Dear Mr.Julien,
Per your request, we have prepared this structural observation letter of the antenna attachments for the
above mentioned project. We performed a site visit on May 31, 2016 to visually observe the antenna
attachment. The site visit was performed by our certified climbers. For our use in preparing this letter,
we were also provided with approved project drawings prepared by B. J.Thomas,P.E.,dated 6/11/2015.
Based on our site visit and our review of the redline drawings, it is our opinion that the new antennas
were installed per the approved plans.

Krazan inspector is on site for new antenna bolt installation and inspection on a 150' monopole with the new
antennas installed at 140'.
Inspector reviewed plans by B.J. Thomas dated 6/11/15, page A2,A2.1,A3, and details. Inspector walked site for
afety inspection, and ascended the monopole to the new antenna height(140') for inspection of the new antennas. Inspector
noted the new Twin PCS EMA(2 per sector), and three new LNX—6515DS-AIM (1 per sector)mounted to the platform
and pipes per details on page A3, with new manufacture supplied hardware. Inspector obtained photos of the new antennas
and hardware.
During inspection inspector noted the Birds Nest Excluder was not in place per plans page A2.1. Inspector descended
he monopole and conducted a post climb site safety inspection.
