Purpose of checklist:
Governmental agencies use this checklist to help determine whether the environmental impacts of
your proposal are significant. This information is also helpful to determine if available avoidance,
minimization or compensatory mitigation measures will address the probable significant impacts
or if an environmental impact statement will be prepared to further analyze the proposal.
Instructions for applicants:
This environmental checklist asks you to describe some basic information about your proposal.
Please answer each question accurately and carefully, to the best of your knowledge. You may
need to consult with an agency specialist or private consultant for some questions. You may use
"not applicable" or "does not apply" only when you can explain why it does not apply and not when
the answer is unknown. You may also attach or incorporate by reference additional studies
reports. Complete and accurate answers to these questions often avoid delays with the SEPA
process as well as later in the decision-making process.
The checklist questions apply to all parts of your proposal. even if you plan to do them over a
period of time or on different parcels of land. Attach any additional information that will help
describe your proposal or its environmental effects. The agency to which you submit this checklist
may ask you to explain your answers or provide additional information reasonably related to
determining if there may be significant adverse impact.
Instructions for Lead Agencies:
Please adjust the format of this template as needed. Additional information may be necessary to
evaluate the existing environment, all interrelated aspects of the proposal and an analysis of
adverse impacts. The checklist is considered the first but not necessarily the only source of
information needed to make an adequate threshold determination. Once a threshold
determination is made, the lead agency is responsible for the completeness and accuracy of the
checklist and other supporting documents.

The Washington Department of Fish and Wildlife has issued a Hydraulic Project Approval (HPA#
2016-6-467+01)for reconstruction of the existing boat ramp, an independent but related project. That
HPA includes provisions related to the protection of fish life and fish habitat.

Mason County has issued an Environmental Permit (MEP 2017-00017), Mitigated Determination of
Nonsignificance (SEP2017-00054), and Shoreline Variance permit(SHR2017-00004) for the Port of
Grapeview Community Floating Dock.

An application(NWS-2018-161) is currently under review by the U.S. Army Corps of Engineers for
construction of a community floating dock to serve the recently reconstructed boat ramp at the Port of
Grapeview property. This community floating dock is located on property less than 100 feet south of
the subject property(Mason County Tax Parcel Number 12105-51-00900).The proposed soft shoreline
stabilization,coupled with the earlier removal of a creosote timber bulkhead at this location comprises
the compensatory mitigation for the potential adverse impacts to result from that dock.

This project consists of mitigation to compensate for the anticipated impacts associated with the
construction and use of the floating dock to the south of the subject property; mitigation has been
divided into Phase I and Phase II. Phase I will include removal of the dysfunctional creosote timber
bulkhead,beach nourishment,beach restoration,and stormwater improvements and was the subject
of a previous application. Phase II will involve shoreline stabilization and is the subject of this

application.
The proposed Phase II Mitigation elements include gentle re-grading of the existing shoreline to
lay back the eroding bank—the final shoreline will have a final grade not to exceed 2:1. The bank
will then be stabilized with biodegradable jute matting and planted with native salt tolerant shrubs
and herbaceous plants. Beach logs with rootwads will be placed along the toe of the slope and
anchored with buried boulders. Additional boulders will be embedded into the beach adjacent to
the beach logs for additional shoreline habitat and to provide additional shoreline protection. Salt
tolerant upper salt marsh vegetation will be planted among the logs and boulders at the toe of the
slope. A 3-foot wide gravel pathway will be provided to the beach. These measures will improve
intertidal habitat for forage fish and juvenile salmonids in order to offset potential impacts to
aquatic habitat from the construction of the new floating community dock on the Port of Grapeview
property.

location of your proposed project, including a street address, if any, and section, township, and
range, if known. If a proposal would occur over a range of area, provide the range or boundaries

required to duplicate maps or detailed plans submitted with any permit applications related to this

The project is located north of 5054 E Grapeview Loop Road, along the shoreline of Case Inlet
(Section 5,Township 21 North,Range 1 West; WM,NE Quarter),at the west end of Griswold Avenue
in the City of Grapeview in Mason County, Washington.
B. ENVIRONMENTAL ELEMENTS hf elpl

shoreline
Upland areas of the mitigation site consists of lawn and sparse ornamental trees and shrubs. The
intertidal beach consists of mud,sand,and small rocks.

Per the Natural Resources Conservation Service(NRCS)soil map,the soil found on this site is classified
as Alderwood gravelly sandy loam.

The shoreline behind the existing creosote timber bulkhead demonstrates active shoreline erosion
hazards,consisting of active toe erosion,sloughing,and shoreline regression(GeoResources,2018).
e. Describe the purpose, type, total area, and approximate quantities and total affected area of
any filling, excavation, and grading proposed. Indicate source of fill. hf elpl
The 63-foot stretch of shoreline,to a width of up to 14 feet, will be gently graded and sloped back to a
slope not to exceed 2:1. This will affect between 20 and 50 cubic yards of material.
f. Could erosion occur as a result of clearing, construction, or use? If so, generally describe.

Active shoreline erosion is observed behind the existing degraded bulkhead (GeoResources, 2018).
While highly degraded,the existing bulkhead may be providing some shoreline protection from the wave
action and removal of the bulkhead may result in some additional shoreline erosion. The purpose of the
soft shoreline stabilization measures is to arrest any future erosion while returning the shoreline to a
more natural condition.
g. About what percent of the site will be covered with impervious surfaces after project
construction (for example, asphalt or buildings)?1hel
The proposed project does not include any changes to impervious surfaces on the site.

Following the bulkhead removal(Phase I of this mitigation project),the Applicant will closely monitor
the shoreline for any substantial increases in shoreline erosion. Sediment and erosion control measures,
include coir logs or sediment curtains will be utilized during project construction to prevent any release
of sediment.

During construction,heavy equipment will give off normal emissions during operation and some dust may
occur. These emissions will be short-term and only when construction equipment is in use. All
construction equipment will be inspected daily for fluid leaks and no equipment will leave the staging area
until any leaks are repaired. Fueling and maintenance of equipment will occur in upland staging areas
only.
b. Are there any off-site sources of emissions or odor that may affect your proposal? If so,

The area upland of the on the subject property is vegetated with lawn grass. Some sea lettuce(Ulva
sp) was found attached within the lower intertidal beach zone. At the location of the existing
stormwater outfall, an area measuring at most 30 square feet is vegetated with small clumps of
pickleweed(Salicornia virginica)and seaside plantain(Plantago maritima).

Following state guidelines for safe disposal of creosote materials will minimize any environmental health
hazards associated with improper disposal of creosote materials.

The Washington State Department of Ecology(WSDOE)database shows a confirmed or suspected
contaminated site (1D# 6309) on the nearby Fair Harbor Marina parcel, due to the presence of
petroleum products in soil and groundwater(from a leaking underground storage tank). However,
this area will not be disturbed during the proposed project. Also, The WSDOE noted the site as
"reported cleaned up"in February 2008.
3) Describe any toxic or hazardous chemicals that might be stored, used, or produced
during the project's development or construction, or at any time during the operating life
