspectors'duties& responsibilities shall be as specified In 1704. All special inspection
reports shall be provided to the Mason County Building Department in accordance with IBC 1704.2,4. Prior to final inspection, the special
inspection agency shall also provide a final, signed report to the building department stating that all items requiring special inspection and testing
were fulfilled and reported, corrections of any discrepancies has been completed, and, to the best of their knowledge, are in conformance with the
approvgd design drawings, specifications, and applicable provisions of the code.

3. Special Inspection Contractor Responsibilities. Third-Party Special Inspection is required in accordance with IBC Sections 1704 & 1705 for the
items identified in the Statement of Special Inspections (as required by IBC Section 1704.3). In accordance with IBC Section 1704.4, each
contractor responsible for the construction of the work listed in the statement of special inspections shall submit a written statement of responsibility
to the building official and the owner prior to the commencement of work on the system or component. The contractor's statement of responsibility
shall cont on in acknowledgment of awareness of the special requirements contained in the statement of special inspections.

4. All Construction: All construction shall meet or exceed all local ordinances and the requirements of the 2012 International Codes as adopted by
Mason County and Washington State. Occupancy is limited to the approved and permitted classification.Any non-approved change of use or
occupanc could result in permit revocation and/or Code Enforcement action.
5. Field Correct: The construction of the permitted project is subject to inspections by Mason County Building Department.All construction must be
in conformance with the 2012 International Codes as adopted by Mason County and Washington State.Any corrections, changes or alterations

7. Permit Validity: IBC 105.4/IRC R105.4:The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any
violation of any of the provisions of the building code,or any other code or ordinance of Mason County. Permits presuming to give authority to
violate or cancel the provisions of the building code or other ordinances of the jurisdiction shall not be valid. The issuance of a permit based on
construction documents and other data shall not prevent the building official from requiring correction of errors in the construction docurents and
other data. The building official is also authorized to prevent occupancy or use of a structure where in violation of this code or of any other
ordinances of this jurisdiction.

8. Permit Expiration: All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building
Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that

circumstances beyond the control of the permit holder have prevented action from being taken.

9. Engineering or Architecture: Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in
writing to the Mason County Building Department prior to construction. All engineering and/or architectural documents are a part of the approved
set of plans and shall remain attached thereto. If documents are removed, or changes are made without approval from the architect or engineer
and the Building Official, approval will not be granted, and a re-Inspection fee may be charged by Mason County prior to any further inspections
being performed or approvals granted.

10.Sales Taxes: This work is being done in Mason County. All sales tax for materials delivered to, or work performed in Mason County shall be
sourced to Mason County. Prior to final approval, a completed supplier/sub-contractor list may be requested by Mason County to identify suppliers
that delivered materials to the project site, and sub-contractors who performed work on the site.

Dear Ms. Adkins,
The construction plans for the Mariah Stevens / T-Mobile Cell Tower Antenna
Alterations have been reviewed for compliance with the 2012 International Building
Code as adopted by the State of Washington, as well as any locally adopted
amendments to the codes.
The construction plans have been found to be in substantial compliance with the
adopted codes, and provided the special conditions identified below are incorporated
into the building permit, we recommend approval of the construction documents in
accordance with IBC Section 107.3.1 .
The following special conditions should be made a part of the building permit:
1 . Electrical Permit Required. The electrical system shall be installed under a
separate electrical permit issued by the Washington State Department of Labor &
Industries (L&I). Electrical "final" inspections must be approved by L&I prior to
requesting a building final.
2. Special Inspection: IBC Chapter 17, Section 1704. In addition to the inspections
